2 INTRODUCTION In December 2014, The Vertex Companies, Inc. (VERTEX) was retained by Dollar Tree Stores, Inc. to coordinate an Identification Survey within the designated retail spaces (i.e. Units 4758, , 4764, 4768 and 4770) of the Gulf Harbor Plaza Shopping Center located on US Highway 19 in New Port Richey, Florida. The purpose of this survey was to facilitate the planned build-out of the spaces (renovation) which will require disturbance of existing building materials and not the demolition of the structures. The inspection was limited to a visual inspection of the aforementioned spaces to identify the presence, locations and quantity of Containing Materials (ACM) or Presumed Containing Materials (PACM) in accordance with OSHA s Construction Standard 29 CFR Bulk samples were collected as part of the survey to confirm/dismiss the presence of asbestos within the suspect materials. This survey was performed on December 18, 2014 by Mike Williams (NV5), Licensed Building Inspector, performed under the direction of Steven Lipson, a Licensed State of Florida Consultant (#AX-43). Restrictions and limitations of VERTEX s services detailed in this report are not all inclusive and only attempt to identify existing conditions which must be evaluated prior to any renovation. Renovation operations which impact previously concealed materials must be controlled and halted should suspect ACM be identified. Quantities of ACM/PACM detailed herein are approximations and to be utilized for informational purposes only. This survey report does not represent nor should it be utilized for a bid specification for asbestos abatement. RESULTS Results indicate that the following asbestos containing materials were identified: Approximately 1,200 square feet of mastic associated with non asbestos 12 floor tile located throughout Space Note: No ACMs were identified within the materials sampled of the remaining spaces inspected (i.e. Spaces 4770, 4768, , and 4758).

3 RESTRICTIONS Restrictions to VERTEX s Identification Survey include: The survey was limited to exposed building materials only. Destructive sampling was not performed to identify or confirm suspect materials within concealed areas. Note: Black mastic was identified as ACM in Space 4764 only. If residual black mastic is visually observed in other areas during future renovation activities in presently concealed locations, then the material should be treated as PACM. No inspection/sampling was performed inside enclosed ceilings/walls, inside equipment, on stored materials or on other permanent building structures. No inspection/sampling was performed on roofing materials or exterior (voiding of warranties). No inspection/sampling was performed below finished grade. Note: Suspect materials which may exist in areas identified in the above restrictions should be considered as Presumed Containing Materials (PACM). Further evaluation should be made of suspect materials prior to any renovation, construction or demolition project. A more detailed presentation of the results of the Identification Survey is found in the attached tables. SAMPLING METHODOLOGY Bulk samples were collected and analyzed in order to determine the identity of suspect materials and their composition. The purpose of the survey was to identify all accessible ACMs within the designated spaces inspected. Bulk samples were grouped into homogenous sampling areas. A homogenous sampling area contains suspect asbestos materials that are uniform in texture, appearance, time of installation, and is unlikely to consist of more than one type or formulation of material. Per AHERA protocol, if any sample within the homogenous sampling area has greater than 1% by weight, then the entire sampling area is assumed to containing asbestos. Bulk samples were collected from both friable and non-friable suspect asbestos containing materials. Representative core samples of each material were collected by penetrating the material to its substrate. Each sample and sample location was incorporated into a sampling log and chain of custody for each sample was documented. All samples were placed in sealed containers and labeled with an identifying code.

4 Bulk samples were analyzed by of (NIST Accredited) via (PLM) in accordance with US Environmental Protection Agency (EPA) Method 600R/R-93/116. PLM is the reference method of analysis to demonstrate that presumed asbestos containing materials do not contain asbestos per OSHA s Construction Standard (29 CFR ) and EPA s Hazard Emergency Response Act (40 CFR 763, Subpart E). The detection limit of the PLM referenced method is one percent (1%) asbestos. EMSL Analytical Inc. is accredited through the National Voluntary Laboratory Accreditation Program (Lab Code # ). Please refer to the attachments of the report which includes the laboratory results. PLM is not consistently reliable in detecting asbestos in floor coverings and similar Non-Friable Organically Bound (NOB) materials. Fiber size (both length & diameter) of NOBs is frequently very small limiting fiber detection by PLM optics. Fibers are also tightly bound in the matrix of NOBs obscuring their detection. Confirmatory TEM analysis of NOB samples with negative or inconclusive asbestos content by PLM is not required by the State of Florida. TEM analysis of NOB samples was not authorized by the client and was not included in the scope of services for this project. LIMITATIONS AND SERVICE CONSTRAINTS Professional opinions presented in this report are based on information made available to VERTEX either by review of data provided by others or data gained by inspection personnel. VERTEX affirms that data gathered and presented in this report was collected in an appropriate manner in accordance with generally accepted methods and practices. Conditions described in this report were observed at the time of the investigation, unless otherwise stated. Exploratory demolition was performed, to the extent feasible, in all permanent ceilings, walls and/or other structures. Reasonable effort was made by inspection personnel to locate and sample suspect materials within the designated spaces. However, for any facility, the existence of unique or concealed ACMs and debris is a possibility. VERTEX does not warrant, guarantee or profess to have the ability to locate or identify all ACMs in a facility. The intent of this report is to be used in planning for renovation activity. If ACMs are discovered during demolition related activities, these materials should be removed and disposed of in accordance with all local, state and federal regulations prior to any demolition-related activities. Abatement of ACM must be performed by an accredited/certified/licensed asbestos-abatement contractor prior to disturbance.

5 VERTEX analyzed only the substances, conditions, and locations described in the report at the time indicated. VERTEX retains the right to revise this report if new information is later discovered or made available. The report must be presented in its entirety. CONCLUSIONS AND RECOMMENDATIONS An asbestos containing materials (ACM) inspection was conducted within the designated retail spaces (i.e. Units 4758, , 4764, 4768 and 4770) of the Gulf Harbor Plaza Shopping Center located on US Highway 19 in New Port Richey, Florida. This inspection was performed on December 18, 2014 by Mike Williams (NV5), Licensed Building Inspector, performed under the direction of Steven Lipson, a Licensed State of Florida Consultant (#AX-43). Through the sampling and analysis of suspect materials, ACMs were found to exist (e.g. mastic associated with non-asbestos floor tile) within Space No ACMs were identified within the remainder of the spaces inspected (i.e. Spaces 4770, 4768, , and 4758). ACM materials which will be impacted or disturbed by renovation activities must first be removed by a licensed asbestos abatement contractor in accordance with all applicable asbestos regulations. If suspect materials are discovered during future renovation activities, VERTEX recommends collecting/analyzing samples of the materials for asbestos content prior to disturbance.
