3 CERTIFICATION OF RESULTS This Work Plan has been prepared for the exclusive use of the Client; Dennis - Yarmouth Regional School District. The Work Plan will be used by the Abatement Contractor for the clean-up of the asbestos materials at the Ezra Baker Elementary. Photocopying of this document by parties other than those designated by the Client, or use of this document for purposes other than it is intended, is prohibited. 4 A. INTRODUCTION Asbestos containing materials are present at the Ezra Baker Elementary. The reason for this asbestos removal Work Plan is for in order to clean-up of this material in conjunction with all applicable rules and regulations noted in this work plan. The VERTEX Companies, Inc. (VERTEX) is contracted with the Dennis-Yarmouth Regional School District, to perform Full-time Perimeter Monitoring and Final Clearances in compliance with AHERA regulations. B. ABATEMENT CONTRACTOR The Abatement Contractor will be performing all the work at the Ezra Baker Elementary School Gymnasium and is required to follow all rules, regulations, and guidelines pertaining to all abatement work. C. WORK PROCEDURES Work procedures as follows: all clean-up work at the Ezra Baker Elementary will be under mini containments with a full three chamber decontamination unit at the entrance of the work areas (plywood stabilizers). All work performed by the abatement contractor will follow all rules and regulations per the EPA and OSHA regulations, NIOSH recommendations, Massachusetts Department of Environmental Protection (DEP) requirements, Massachusetts Department of Labor Standards (DLS) regulations, and any other applicable Federal, Commonwealth and local government regulations and guidelines. 1. A Full three-chamber decontamination unit chamber will be erected prior to any work. 2. Amended Water will be used during Abatement and Cleaning Operations. 3. Mini containment will consist of walls, ceilings and Hepa exhaust. 4. Full abatement clean-up will consist of manually/mechanically removal of the plywood flooring and potential associated debris which will be treated as asbestos contaminated waste. 5. The contaminated plywood sheets will be wrapped in the constructed containment area with 2 layers of 6mil poly and proper labeling prior to the removal from the containment area. 6. All other waste generated (i.e. potential debris, rags, utilized PPE, etc.) will be double bagged within a 6-mil poly bags with adequate water, labeled and properly disposed. 7. All areas will be HEPA vacuumed, wiped/washed down floors, walls, ceilings and equipment, all other non-porous items and decon etc. 8. Upon Completion of a successful Final Visual Clearance, PCM Clearance test will be performed by owner s consultant (VERTEX). All monitoring will be in compliance with AHERA Regulations. 1

5 D. RELATED CODES AND STANDARDS The publications listed below form part of this asbestos removal Work Plan to the extent referenced and applicable to the work described herein. The work procedures of this Work Plan have been developed to incorporate the substantive requirements of these codes and standards. The current edition of each reference shall be applicable. 1. 6 E. PERSONAL PROTECTION EQUIPMENT 1. Respiratory Protection Respirators will be used during the following activities: a. All work performed within a regulated area where respirators are required; b. All work where employees are or may be exposed to airborne asbestos fiber concentrations at or above the OSHA permissible exposure limit (PEL). The Contractor will be responsible for the proper execution of the asbestos removal work plan and provide proper waste packaging as required for this project. Contractor will provide respirators to the asbestos removal workers, selecting the appropriate type from among those approved by the National Institute of Occupational Safety and Health (NIOSH) Administration. The Contractor shall have written respiratory protection programs that will be in force for this project. The respiratory protection program shall comply with governing regulations and will include respirator selection and use, medical clearance for respirator use and medical surveillance (reference 29 CFR ). The contractor will provide, at a minimum, half-mask air purifying respirators equipped with high-efficiency particulate air (HEPA) filters for this project. F. PROTECTIVE CLOTHING The contractors will require workers to wear protective clothing consisting of, at minimum, disposable suits. As appropriate this will include head and foot coverings and gloves. The protective clothing will be required for all work. The contractors will package, transport and dispose of the asbestos-contaminated materials and clothing in sealed, impermeable bags or other approved containers bearing appropriate asbestos warning labels. The Supervisor will periodically examine employee protective clothing to ensure compliance with this Work Plan. Rips or tears found while the employee is working must be mended or replaced immediately. G. REGULATED WORK AREAS All affected areas within the first floor of the Ezra Gymnasium (see attached drawing) shall be contained within regulated work areas as described above. All work or any other operations only authorized (certified) personnel may enter the regulated work area. The designated competent person will supervise all work performed in the area. 4

