3 Page 3 of 28 Water User Manual 1.Application for water supply and water meter installation 1.1 Obtain the application form and the service rate for water meter installation at Amata Water Co., Ltd. Amata Nakorn Industrial Estate at Amata Service Center Building 2 nd Floor, Tel ext 782 Fax: or download applications from Amata Nakorn Industrial Estate and Amata City Industrial Estate (General Zone) (See details as appendix no.1,page 7) Amata Nakorn Industrial Estate and Amata City Industrial Estate (Adjacent with Canal Zone) (See appendix no.1,pages 7) 1.2 Submit the application form to Co., Ltd together with the following documents: Power of Attorney Letter issued from the landlord of the premise Affidavit issued by the company s applicant Copies of identification card or passport of the authorizer and the authorized person Topographical map and master plan of the location and area of the user s premise in the industrial estate Site plan showing the spot on which the water meter will be installed. 1.3 Submit Application form at Amta Water Co.,Ltd.office.or Amata City office. Application fee and documents will be submitted on the application date. 1.4 Payment for water meter, pipes, related equipment and installation fee will be made on the application date.(see details in appendix 6, pages 12), if the payment was paid after the 25 th date of the month, the date in the receipt would change to the date of next month. 1.5 For contractors or plant operators (who is not a land owner), a deposit of 53,500 Baht for water use (VAT included) will be placed on the application date. Note: The deposit money for water use will be returned when the water meter is cancelled and the outstanding balance is fully paid. 1.6 After completing No , the installation will be arranged and completed within days.(depend on size of meter) 1.7 Examination & acceptance of the account and settling the starting value of the meter agreed by the company and the user (see details in appendix no.2, page8) 1.8 Ask for an application form as follows offices: Amata Nakorn I.E. at Amata Service Center Building Tel: ext 782 Amata City I.E. at Amata City Office Tel: Or download application form at Coordinating officers for installation and examination: Amata Nakorn Industrial Estate Tel , , , Amata City Industrial Estate Tel , ,

4 Page 4 of The company will conduct the meter testing and calibration service as appropriate The company will charge for the water meter maintenance fee on a monthly basis (see appendix no.12 pages 22) Remarks: The company reserved the right to fix water meter specification and water meter installation including change of water meter price without prior notice. 2.Cancellation of water supply and return of deposit 2.1) The user can fill out the water supply cancellation and return of deposit form (see appendix no.3,page 9) 2.2) The company will proceed the closing of the valve, read the meter and record the value with the user as indicated in the form (see appendix no.2, page 8) and remove the meter to return to the user. 2.3) After the outstanding balance of the user s water bill and remaining debts to the company is fully paid, the deposit money will be returned (within 30 days after the cancellation of water supply as indicated in 2.2) 3.Water rights transfer/change 3.1) The user can fill out an application for change of water right form (see appendix no.4 ) 3.2) The company will conduct the check, reading and recording the number of the meter with the user as indicated in the form. (see appendix no.4,page 10) 3.3) The company will authorize the water right to the new user after completing the check of water bill receipt and outstanding balance. 4. Charge for water meter & related facilities damage 4.1) The company reserved the right to remedy/fix water meter and/or related facilities immediately. 4.2) Any expenses concerned from 5.1 shall be paid by the water user such as fixing pipe expense, water loss etc. 4.3) Details of fixing expenses as shown in appendix no.8,pages Meter reading and volume check 5.1) Meter reading and the amount of water use will be conducted on the 25 th of every month.. 5.2) Charges for the amount of water consumed each month are based on the number of units you use during a billing period starting from the 25 th of the previous month to the 24 th of the current month. For example, the water bill for November 2008 is based on the water use

5 Page 5 of 28 during October 25 th November 24 th, 2008, 31 days in total. The water bill for December 2008 is based on the water use during November 25 th -December 24 th, 2008, 3:0 days in total. 5.3) Water rates are separate in to 2 parts which are water rate for Customer and Contractor; 5.3.1) For Customer Water rate within quotas according to Land Purchase & Sales Agreement Water meter maintenance monthly fee (see rate of monthly fee as in appendix no.12,pages 22) Wastewater treatment charge shall be collected according to Announcement no.สน.อน.001/2555 for Amata Nakorn I.E. and Announcement no. สน.อต. 001/2555 for Amata City I.E. and related announcement stipulated by Industrial Estate Authority of Thailand (IEAT) (as if) 5.3.2) For Contractor Water rate at 40 baht/cu.m. (No wastewater treatment charge) 6.Water billing and infrastructure service payment 6.1) Meter reading and recording the number of the meter will be conducted on the 25 th of every month. 6.2) Invoices/tax invoices will be dated on 1 st -5 th of the next month 6.3) the issuance of authenticated invoices/tax invoices to customers will follow these conditions: 6.3.1) For the mailing addresses in Amata Nakorn and Amata City Industrial Estates, the invoice/tax invoice will be delivered within 5 working days from the date indicated on the invoice ) For the mailing addresses outside Amata Nakorn and Amata City Industrial Estates, the invoice/tax invoice will be delivered by post. 6.4) Service payment 6.4.1) Payment can be made in cash or by crossed check made payable to Co., Ltd with the following options: Pay in person at the office of Co., Ltd. on 2 nd Floor, Amata Service Center Building, Amata Nakorn Industrial Estate Money transfer to a bank account of Co., Ltd : o Kasikorn Bank Public Company Limited, New Petchburi Branch, Savings account o Kasikorn Bank Public Company Limited, Bangpakong Industrial Estate Branch, Savings account o Bangkok Bank Public Company Limited, Amata City Industrial Estate, Savings account The company will send you the receipt by post within 7 days after receiving the payment slip 6.4.2) The payment must be made within 20 days from the date indicated on the invoice. If the invoice is overdue, the company reserves the right to discontinue the water supply in compliance with the regulation of the Industrial Estate of Thailand. The company will resume the service as soon as the invoice is paid including 15% interest

6 Page 6 of 28 rate of the outstanding balance starting from the due date along with 10 % meter reactivation fee (incase of temporary water cut) of the re-installation fee of water supply based on the size of the meter ) As for the name of withholding tax agent, use Co., Ltd. 700/2 Moo 1, Klong Tumru, Amphur Muang, Chonburi 20000, tax identification number Note: Meter installation fee and deposit of water use can be paid during 1 st -24 th of every month only. 7. Steel Sleep Pipe model to protect central water meter pipe inside the Industrial Estate. 7.1 In case of frontage entrance of water user passing water meter pipe of the Industrial Estate. The water user has to use steel sleep pipe model according to design and size specification as shown in (appendix no.11,pages 20) 7.2 The company reserved the right to not allow water user construct frontage entrance passing manhole and/or changing position of central facilities such as factory connection or fire hydrant etc. 8. Announcements and Related Regulations 8.1 Announcement of the Industrial Estate of Thailand No.สน.อน.001/2555 for Amata Nakorn I.E. and Announcement of the Industrial Estate of Thailand No.สน.อต.001/2555 for Amata City I.E.
