If you intend to apply for formal approval of a certification scheme, you should request an fillable application form and state the biomass categories to which the application pertains via duurzaamheidvastebiomassa@rvo.nl. You will subsequently receive an application form by pertaining to the categories which you indicated. Application form for the approval of a certification scheme demonstrating the sustainability of solid biomass for energy applications in the Netherlands This document pertains to a new application. A separate application form will be available to handle requests for the modification or extension of certification schemes that have already been partly or fully approved. 2 The Minister of Economic Affairs may decide to request a recommendation on your application from the Advisory Committee on the Sustainability of Biomass for Energy Applications (ADBE). By signing this application, you agree with: 1. publication by the ADBE of a request from the Minister to issue a recommendation on your scheme, including (a) the sustainability criteria and the biomass categories for which you submitted an application for approval and (b)(a link to) the scheme documents that you have added to your application; 2. the collection of information on the scheme through an external contribution by third parties via the ADBE website; 3. the ADBE assessment procedure (see in Dutch language only). 6 Part D Part D should be completed in case your certification scheme uses a risk-based approach by the biomass producer to one or more of the sustainability criteria for sustainable forest management as referred to in Part B under Principles P6 to P11 inclusive. This approach is permitted for category 2 biomass subject to conditions. In this event, your approach to risk assessment and the implementation of mitigating measures will be assessed. It is your task to compare your risk assessment to the criteria for a risk-based approach as referred to in Chapter 8 of the Verification Protocol. To that end, copy the five steps from the approach in Chapter 8 of the Verification Protocol to another document, where you should demonstrate how your approach complies with each step of the Verification Protocol. Please include this document as an appendix with your application and fill in the name of that appendix here below. 7 Explanatory notes to this application form General explanatory notes This application form pertains to an application for approval as referred to in Sections 10 and 12 of the Decree on the conformity assessment of solid biomass for energy applications (Besluit conformiteitsbeoordeling vaste biomassa voor energietoepassingen). Some of the terms used in this form, including beheerseisen ( management requirements ), certificatie ( certification ), duurzaamheidseisen ( sustainability criteria ), goedkeuring ( approval ) and schemabeheerder ( scheme manager ) have been defined in that Decree and have the same meaning in this application form. This Decree is expected to be published in the Government Gazette by the end of 2017, until which time the definitions of the aforementioned terms in the Decree will be provided upon request. An application for approval of a certification scheme is to be filed by or on behalf of the scheme manager. The scheme manager is a legal entity such as a private limited company or an incorporation, represented by one or more persons. This formal representation has been established for the Netherlands by the Dutch Chamber of Commerce. The application should include documentation which proves that the scheme manager is formally represented by the applicant. In the Netherlands, this documentation may take the form of an extract of the scheme manager legal entity from the Dutch Chamber of Commerce register which states the applicant's name. In case of another country, an equivalent document should be included. The application can also be submitted by a person authorised by a formal representative of the scheme manager. In this event, the application should include: 1. the documentation referred to above; and 2. a letter signed by the formal representative as referred to in the documentation which states that the applicant has been authorised to submit the application on behalf of the scheme manager. Upon receipt, the application will be double-checked on the basis of public information and/or communications with the management or secretariat, in order to verify again that the application has been submitted on behalf of the scheme manager. Please state in your application for which region or countries you are submitting your application, or state Internationaal ( International ) in the event that your scheme lacks geographically specific scheme documents. For example, you can submit an application for SBS Nederland, SBS Europa or SBS Internationaal (in which example the abbreviation SBS for sustainable biomass scheme has been randomly chosen). You must submit your application in Dutch, even though it is possible that scheme documents have been drawn up in a different language than Dutch. In this event, you may be requested to supply Dutch translations of parts or all of your scheme documents during the application process, especially if your scheme has been drawn up in a different language than English. Request for early assessment in 2017 Informal requests for approval can already be submitted by scheme managers in In order to assess these schemes, the Minister of Economic Affairs will request a recommendation from the Advisory Committee on the Sustainability of Biomass for Page 7 of 10