7 The Contractor will demarcate the regulated area in any manner that minimizes the number of persons within the area and prevents access by unauthorized personnel and protects persons outside the area from exposure to airborne particulates. Asbestos Warning Signage will be used to define the regulated work area. Additionally, proper asbestos warning signs will be prominently displayed at all points of access to the regulated area. The signs will bear the following information: DANGER ASBESTOS CANCER AND LUNG DISEASE HAZARD AUTHORIZED PERSONNEL ONLY RESPIRATORY AND PROTECTIVE CLOTHING ARE REQUIRED IN THIS AREA The contractor will provide all employees with appropriate respirators. Visitors (certified) will only be permitted to enter the Work Area provided they are qualified and medically approved to wear negative pressure air purifying respirators and enter areas where exposure is possible. Employees will not eat, drink, smoke, chew tobacco or gum, or apply cosmetics in regulated areas. The contractor will inform other building/site occupants (construction personnel) of the following: 1. Nature of the work, 2. Regulated work area requirements, and 3. Measures taken to protect employees. H. HYGIENE FACILITIES The Contractor will establish a three-chamber decontamination facility (DF), which shall also be utilized for all work pertaining to that area. The Contractor will ensure employees enter and exit the regulated work area through the DF. This DF shall be used throughout work until all material is removed. The DF will be equipped with water (hot and cold or warm), soap, disposable towels and other items necessary for the proper containment and control of particulates/dust/fibers. 5

8 I. HOUSEKEEPING Asbestos waste, scrap, debris, bags, containers, equipment, and contaminated clothing consigned for disposal will be collected and disposed of in sealed, labeled, impermeable bags or other approved impermeable containers and not left to accumulate on site at end of shift. The Contractor will affix warning labels to all asbestos containers. Labels will be printed in large, bold letters on a contrasting background and used in accordance with the OSHA Hazard Communication Standard (29 CFR ). All labels will contain a warning statement against breathing fibers and contain the following wording: DANGER CONTAINS ASBESTOS FIBERS AVOID CREATING DUST CANCER AND LUNG DISEASE HAZARD J. ACCESS AND NOTIFICATION OF OTHER WORKERS AT MULTI-EMPLOYER WORKSITE All personnel entering the regulated area will be required to read the Work Plan before they are permitted to enter any regulated work area at the site. Access to the regulated work areas will only be permitted to personnel with proper trained and certified licenses (MA trained certified) and state officials only. No other trades will be permitted into the regulated work area without proper training (MA trained certified) in accordance with all Federal and State regulations. K. PROJECT MONITORING 1. Personal Monitoring The contractors shall have a competent person (as defined by OSHA) oversee exposure monitoring to accurately determine the airborne concentrations to which employees may be exposed. This monitoring shall be conducted in accordance with the OSHA Construction Standard (29 CFR ). A sufficient number of workers performing removal activities will be monitored by the contractors to effectively monitor worker exposure for specific work activities. Workers will be using low-flow personal air pumps to collect all air samples that will be worn within the workers breathing zone. These pumps will be calibrated prior to use in accordance with manufacturer s instructions and checked daily with quality rotometers. Samples will be analyzed by properly qualified and licensed personnel. 6

9 2. Initial Exposure Assessment (IEA) The contractors will ensure that no employee is exposed to airborne concentrations exceeding 0.1 fibers per cubic centimeter (f/cc) as an 8-hour time-weighted average (TWA). Additionally, the contractors will ensure that no employee is exposed to airborne fiber concentrations exceeding 1.0 f/cc as averaged over a 30 minute, the OSHA Excursion Limit, PEL. 3. Perimeter Monitoring VERTEX s certified asbestos project monitor (APM) will conduct full time Perimeter monitoring which includes, Pre Abatement Activities, Inspection Monitoring, Post Visual and Final Visual Clearances for the related abatement activities. Perimeter air samples will be analyzed by Phase Contrast Microscopy (PCM). The collection and analysis of PCM air samples shall be in accordance with the NIOSH 7400 Method. 4. Final Air Clearance. VERTEX will then re-enter the work spaces in order to perform aggressive clearance air sampling according to AHERA protocol for asbestos abatement within schools. Fans will be placed in an upward facing direction and turned on to the "low" setting to continually agitate the air during testing. VERTEX will also walk the entire work area with a leaf blower and will direct the leaf blower at all areas so as to guarantee the thorough agitation of areas such as above ductwork, above light fixtures and in comers and crevices. High volume air intake pumps will be set up at various locations, within the work area and outside the work area perimeters. Approximately 1,200-1,500 Liters of air will be collected on mixed cellulose ester filters from each pump. At that time, we will retrieve the samples for analysis, by Phase Contrast Microscopy (PCM). If all samples fall below regulated levels for occupancy following abatement under AHERA, the contractor will be allowed to tear-down the containment. Finally, following containment tear-down, Vertex will complete a Post Visual Checklist to assure and document that all areas are returned to you in at least the condition in which they were relinquished. 7
