1 Establishing Allied Health Rural Generalist Training Psitins Purpse f this dcument This dcument defines the rle requirements fr an Allied Health Rural Generalist Training Psitin, details the assciated resurcing implicatins, and prvides infrmatin n issues t cnsider when determining wrkplace suitability fr establishing a psitin. Infrmatin is als prvided t guide the peratinal decisins and planning fr establishing an Allied Health Rural Generalist Training Psitin. Rle requirements f Allied Health Rural Generalist Training Psitins An Allied Health Rural Generalist Training Psitin (AHRG Training Psitin) is a rle designed t supprt the develpment f an early career prfessinal frm graduate-level cmpetency thrugh t a prficient rural generalist practitiner in an allied health prfessin. T be cnsidered an AHRG Training Psitin, and use this title, the psitin shuld include the fllwing cmpnents. These cmpnents shuld be explicitly identified in rle dcumentatin and supprting prcesses. 1. Dedicated develpment and supervisin time f at least 0.1FTE 2. Participatin in the allied health Rural Generalist Prgram thrugh James Ck University (refer t 3. A frmal develpment plan aligned with the Level 1 and/r Level 2 Rural Generalist Prgram relevant t the prfessin (where available) and requirements f the emplying service, 4. Develpment funding t supprt participatin in the Rural Generalist Prgram. 5. Regular frmal supervisin with a cllcated r highly accessible prfessin-specific lcal supervisr. Highly accessible means present n-site and available t prvide ad hc supprt and advice fr 50 per cent r mre f wrk hurs. Scheduled and frmal supervisin arrangements shuld als be established and strictly maintained. This may be thrugh the lcal supervisr r anther experienced prfessinal in the team r anther site. Fr practitiners with greater experience, mre flexible prfessin-specific and inter-prfessinal supervisin arrangements shuld be implemented which may include a cmbinatin f n-site and remte supervisin. Scheduled and frmal supervisin arrangements shuld als be established and strictly maintained. 6. Cntributin t rural generalist service develpment strategies that imprve client care and service utcmes. This is a mandatry requirement f AHRG Training Psitins. It is als an expectatin f the university that all participants in the Rural Generalist Prgram will be invlved in a service develpment prject and apply learnings frm their training mdules t the lcal strategy. 2 Extended scpe f practice, including: skill sharing between allied health prfessins (trans-prfessinal practice), r extended scpe cmplex practices such as prescribing, rdering and interpreting imaging and pathlgy (if this sits utside the prfessin s entry standards), Delegatin t supprt wrkers (e.g. allied health assistants) and similar supprt functins (e.g. X-ray peratr services), Telehealth-supprted service delivery, New service mdels, including partnerships that supprt implementatin f a generalist scpe f services previusly nly prvided by specialised units in urban centres. This strategy is abut mving care clser t rural and remte cmmunity members by expanding the range f services ffered lcally. Examples include: cmpressin garment prescriptin, fitting and trialling (see Queensland Health, develping shared care mdels with metrplitan cancer services fr lcal speech pathlgy management f clients fllwing treatment fr head and neck cancer (see Queensland Health dietetics management fr clients accessing a tele-chemtherapy service in their lcal district hspital. Resurcing requirements fr AHRG Training Psitins Psitin csts are cnsistent with any ther entry level r early career rle (salary, n-csts, entitlements, accmmdatin, telecmmunicatins, etc.) excepting that FTE f the psitin will be allcated t develpment, training and supervisin activities. This will need t be accunted fr in wrkflw and activity expectatins f the psitin, and therefre in the budget mdelling. The primary clinical supervisr will allcate apprximately ne hur per week fr frmal supervisin and additinal time fr wrk-place based supprt and teaching. This shuld be bradly cnsistent with the cmmitment t supervisin and supprt fr any ther early career practitiner in the wrk unit. Time investment f the manager and team and any related resurcing requirements fr the service develpment prject. Resurce requirements will be defined by the prject adpted by the team in cnsultatin with service leaders e.g. purchase f telehealth equipment fr a new service etc. The csts f the Rural Generalist Prgram are defined by James Ck University (JCU), the administering institutin. Fee estimates at March 2017 are prvided belw but current infrmatin shuld be surced frm JCU at Level 1: Twelve mdules (equivalent t 6 credit pints) at $650 (excl GST) per mdule. Delivered nline. Level 2: Eight subjects (24 credit pints) Graduate Diplma curse. Subject csts cnsistent with similar JCU pstgraduate curses (apprximately $3,200 per subject). Participants cmpleting Level 1 can apply fr credit fr 6 credit pints. Depending n subject chice, sme blck study r ther travel may be required. Additinal csts be incurred fr study resurces, IT resurces and data use. Fr the trial f the Rural Generalist Prgram ( ) AHRG Training Psitins can be created that supprt the incumbent t cmplete the Level 1 prgram nly (cmmencing May 2017 fr up t 24 mnths) r Level 2 prgram nly (up t 24 mnths cmmencing early 2018) r the Level 1 and 2 (cmmencing May 2017 and cncluding Dec 2019). 