2 Park Place Water Resources 2015 Projects = Project Completion Quarterly BacT and monthly usage reported to DNR (all testing satisfactory) Three-year lead & copper (all testing satisfactory) Published CCR (presented with 2012 test results) o CTW will use current data going forward Successful Three-Year DNR audit (Sept, 2015) o No Significant Deficiencies outcome, versus one filed in 2012 o No Deficiencies outcome, versus six filed in 2012 o Recommendation(s): gazebo set-back Park Place Emergency Response Plan Compliance: Updated and submitted to DNR; accepted/filed/posted Updated DNR site plan with final list of water test sites filed (with corrected addresses) Facility #1 repairs o new vertical pump and shaft (Jan, 2015) o pump oil change - May, 2015 (annual requirement) o new tank compressor, o new discharge air/vacuum relief valve, o new tank (air-line) check valve, o address longstanding roof leak problems (and related collateral damage): install metal diverter above roof access cap; reinforce rotted structural members; replace insulation, adjacent ceiling drywall and overhead access trim; add new doors, anti-rodent screening, shutters, new locks/keys with handle o new dehumidifier o prepare #1 for 2016 centralized water-sample collection o reduced (not eliminated) well-head area wetness: relief valve, line valve extension, redirect RV termination, install new PVC well-head drainage line o attempt to reprogramming problematic/unreliable Sensaphone monitoring (efforts failed) o hornet infestation elimination (annual Spring treatment now in place) o space heater replaced o new Badger Meter water-volume meter installed o chlorination tank markings added, per DNR o unclog/re-open floor drain outlet to the east of #1 (temporary fix) o thick-wall PVC jacket added to air/vacuum discharge receiving pipe, stop water splashing/spewing near well-head Facility #2 repairs o floor cracks filled o new poured 6 code-compliant concrete well-head collar o well-head flange bolts installed o space heater replaced o hornet infestation elimination (annual Spring treatment now in place ) o system flush (Sep, 2015) Hydrants o system flushed (summer) o checked: nine rated with abnormal torque; two hydrants repaired (top valve-nut) o note: torque-rating assessment (data collection) will be made in 2016, during Spring/Summer flushing, to ascertain problem-severity and required repairs Bruce Sipiora, Park Place Water Resources Status Report, Q-1,

3 = Project Completion Park Place Water Resources 2016 Projects Monthly bacterial water testing, with annual nitrate [#1 & #2 sites] required in 2016 On-Going o bac-t monthly testing is now DNR mandate, beginning 2016 and going forward o CTW: $170/monthly sample-collection charge with system check (was $190) Diagnose and repair chronic #1 well-head leaks High Priority o concrete vertical-pump pad and surrounding floor chronic wetness o goal: 99% dry state complete New well-head outbound check valve, o eliminated water backflow into well o elimination of floor wetness surrounding well-head base with added capped base-fitting o significantly reduced audible clanging sound upon pump-cycle completion Mandatory #1 Facility tank cleaning (June, 2016), includes replacing faulty (outbound/inbound) tank butterflyvalve o cleaning is DNR-required every 5-years; 2011 tank cleaning cost was $2000 o butterfly valve purchased previously, presently in storage in #1 and ready for deployment; install during cleaning o two options for tank cleaning 1. auxiliary tank set-up (preferred); cost: ~$4,000 (recommended) 2. at #2, employ continuous-discharge and high duty-cycle with excess routed into pond; cost: ~$3,000 o using auxiliary tank, 3-days to complete; two DNR water samples required, analyzed 24-hours apart o off-street tank placement near hydrant #10 at north end of Waterleaf Drive (see map), with night markers o other maintenance operations, for example, tank sight glasses cleaning or replace New web-based (cloud) safety monitoring systems, with mobile, and text-alert functionality, for both #1 and #2 High Priority o measure all key operational parameters of entire system, with peer-to-peer alerts o employ Sensaphone model Sentinel Cloud-Based Monitoring with wireless (cellular) communication link-up and battery back-up ambient temperature system pressure (#1) high water --- low air in tank (#1) power loss (110 and 3-phase) flooding (spot water detection) databased water usage (volume) interface with updated, digitally-enabled Badger flowmeter CO 2 levels eliminate unnecessary/outdated AT&T phone line o upgrade existing sensors reliability unknown configure with future-testing functionality (ability to test the tester -- very important) o total upgrade cost ~$5,500, includes all hardware, installation and alerts set-up (combined Fliteway & CTW charges) $600 annual communications fee (Verizon cellular) versus AT&T phone line (at $500 annually) Bruce Sipiora, Park Place Water Resources Status Report, Q-1,

Install Drainage Line (a french drain ) for #1 Facility Floor Drain High Priority o install ~30-feet of 4-inch perforated line, buried ~12 underground with 2 of crushed stone o clean water discharge percolates into soil, with minimal open-outlet termination (anti-rodent screen added) o route percolation line directly to east of building o eliminate on-going drain clogging (or line freezing), with potential discharge back-up into #1 o estimated cost: <$1,400 o Spring, 2016 o notify homeowners, north of #1 #1 Well-Head CO 2 Venting o safety hazard; remediation action required o extend existing vent pipe directly into attic area o #1 attic space is currently well-ventilated o modest project involving minimal cost o post No-Entry sign above #1 entrance doors Critical well-system components failure-planning and response o pump replaced o shaft replaced o well-head relief valve, tank compressor, tank-and-air-line check valve relief valve replaced compressor replaced 12/2015 o essential tank-and-air-line check valve replaced Develop condensed water operations reference guide for future water-resource oversight o Work-in-process Hydrants & Data Compilation o exercise hydrant s-internal shut-off valve all o exercise adjacent street shut-off valve all o flush system (Summer, coordinate this operation with tank cleaning, as both require system flushing) o measure all hydrants for abnormal torque o develop data-base to compare and annually track o rebuild any deemed in high need o revisit low/no drain issue on select hydrants (an issue of residual water potentially freezing with internal damage) Ensure all homes have hose-bib vacuum breakers Medium Priority o report-back mechanism goal, before June HOA meeting Improve service logs and maintenance/record-keeping o CTW recording all work performed on permanent clipboard posted in #1 Bi-annual #2 well flush o discharge for 45 minutes into pond o maintain system freshness Bruce Sipiora, Park Place Water Resources Status Report, Q-1,
