2 Overview of the Economic Impact of the Investments and Operations of Catholic Health Partners on the State of Ohio Catholic Health Partners (CHP) operates 22 hospitals and dozens of physicians groups, nursing homes, outpatient centers, and other healthcare-related facilities in six networks across the State of Ohio. This report summarizes annual economic impacts generated as a result of both CHP s investments and its operations during Investments, which consist of new construction and major capital expenditures, and operations of CHP s healthcare facilities create increased economic activity in the State. This occurs directly, through CHP s payment of employees and spending on goods and services, and indirectly, through the consequent spending by these employees and vendors. In 2011, CHP s facilities together spent $3.81 billion, which produced: A total annual economic impact of $8.95 billion, including $3.13 billion in household earnings. These expenditures by CHP s network of facilities generated: 30,429 jobs across the CHP network, making it the fourth-largest employer in the State, and A total employment impact of 64,424 jobs. As Ohio s fourth largest employer and largest health system, CHP s faith-based care provides patients with accessible, safe, affordable and top- quartile quality services. Being the market leader on this value equation is our mission. CHP s six regional Ohio networks encompass 28 of the State s 88 counties; 38 percent of Ohio s residents live in these counties. Serving the healthcare needs of these people has made CHP, including the Home Office Headquarters, one of the ten largest employers in the Cincinnati region. In Lorain, CHP is the second largest employer, and it is the largest employer in each of the other four networks where CHP hospitals are located. Other benefits that CHP produced for Ohio in 2011 included: $235 million in income, sales, and other taxes for local governments and the State of Ohio, and $326 million in uncompensated care for the poor and other wellness benefits for Ohioans. 4 CHP Operations Size of CHP s Operations With six hospital-based regional networks spread across the state, CHP provides quality healthcare across a wider portion of Ohio than any other health system. Collectively, CHP s networks, which encompass 28 of the State s 88 counties, include 38 percent of all Ohio residents (4.39 million of million). Through its integrated networks that include a wide range of healthcare facilities and services, from physicians to clinics to laboratories, CHP offers a comprehensive response to these communities healthcare needs. 5 Based on industry characteristics, more than one-fourth of CHP employees are nurses and nearly two-thirds are in some type of healthcare occupation. Of these, about 855 are physicians and surgeons. It also makes substantial investments in new technology and equipment each year. During 2011, these major capital expenditures on construction, information technology (IT), and other capital equipment totaled nearly $400 million, a level of spending that is likely to continue in the coming years. CHP spent a total of $395 million on capital expenditures in These expenditures generated an indirect impact of $468 million. Combined, the direct and indirect impacts amounted to a total economic impact of $863 million from capital expenditures. These capital expenditures generated direct household income of $134 million with an indirect impact of $133 million, resulting in a total household earnings impact of $267 million. A total of 2,524 jobs, nearly all in construction or IT, were created directly through these capital expenditures, creating another 3,388 indirect jobs and a total employment impact of 5,912 jobs. 6 Impact on Earnings and Employment CHP paid its employees nearly $1.61 billion in Its operating expenditures, including both these payments to employees and other spending on goods and services, required other businesses to employ workers, which generated an indirect earnings impact of $1.25 billion. The result was a total household earnings impact of $2.86 billion. In addition to 30,429 jobs for its own employees, CHP s operating expenditures created another 28,083 indirect jobs and a total employment impact of 58,512 jobs annually. Spending by employee households benefits employment in retail trade, while CHP and its vendors are the primary purchasers of administrative services. 7 Operations Impact Operating expenditures by CHP include employee payroll and benefits, non-employee personnel expenses, and spending on all supplies and services purchased from outside vendors. Together all facilities spent a total of $3.41 billion on operating expenditures in This spending is expected to have generated an indirect impact of $4.67 billion in additional economic activity in the State of Ohio. Put together, a total annual economic impact of $8.09 billion was generated by operating expenditures of all CHP healthcare and related facilities, with 70 percent from its hospitals. This included $3.13 billion in household earnings for the 64,424 people holding jobs in the State of Ohio as a result of CHP s presence. The following table lists combined annual economic impacts of CHP s healthcare facilities from their capital and operating expenditures. On average, CHP is about a 50% partner in these activities. Expenditures by 12 of these joint ventures total more than $65 million.

