2 Foreword This Sustainable Development Management Plan has been developed in response to the NHS Carbon Reduction Strategy (2009) which reinforced the urgent need for all sections of the NHS to take action to reduce carbon emissions arising from their operations and to embed sustainability throughout the organisation. We recognise the impact that NHS Northamptonshire operations have on the environment as well as the strong link between sustainability and the health of the public. We are committed to demonstrating leadership in sustainability and this comprehensive Management Plan represents a route map for us to deliver significant improvements, with the help of all our staff, key partners and other stakeholders. Executive Summary The urgent need for organisations to reduce carbon emissions and embed sustainability in their operations is particularly important in the NHS given its significant contribution to England s overall emissions. As well as reinforcing the link between sustainability and public health a clear focus on sustainable development will enable NHS Northamptonshire (the Trust) to capture a range of financial and nonfinancial benefits as well as a reduced carbon footprint. The NHS Carbon Reduction Strategy (2009) for England has provided a framework for the Trust to follow which has responded by producing a Sustainable Development Management Plan. This focuses on key priorities, being: environmental legislation governance organisational and workforce development partnerships finance energy and carbon management water and waste management travel and transport design and operation of buildings The development of the Management Plan demonstrates the Trust s commitment to carbon reduction through a range of practical but ambitious measures, sharing of good practice and active engagement and support of its staff. The Action Plans within the Management Plan include the following: The embedding of sustainability within the Trust s policies and procedures and reinforcement of Board level commitment and responsibility Enhanced data management relating to energy, waste and water and the development of a robust carbon footprint. The development of a communications strategy to ensure the effective implementation of the Plan cascades throughout the Trust. The development and establishment of Partnerships with key stakeholders, through Local Strategic Partnership and others NHSN Sustainable Development Management Plan Page 4 of 31

5 The development of a sustainable procurement strategy with the Trust s procurement managers. Identification of opportunities to reduce the Trust s carbon emissions in particular through energy, transport and procurement management 1.1 Carbon Management and NHS Northamptonshire the need for action The UK Government has therefore introduced a range of legislation and other drivers to require and encourage public sector organisations to take action to reduce their carbon impact. The Climate Change Act 2008 introduced for the first time statutory targets to reduce the UK s carbon emissions by 80% by 2050, while the NHS Carbon Reduction Strategy for England¹ requires NHS organisations to develop a Sustainable Development Management Plan identifying a clear strategy for tackling these significant challenges. This Strategy reinforces six key reasons for action: Legally binding Government framework and national targets The strength of scientific evidence to act now on climate change The health co-benefits now for patients, population and the NHS Cost reduction and energy resilience The willingness and commitment of NHS organisations to act now The need for the NHS to be a leading public sector exemplar The NHS Carbon Reduction Strategy highlights that its vision is to deliver a low carbon quality healthcare environment that is sustainable, resilient and safeguards high quality patient care. NHS Northamptonshire is committed to the commissioning and delivery of high quality and sustainable health care and to work with key partners to enable the aims and aspirations of the Strategy to be achieved. It recognises that major challenges lie ahead but there are also significant opportunities. The trust spends 900 million a year on the commissioning and delivery of health care services. It has an opportunity to set an example to other NHS organisations as well as the wider community. This Sustainable Development Management Plan sets out NHS Northamptonshire s goals and targets as well as a clear practical, realistic but ambitious set of Action Plans and the Environmental Policy in Appendix 1 sets out the Trust s commitment and priorities. ¹ NHS Carbon Reduction Strategy for England, January 2009, NHS Sustainable Development Unit NHSN Sustainable Development Management Plan Page 5 of 31

6 2. Partnership is the key Establishing and developing key partnerships is a vital element of NHS Northamptonshire s approach to Carbon Management. Partners include the Department of Health, NHS Shared Business Services, other NHS organisations and local or regional public and private sector bodies through the Local Strategic Partnership (see Figure 1). Examples of good practice need to be reviewed and replicated within the Trust if suitable, while practical and effective solutions will be developed in conjunction with the key partners. This will include learning from the Department of Health, Health Protection Agency and other PCT s. The Trust operates as a diverse organisation with community hospitals, health centres, treatment centres, offices and staff bases among its estate portfolio. Many are freehold while others are leasehold properties throughout Northamptonshire. As such, some of its carbon impacts relate directly to its operations, and may be controllable, while others are indirect and it may or may not be able to easily influence. Liaison with partners who may for example be the PCT s landlords (health service or otherwise) or tenants is therefore essential. NHSN Sustainable Development Management Plan Page 6 of 31

