5 Determnaton (To Be Completed By Lead Agency) On the bass of ths ntal evaluaton: o. o o o fnd that the proposed project COULD NOT have a sgnfcant effect on the envronment and a NEGATE DECLARATON wll be prepared. fnd that although the proposed project could have a sgnfcant effect on the envronment there wll not be a sgnfcant effect n ths case because revsons on the project have been made by or agreed to by the project proponent. A MTGATED NEGATE DECLARATON wll be prepared. fnd the proposed project MAY have a sgnfcant effect on the envronment and an ENRONMENTAL REPORT s requred. MPACT fnd the proposed project MAY have a "potentally sgnfcant mpact" or "potentally sgnfcant unless mtgated" mpact on the envronment but at least one effect 1) has been adequately analyzed n an earler document pursuant to applcable legal standards and 2) has been addressed by mtgaton measures based on earler analyss as descrbed on attached sheets. An ENRONMENTAL MPACT REPORT s requred but t must analyze only the effects that reman to be addressed. fnd that although the proposed project could have a sgnfcant effect on the envronment because all potentally sgnfcant effects (a) have been analyzed adequately n an earler ER or NEGATE DECLARATON pursuant to applcable standards and (b) have been avoded or mtgated pursuant to that earler ER or NEGATE DECLARATON ncludng revsons or mtgaton measures that are mposed upon the proposed project nothng further s requred. Cty Plannng Assocate (213) ========================================================.4 Sgnature Ttle Phone Evaluaton Of Envronmental mpacts: 1. A bref explanaton s requred for all answers except "No mpact=answers that are adequately supported by the nformaton sources a lead agency ctes n the parentheses followng each queston. A "No mpact" answer s adequately supported f the referenced nformaton sources show that the mpact smply does not apply to projects lke the one nvolved (e.g. the project falls outsde a fault rupture zone). A "No mpact" answer should be explaned where t s based on project-specfc factors as well as general standards (e.g. the project wll not expose senstve receptors to pollutants based on a project-specfc screenng analyss). 2. All answers must take account of the whole acton nvolved ncludng off-ste as well as on-ste cumulatve as well as project-lever ndrect as well as drect and constructon as well as operatonal mpacts. 3. Once the lead agency has determned that a partcular physcal mpact may occur then the checklst answers must ndcate whether the mpact s potentally sgnfcant less that sgnfcant wth mtgaton or less than sgnfcant. "Potentally Sgnfcant mpact" s approprate f there s substantal evdence that an effect may be sgnfcant. f there are one or more "Potentally Sgnfcant mpact" entres when the determnaton s made an ER s requred. 4. "Negatve Declaraton: Less Than Sgnfcant Wth Mtgaton ncorporated" apples where the ncorporaton of a mtgaton measure has reduced an effect from "Potentally Sgnfcant mpact" to "Less Than Sgnfcant mpact." The lead agency must descrbe the mtgaton measures and brefly explan how they reduce the effect to a less than sgnfcant level (mtgaton measures from "Earler Analyses" as descrbed n (5) below may be cross-referenced). 5. Earler analyses may be used where pursuant to the terng program ER or other CEQA process an effect has been adequately analyzed n an earler ER or negatve declaraton. Secton (c)(3)(d). n ths case a bref dscusson should dentfy the followng: a. Earler Analyss Used. dentfy and state where they are avalable for revew. b. mpacts Adequately Addressed. dentfy whch effects from the above checklst were wthn the scope of and adequately analyzed n an earler document pursuant to applcable legal standards and state whether such effects were addressed by mtgaton measures based on the earler analyss. c. Mtgaton Measures. For effects that are "Less than Sgnfcant wth Mtgaton Measures ncorporated" descrbe the mtgaton measures whch were ncorporated or refned from the earler document and the extent to whch they address ste-specfc condtons for the project. = EN MND Page 5 of 16

6 6. Lead agences are encouraged to ncorporate nto the checklst references to nformaton sources for potental mpacts (e.g. general plans zonng ordnances). Reference to a prevously prepared or outsde document should where approprate nclude a reference to the page or pages where the statement s substantated. 7. Supportng nformaton Sources: A sources lst should be attached and other sources used or ndvduals contacted should be cted n the dscusson. 8. Ths s only a suggested form and lead agences are free to use dfferent formats; however lead agences should normally address the questons from ths checklst that are relevant to a project's envronmental effects n whatever format s selected. 9. The explanaton of each ssue should dentfy: a. The sgnfcance crtera or threshold f any used to evaluate each queston; and b. The mtgaton measure dentfed f any to reduce the mpact to less than sgnfcance. EN-Z013-Z366-MND Page 6 of 16

13 DSCUSSON OF THE ENRONMENTAL EALUATON (Attach addtonal sheetsf necessary) The Envronmental mpact Assessment ncludes the use of offcal Cty of Los Angeles and other government source reference materals related to varous envronmental mpact categores (e.g. Hydrology Ar Qualty Bology Cultural Resources etc.). The State of Calforna Department of Conservaton Dvson of Mnes and Geology - Sesmc Hazard Maps and reports are used to dentfy potental future sgnfcant sesmc events; ncludng probable magntudes lquefacton and landslde hazards. Based on applcant nformaton provded n the Master Land Use Applcaton and Envronmental Assessment Form mpact evaluatons were based on stated facts contaned theren ncludng but not lmted to reference materals ndcated above feld nvestgaton of the project ste and any other relable reference materals known at the tme. Project specfc mpacts were evaluated based on all relevant facts ndcated n the Envronmental Assessment Form and expressed through the applcant's project descrpton and supportve materals. Both the ntal Study Checklst and Checklst Explanatons n conjuncton wth the Cty of Los Angeles's Adopted Thresholds Gude and CEQA Gudelnes were used to reach reasonable conclusons on envronmental mpacts as mandated under the Calforna Envronmental Qualty Act (CEQA). The project as dentfed n the project descrpton may cause potentally sgnfcant mpacts on the envronment wthout mtgaton. Therefore ths envronmental analyss concludes that a Mtgated Negatve Declaraton shall be ssued to avod and mtgate all potental adverse mpacts on the envronment by the mposton of mtgaton measures and/or condtons contaned and expressed n ths document; the envronmental case fle known asen mnd and the assocated case(s) APCS ZC-CDO. Fnally based on the fact that these mpacts can be feasbly mtgated to less than sgnfcant and based on the fndngs and thresholds for Mandatory Fndngs of Sgnfcance as descrbed n the Calforna Envronmental Qualty Act secton the overall project mpact(s) on the envronment (after mtgaton) wll not: Substantally degrade envronmental qualty. Substantally reduce fsh or wldlfe habtat. Cause a fsh or wldlfe habtat to drop below self sustanng levels. Threaten to elmnate a plant or anmal communty. Reduce number or restrct range of a rare threatened or endangered speces. Elmnate mportant examples of major perods of Calforna hstory or prehstory. Acheve short-term goals to the dsadvantage of long-term goals. Result n envronmental effects that are ndvdually lmted but cumulatvely consderable. Result n envronmental effects that wll cause substantal adverse effects on human bengs.