8 Other Benefits Fiscal Impacts: Local and State Taxes CHP is a major generator of income and sales taxes for local governments and the State of Ohio. In 2011, CHP s direct fiscal impact, including property and other taxes, exceeded $71 million. Thus, CHP s total annual fiscal impact in the Ohio exceeded $235 million. This amount, which was more than 9 percent of CHP s total operating expenditures, was more than enough to support the operations of any single CHP hospital except the largest one. 3 Another way to view this community benefit is to calculate the net savings it produces for local taxpayers. Non-profit hospitals have to satisfy a community benefit standard to maintain their tax exempt status. If it were a for-profit corporation, CHP estimates that its property tax obligation would have totaled $138.5 million in At the same time, it provided $ million in community benefit by providing financial assistance and services for those in need. In other words, CHP provided $236 in community benefit for every $100 it estimates it would have paid in property taxes producing a net savings for the local community. 3 This is nearly 95% of the total Community Benefit spending of $345.7 million identified in CHP s Report to the Community (which also included CHP s Kentucky network).

9 The impact of such spending directly benefits the unemployed, low-income, and uninsured persons of the community. While such community impacts vary, spending such as these enhance the general health and well being of the communities served by CHP or those without the ability to pay. With more than three-fourths of community benefit spending covering uncompensated care (77.8%), CHP spent about $1.4 million a week to support wellness programs and other efforts to help improve the health of people in the communities CHP serves. By comparison, CHP spent approximately 25 percent more on its community benefit activities (including uncompensated care) than the collective budgets of all city health departments in Ohio. 4 Diversity Spending In recent years, CHP has made a concerted effort to change its purchasing practices to increase the proportion of its purchases that are made from vendors who are women or minority-owned business enterprises (W/MBEs). One significant example of this change occurred in the selection of a W/MBE vendor for the upgrade of CHP s Health Information Technology platform in all of its facilities and physician practices. The overall cost of this system exceeded $72 million, including $27.7 million in 2011, which accounts for the majority of system-wide IT purchases. Other purchases from W/MBE vendors more than tripled in 2011, increasing from an average of $16,625,700 in to $55,086,000 in This included an increase from $6.6 to $15.9 million and an increase at all other locations from $10.0 to $39.2 million. CHP s goals for 2012 anticipated continued expansion of its diversity spending, with spending expected to grow by 16.0 percent system-wide, including by 19.4 percent at the Home Office and by an average of 14.6 percent at other locations. New Money To the extent that CHP received out-of-state funding for its clinical, research, and educational services, it may be viewed as one of Ohio s exporting firms. For example, although patients from outside of the Ohio area represent only 2.7 percent of all CHP hospital patients, those who come to CHP generally need a larger amount of healthcare, requiring larger proportions of surgery and inpatient care. Consequently, 4.6 percent of total patient revenue, or $453 million, comes from this small percentage of patients coming from outside of Ohio. 4 Based on data from the 2010 Annual Surveys of State and Local Government Finances and the quinquennial Census of Governments.

12 Methodology The economic impact of any spending includes two elements, as defined here: the direct impact and the indirect and induced impact. The direct impact comprises purchases of local resources (labor, goods, and services) for construction, capital improvements, and operations. The indirect and induced impact is the purchases of local resources made by local businesses in order to produce the goods and services purchased by the healthcare facilities, along with consequent purchases by other businesses that supply the first group of businesses. It also includes the local household spending of the earnings generated from the healthcare facilities capital and operating expenditures. CHP provided the Economics Center with data related to its healthcare facilities annual capital and operating expenditures. Measuring the indirect and induced impacts in the State of Ohio is made possible by multipliers derived from an input-output table ( I-O table ) for the State. The Bureau of Economic Analysis of the U.S. Department of Commerce has constructed these tables for various regions in the U.S. through the second generation of its Regional Input-Output Modeling System (RIMS II). The RIMS II tables for Ohio provide multipliers to measure the indirect and induced impacts of virtually any type of economic activity in the region.