7 Other Other PCT s/nhs Organisations Department of Health NHS Suppliers Providers Employees Local Authority Employees Voluntary Sector Local Strategic Partnerships Figure 1: Some key NHS Northamptonshire partners The split of the PCT in April 2009 into the commissioning body, NHS Northamptonshire, and Provider Services, NHS Northamptonshire Health care, means that two organisations will exist to commission and service much of Northamptonshire s health needs. The Trust will ensure that sustainability will be embedded across both organisations and delivered in the most effective way as a close partnership. The most important partners are the employees of the Trust. Their enthusiasm and commitment for Carbon Management is essential if the PCT is to deliver the opportunities available. As a result engagement and communication with and involvement of employees will be a key element of the Management Plan. As Europe s biggest employer, the NHS has a big opportunity to have exemplar employees who are likely to have far reaching positive impacts, not only on the NHS supply chain, but also on communities throughout the UK. ² ² Regional Development Agencies see NHS Carbon Reduction Strategy for England NHSN Sustainable Development Management Plan Page 7 of 31

8 2.1 What is Sustainable Development? Sustainable development considers how we can live today without causing irreversible change that will threaten the lives and health of future generations development that meets the needs of the present without compromising the ability of future generations to meet their own needs Brundtland Report. Sustaining a modern health service therefore requires a strong link between the three tiers of sustainable development Economics, Environment and Society. Underpinning the UK Government s commitment to sustainable development are five guiding principles, as Sustainability Economy Society Environment NHSN Sustainable Development Management Plan Page 8 of 31

9 Living Within Environmental Limits Respecting the limits of the planet s environmental resources and biodiversity to improve our environment and ensure that the natural resources needed for life are unimpaired and remain so for future generations Ensuring a Strong, Healthy & Just Society Meeting the diverse needs of all people in existing and future communities, promoting personal well-being, social cohesion and inclusion, and creating equal opportunity for all. Achieving a Sustainable Economy Building a strong, stable and sustainable economy which provides prosperity and opportunities for all, and in which environmental and social costs fall on those who impose them (Polluter Pays), And efficient resource use is incentivised. Using Sound Science Responsibly Ensuring policy is developed and implemented on the basis of strong scientific evidence, whilst taking into account scientific uncertainty (through the Precautionary Principle) as well as public attitudes and values. Promoting Good Governance Actively promoting effective, participative systems of governance in all levels of society engaging people s creativity, energy, and diversity. Figure 3: Sustainable Development UK Government Principles³ The link between sustainability and health is reinforced in these principles and backed up further by the World Health Organisation which defines health as: a state of physical, mental, and social well-being, and not merely the absence of disease or infirmity. ⁴ Public health is inevitably affected by a range of environmental factors, including water, sanitation, environmental conditions, food, housing, as well as the provision of health care services⁵. The effect of climate change on health is well documented⁶ and includes the potential for increasing deaths and illness/accidents due to natural disasters, higher temperatures, flooding and infectious disease. The NHS recognises this link and its role in embedding sustainable development which it refers as good corporate citizenship. Embedding sustainable development into NHS operations brings with it a virtuous circle, of potentially improved health and reduced demand for its services, as detailed in Figure 4. ³ DEFRA, UK Government s 5 guiding principles on sustainable development ⁴ World Health Organisation, NHSN Sustainable Development Management Plan Page 9 of 31

This requires a clear focus on protecting the environment and reducing carbon emissions arising from its operations. NHS resources which influence levels of demand for health services which impacts on the health of the local population which can help reduce the NHS s capacity to provide quality services which affects local economic, social and environmental conditions which impact on the health of the local population, which can help reduce. levels of demand for health services which impacts on Figure 4: Virtuous circle the link between sustainability and demands on health services 2.2 Carbon Management in practice There is carbon in everything, such as the energy we use, the fuel we buy to run vehicles, the goods we purchase or the waste we dispose of. An effective approach to carbon management will focus on those areas which the PCT can control or significantly influence, prioritised according to their carbon impacts. NHS Northamptonshire s approach to embedding sustainability in its operations is to therefore focus on carbon emissions arising from its suppliers and the wider community. In order to deliver this it has identified the objectives set out in Figure 5. NHSN Sustainable Development Management Plan Page 10 of 31