8 Energy Applications (ADBE). This procedure will allow the Minister to approve certification schemes after the entry into force of the Decree on the conformity assessment of solid biomass for energy applications (Besluit conformiteitsbeoordeling vaste biomassa voor energietoepassingen) in early 2018, using the recommendations that have by then been completed by the advisory committee. The present application form includes the sustainability criteria from the draft Regulation on the conformity assessment of solid biomass for energy applications (Regeling conformiteitsbeoordeling vaste biomassa voor energietoepassingen). In case of changes to these criteria, you will be requested by the advisory committee to amend Part B of this application form accordingly. Note that no changes to the criteria are expected, as these have already been extensively discussed with stakeholders. Finally, you are required to resubmit your application for approval after the entry into force of the Decree, even if the advisory committee has already issued a recommendation to the Minister pursuant to the procedure as referred to at Although this reapplication will be identical to your current application in most cases, it will then constitute a formal application under the operative Decree. Your current application is an informal application designed to speed up the process by allowing the advisory committee to commence its activities in advance. Complete application Your application will be processed as soon as it is complete. Your application is complete when: 1. all relevant fields shaded pale green in Parts A, B and C of this form have been filled in. These include all fields shaded pale green in Parts A and C of this form, as well as all fields shaded pale green in Part B related to the criteria for which approval is requested; and 2. all documents as referred to in the fields shaded pale pale red red in Parts B1, C1 and D have been submitted as appendices together with the application. You are required to submit these appendices digitally or otherwise in hard copy; a link to online documents is not sufficient. Prepare your application carefully and include references to your scheme documents Please describe for each criterion in Parts B2 and C2 of this form why you consider your certification scheme to demonstrate compliance with Dutch sustainability criteria. in When in the third or fifth column of Tables B2 and C2 anaccurate descriptions from or references to scheme documents are used, this may lead to a subsequent request for an explanation, or to a refusal to approve the criterion which has an incomplete or inaccurate description and/or reference. Explanatory notes to Part A Explanatory notes to biomass categories You are required to state in your application to which of the following biomass categories it pertains: 1. Woody biomass from large Forest Management Units (FMUs 500 ha) Branches, tops, trees and primary felling residues sourced directly from forests of 500 ha or larger. Unused wood that has the same composition as wood growing in the forest and that has not been mixed with or contaminated by foreign materials or substances. 1 These criteria have been incorporated into the Regulation on the conformity assessment of solid biomass for energy applications (Regeling conformiteitsbeoordeling vaste biomassa voor energietoepassingen). 9 2. Woody biomass from small Forest Management Units (FMUs <500 ha) Branches tops, trees and primary felling residues sourced directly from forests of less than 500 ha. Unused wood that has the same composition as wood growing in the forest and that has not been mixed with or contaminated by foreign materials or substances. 3. Residues from nature and landscape management Biomass residues (branches, tops, trees) produced in the course of managing urban and rural green spaces and nature areas, other than forests designated for the preservation, restoration or enhancement of specific natural, recreational or aesthetic functions. These also include biomass residues produced during routine maintenance of public green spaces and parks. 4. Agricultural residues Residues obtained directly from agricultural business. Short rotation crops are excluded, with the exception of the residues thereof. 5. Biogenic residues and waste flows Waste flows and residues from the agro-food and timber industry (secondary residual flows) and tertiary residual flows such as post-consumer wood waste. Explanatory notes to sustainability criteria A scheme manager is required to state in Part A of the application for approval to which sustainability criteria from Part B the application pertains. You are advised to use the coding system from Part B of the application form; for example, P1 P3, 4.1, 4.2 can be used to refer to all criteria under Principles 1, 2 and 3 (in other words, criteria 1.1, 2, 3.1, 3.2 and 3.3) as well as criteria 4.1 and 4.2. Such a statement does not apply to the management requirements from Part C, as the scheme must comply with all management requirements in order to be eligible for approval. Explanatory notes to risk-based approach Biomass producers are temporarily allowed to apply a risk-based approach to category 2 biomass in order to demonstrate compliance with the sustainability criteria under Principles P6 to P11 inclusive. Chapter 8 of the Verification Protocol explains what a riskbased approach entails and how it can be applied. If you apply a risk-based approach, you are required to fill in Part D of the application form and include the document referred to in Part D as an appendix to the application. Explanatory notes to Parts B and C Please state in Parts B1 and C1 on the basis of which scheme documents the scheme should be assessed or approved. This assessment or approval will normally be based on a selection of the total body of documents within a certification scheme, as in many cases a portion of the scheme documents concern aspects that are not relevant to the current application, such as social and economic criteria or older versions of the scheme. You are required to include the documents mentioned and submit them together with your application for approval, preferably in digital form. It is not sufficient to refer to documents accessible online, because changes could be made to such online documents and this could afterwards cause confusion as to which document version was used for assessing the application. In this respect, it is also important that you state which version will be assessed if multiple versions of your certification scheme exist. Page 9 of 10

10 You are only required to fill in Part B2 for sustainability criteria for which you request approval, as stated by you in Part A. In other words, the fields shaded pale green in Part B will remain empty if they concern criteria for which no approval is requested In Parts B2 and C2, you should substantiate for each of the relevant criteria whether and, if so, how your certification scheme demonstrates compliance with Dutch sustainability criteria. You can use the fourth column of these parts to score the extent to which you consider the criterion from your certification scheme to comply with the relevant Dutch sustainability criterion. Please select one of the following scores: + : : the criterion from your certification scheme fully complies; ± : the criterion from your certification scheme complies approximately; or - : the criterion from your certification scheme fails to comply.
