Inspection (Yell Co) AFIN: Permit No.: AR and ARR Dear Mr. Childers: On October 25 and 28, 2013, I performed routine compliance inspections of the above referenced permits in accordance with the provisions of the Federal Clean Water Act, the Arkansas Water and Air Pollution Control Act, and the regulations promulgated thereunder. A copy of the inspection report is enclosed for your records. Please refer to the Summary of Findings sections of the attached inspection reports and provide a written response for each violation that was noted. This response should be mailed to the attention of the Water Division Inspection Branch at the address at the bottom of this letter or e- mailed to Water-Inspection-Report@adeq.state.ar.us. There is no record of flow and debris measurements. Part I Section A of the permit requires you to monitor flow and debris twice/week. 2. A large space was observed in the weir box at the pipe, which allows effluent to flow out of the box without passing over the weir plate. Soil is eroded from around the box, suggesting this has been going on for some time. This explains why there has never been a discharge recorded at the facility and should be corrected immediately. This is a violation of Part II, 3 of your permit. 3. There is no staff gauge or secondary method of determining flow over the weir. You need to install an accurate gauge to determine head height over the weir. This is a violation of Part II, 3 of your permit. 4. The "as built" freeboard capacity of the ponds appears to be less than the approved design. At the time of this inspection, freeboard in the debris pond was approximately 24 inches and freeboard in the settling pond was approximately 18 inches. 5. Vegetation needs to be maintained around the ponds and outfall. Trees and shrubs must be removed from the pond levees. Vegetation should be removed from the area around the weir box and outfall so they may be visually inspected. This is a violation of Part III, Section B. 1 of your permit. I reminded Mr. Reynolds to submit the NOI for the upcoming permit renewal. Review of facility DMR indicate no discharge from the facility for the period of record. I inspected the effluent box and discovered it has a large and obvious hole where effluent leaks out. I expressed the importance of correcting this to Mr. Reynolds. No samples can be collected until the effluent box is built correctly.