11 Responsibility for carbon management will be taken by the Sustainability lead within the NHS Northamptonshire Board to ensure sufficient commitment and support. This lead is the Director of Operations & Policy. As the NHS Carbon Reduction Strategy confirms, the carbon footprint of the NHS amounts to 18 million tones CO₂ every year. Of this 60% arises from the procurement of materials (much of this from pharmaceuticals), 22% from NHS buildings and 18% from travel - Binley s NHS Guide (2009). These issues are also key to NHS Northamptonshire s Management Plan as well as working with key partners, engaging employees, ensuring the PCT becomes carbon literate while embedding sustainability into its core strategy. Underpinning the Plan is a set of targets what will be modified as the Plan evolves and the NHS develops appropriate metrics. Embedding sustainability into the Trust s strategy and operations touches on everything that the Trust does and effective implementation of the Plan will seek to address systems, IT, communications and organisational issues. The Trust will review the use of a structured system, such as an Environmental Management Systems, as a potential way to effectively deliver the required outputs. NHS Northamptonshire has undertaken a number of measures in the past which focused on carbon reduction, directly of indirectly. These include: Enhanced data management relating to energy, waste and water Improvements in clinical waste and general waste management and contracts which have resulted so far of savings Participation in local NHS sustainability and other forums Plans to develop sustainability across the Trust as reported in Use of Resources returns. It is however recognised that there are significant potential opportunities to improve sustainability within the NHS Northamptonshire and as result key areas that the NHS Northamptonshire Sustainable Development Management Plan focuses on are: NHSN Sustainable Development Management Plan Page 12 of 31

This requires a culture that embraces sustainability as a necessary and positive outcome and the Action Plans later in this report set out details measures necessary to effectively implement the strategy. 3. NHS Northamptonshire s Carbon Impact NHS Northamptonshire is committed to reducing its carbon emissions through development of a sound carbon management programme, the basis of which will be the calculation of a carbon footprint of its activities. The carbon footprint will establish a baseline and enable the trust to set benchmarks for future development. Data relating to energy, waste and travel is reported in the annual ERIC returns to the Department of Health, while some of this is routinely recorded by Estates Managers. NHS Northamptonshire will work with staff and suppliers to record and monitor the data required to accurately calculate footprint going forward. This includes information on the energy data for buildings including electricity and gas consumption, waste produced, mileage records for business transport, patient and visitor movements and staff commuting. NHS Northamptonshire has embedded the targets identified by the NHS Sustainable Development Unit (SDU) and aims to reduce the emissions arising from its operations by at least 10% by Arising from the Climate Change Act 2008 progressive targets have been set for UK emissions; 34% reduction by 2010 and 89% by NHS Northamptonshire sees its target to reduce by 10% by 2015 as the first step in a challenging goal of significantly reducing long-term emissions and will seek to follow NHSN Sustainable Development Management Plan Page 13 of 31

14 or where possible improve on NHS guidelines which will set targets for 2020, 2030 and The benefits of Carbon management As indicated earlier there are a number of reasons why NHS Northamptonshire is taking action to reduce its carbon emissions, in addition to contributing to the UK Government s push to reduce climate change impacts. While a major driver for the motivation behind the implementation of the Plan is the need to meet the NHS targets there are a range of other reasons why the Trust is focusing on carbon management. Appendix 2 focuses on key issues relating to the Business case for Sustainable Development. 3.2 Financial resource savings It is inevitable that some savings will arise from zero or low cost opportunities. Indeed, research by the Institute for Environmental Management & Assessment (IEMA) shows that a 1% (of total energy spend) investment in energy awareness programmes and training can produce a 5-20% decrease in energy costs. In addition, improved regular monitoring and profiling of data (at little cost) can highlight opportunities for improvement in performance and generate significant financial savings. Some energy savings may only arise following capital investment in equipment, which would be subject to a full analysis of costs against benefits. In addition to potential savings that may arise from a focus on energy, waste or indeed water issues opportunities arising from transport management, such as logistics, video-conferencing, car-sharing schemes will be considered. 3.3 Non-financial, or non-quantifiable, benefits In addition to the direct financial savings likely to be achieved there are a number of other avoided costs or potential non-financial added values. These include: Reinforcement of the strong link between the Trust s role as a commissioner and provider of health services and sustainability. The link between health and sustainability has been made earlier and the Trust has a significant role in demonstrating this link. Avoided costs Avoidance of financial and other penalties for environmental non-compliance. There is an extensive range of environmental legislation already in place and this is increasing together with a new focus on carbon management through additional legislation. 15 Satisfaction of outside pressures The Trust already is and will be under increasing pressure to contribute to NHS and wider targets such as the UK Government s push for sustainable procurement. A fully integrated and embedded plan will enable the Trust to demonstrate compliance while encouraging others and capturing further opportunities. League tables Failure to deliver on the NHS targets and other requirements may lead to increased scrutiny by key stakeholders and identification of poor performers. Reputational added value Increasingly, reputation is being linked to the sustainability of an organisation. The Trust s reputation is directly linked to its sustainability credentials given the link between this and public health. Improved staff satisfaction A number of studies have identified a correlation between a focus on sustainability and staff satisfaction (particularly where staff are fully involved) and this can lead to improved productivity. Improved engagement with key stakeholders Key stakeholders including the local community of the Trust are increasingly focusing on sustainability. The Trust s engagement and enhanced commitment and leadership with this agenda will therefore improve its relationship with them. It will seek to become an exemplar of good practice and so engage others in making a positive contribution to sustainable development. Carbon and other reduction Although some measures may not result in direct financial savings there is likely to be the opportunity to identify carbon or other savings, which will therefore be able to contribute to the NHS targets. Taking Action Sustainable Development Management Plan 4.1 ACTION PLANS The following Action Plans reflect the focus of this Management Plan and are intended to ensure that the Trust is operating in accordance with relevant environmental legislation as well as embedded sustainability throughout its operations. The measurement, performance and relevance of these Action Plans will be reviewed regularly to ensure they are achieving the necessary objectives Compliance with environmental legislation Commitment: NHS Northamptonshire is committed to complying with all relevant UK environmental legislation and other environmental requirements. 27 Sustainable Development Management Plan APPENDIX 1 NHS Northamptonshire Environmental Policy Statement Environmental Policy NHS Northamptonshire is responsible for the commissioning and delivery of health care across most of Northamptonshire. The aim of the Trust is to provide high quality sustainable health care in this region and it is committed to embedding sustainability into its operations and to encourage key partners and stakeholders to do the same. The Trust recognises that good maintenance and care of the environment contributes a great deal to the long term health of people, their social wellbeing and economic prosperity. The Trust is therefore committed to the care of the environment, prevention of pollution and to continually improve the quality of their services and environmental performance. This is delivered through legislation and other requirements The Trust will seek to implement the principles of sustainable development within the following specific areas: Energy The Trust is committed to minimising its consumption of non-renewable energy sources and to continually research alternative sources and technology to reduce consumption of energy across all activities. Waste The Trust works to minimise the waste it generates and their impacts by seeking opportunities to reduce, reuse, recycle and recover otherwise redundant resources. The Trust works to manage its clinical and hazardous waste is as safe a manner as possible and to minimise its environmental impacts Transport The Trust is aware of the environmental impacts of their transport operations and will develop a comprehensive management plan to focus on reducing the environmental impact of transport, including unnecessary travel Training The Trust is committed to the ongoing training and education of their staff so that is able to deliver its services in the most effective and sustainable manner possible NHSN Sustainable Development Management Plan Page 27 of 31

28 Procurement The Trust works with procurement managers and suppliers to ensure that the products, services and materials which it purchases are as sustainable as practical. The Trust has action plan that will set, seek to achieve and review measureable sustainable development objectives in accordance with the NHS national strategy. The Trust Board will provide appropriate resources as required in order to enable the company to meet these objectives. All staff are made aware of the aims and objectives of this policy and Sustainable Development Management Plan Sustainable Communities Strategy for Northamptonshire through notice boards, staff induction, intranet and training and are expected to adhere to these requirements. Everyone is encouraged to suggest ways in which the Plan can be improved. Where appropriate sub-contractors and visitors to the Trust s premises are required to acknowledge and adhere to the company s Sustainability Policy and relevant operating procedures. 29 Sustainable Development Management Plan APPENDIX 2 The business case for Sustainable Development Management Plan As indicated in the body of the SDMP there are a number of reasons why it makes sense for any organisation to focus on sustainable development. While it should not be forgotten that the Trust faces mandatory carbon reduction targets reasons for this focus include financial the SDMP highlights the potential savings resulting from meeting the As indicated in the body of the SDMP there are a number of reasons why it makes sense initial NHS target as well as reputational and other non-financial. Financial savings will result from either: No cost or low cost solutions these are likely to arise from the raising of awareness among Trust staff, travel coordination and simple measures such as improved monitoring. Typically, up to 40% of opportunities for savings may be no cost or low cost. Opportunities that require capital investment these will vary in payback and scale and may include a roll out of automatic metering, new lighting systems, waste contracts, heating systems etc. Each opportunity must be considered on its merits but must also be priorities as part of a focused carbon management strategy. The Trust faces increased scrutiny and also has a significant opportunity to act as an exemplar to other organisations and the public which it serves. Carbon Reduction Commitment (CRC) In 2010 Department for Environment Food and Rural Affairs (DEFRA) Carbon Reduction Commitment will come into effect. This scheme will engage major energy users who have an annual electricity consumption greater than 6,000MWh during the baseline year of The CRC will be a cap and trade scheme based around an organisation s total emission (excluding transport). Any emissions covered by the European Union Emission Trading System (EU ETS) or Carbon Credit Allowance (CCA) will be excluded. The impact upon the Health Protection Agency (HPA) will be that each year an allowance will need to be purchased. At the end of the emissions year the participant will then receive a payment back based upon their yearly performance. The performance will be based upon the position of the organisation in a league table. For the early years the difference in rebates/penalties will be as shown in Figure 11 below. Subsequent years will see an increase in the difference between the top performing and worst performing. NHSN Sustainable Development Management Plan Page 29 of 31

The purchase and recycling of credits is still yet to be finalised by DEFRA although it has been proposed that the approach detailed overleaf be adopted.