3 Other develpment csts may be incurred fr wrk shadwing r clinical placements, additinal curses utside the Rural Generalist Prgram, mandatry training etc. Wrkplace suitability fr establishing AHRG Training Psitins Successful implementatin f an AHRG Training Psitin requires emplying rganisatins t meet certain bligatins. At any given time, rganisatins will differ in their capacity t fulfil these bligatins. Wrk units and psitins suitable fr implementing the supervisin, supprt, resurcing, and leadership required f an AHRG Training Psitin include: Early career psitins with a c-lcated r highly accessible, experienced, prfessin-specific supervisr rle with reasnable expectatin f stable staffing. Entry level psitins with a recent histry f unstable staffing r average tenure f less than 2 years present a gd pprtunity fr cnversin t an AHRG Training Psitin as the marginal cst is likely t be minimal, and will be utweighed by the ptential benefits f increased tenure and lcally develped wrkfrce. A wrk unit that can cmmit t strictly quarantining the 0.1FTE 0.2FTE develpment and supervisin time. If this is nt feasible, the AHRG Training Psitin is a pr chice as a staffing strategy as it will be challenging fr the incumbent t successfully take part in the Rural Generalist Prgram. A wrk unit must have the capacity t develp, implement and trial service develpment strategies as part f their cntinuus quality imprvement cycle. The AHRG Training Psitin incumbent must participate in service develpment activities t cmplete the Level 1 Rural Generalist Prgram. As an early career practitiner, the AHRG Training Psitin incumbent will nt lead the service develpment prject. The team leader r ther senir practitiner wuld be respnsible fr managing the initiative. The brader team must be engaged in, r cntribute t, the service develpment strategy/ies and have access t the required skill sets and resurces t implement and evaluate / mnitr the service change. Teams with limited capacity t supprt rural generalist service develpment strategies are a pr ptin fr establishing an AHRG Training Psitin. A wrk unit shuld have adequate clinical demand and a caselad mix apprpriate fr emplyment f an early career practitiner. A service with severe demand management prblems (e.g. lng waiting lists and high vertime) is a pr ptin fr adding an inexperienced staff member as the training and supervisin lad can amplify team stress and service capacity issues. Services with a narrw fcus n a generally cmplex caselad are als nt suited t the additin f an early career practitiner in an AHRG Training Psitin. Fr advice n assessing the suitability f yur rganisatin r wrk unit implementing an AHRG Training Psitin cntact SARRAH. Creating an AHRG Training Psitin Guided by the infrmatin abve, fur key questins need t be answered t create an AHRG Training Psitin. Answers t these questins will vary depending n the lcal wrk unit structure, available resurces, and lcal recruitment needs. 1. Hw will the psitin be created n the wrkfrce establishment? 2. Hw will the psitin be resurced? 3. What is the term and status f the psitin? 4. What is the term and status f the incumbent s emplyment? Answers t these questins will guide the prcesses fr implementing the AHRG Training Psitin. Figure 1 n the fllwing page presents pssible answers t each f these questins. SUPERNUMERARY POSITION Funding investment fr a full psitin. MIXED Redesign an existing fractinal psitin that has pr recruitment ptential, t 1.0 FTE using existing and sme tp-up funding if required. Hw will the psitin be resurced? WORKFORCE REDESIGN One r mre unfilled / difficult-t-fill psitins are redesigned int designated early career AHRG Training Psitin/s. GROWTH FUNDING Grwth funding fr a service is directed t creating AHRG Training Psitins. CENTRALISED FUNDING Funding fr the AHRG Training Psitin, r fr specific csts such as educatin fees, are administered by a central unit in the rganisatin rather than the emplying team. [See templates in the AHRG Training Psitin Tlkit.] EXTERNAL FUNDING Resurcing received frm utside the wrk unit and rganisatin (i.e. a third party). [Cnsider adapting relevant the AHRG Training Psitin Tlkit.] What is the term and status f the psitin? TEMPORARY POSITION AHRG Training Psitin is nn-recurrently funded. A minimum term f 2 years is required t cmplete the Level 1 r Level 2 Rural Generalist Prgram, 3 years is required t cmplete Levels 1 and 2. PERMANENT POSITION AHRG Training Psitins recurrently funded in the lcal wrk unit r rganisatin. What is the term and status f the incumbent s emplyment? TEMPORARY EMPLOYMENT Emplyment term must enable cmpletin f the Rural Generalist Prgram (Level 1, tw years; Level 1 and Level 2, three years). Apprpriate fr psitins with nn-recurrent funding. Apprpriate fr rganisatins seeking t build a grup f clinicians with lcal experience. PERMANENT EMPLOYMENT The AHRG Training Psitin incumbent transitins t a standard psitin nce the Rural Generalist Prgram milestnes are cmpleted. 5 1. Hw will the psitin be created n the wrkfrce establishment? There are many lcal variants t the three mdels presented in Figure 1. If the requirements fr the psitin are met, the lcal service can be pprtunistic and establish the psitin using the business mdel that makes sense fr their cntext. 2. Hw will the psitin be resurced? The answer t the first questin will infrm the quantum f resurces needed t meet the requirements f an AHRG Training Psitin. There are a number f ways t apprach securing the required resurces. Service redesign: a service can create an AHRG Training Psitin by redesigning hw the internal resurces f the lcal wrk unit are used. Fr example, a health service has a ttal f 4.4FTE ccupatinal therapy (OT) psitins. Three psitins have stable, lng-term incumbents with between 6 and 18 years prfessinal experience. The ther tw 1.0 FTE psitins are chrnically unfilled r are generally nly filled temprarily by lcums. This results in perids f limited staffing, a need t restrict services, and significant stress fr the permanent staff. At a time when bth psitins are nt filled, the team redesigns the rganisatinal structure and wrkfrce establishment t create tw OT AHRG Training Psitins. Surplus funding available frm appintment f staff t these junir psitins is used t tp-up ne f the fractinal senir OT psitins by 0.2 FTE t prvide supervisin and wrk-based training fr the AHRG Training Psitins. The ther senir psitins als cntribute t the training and supervisin f the AHRG Training Psitins t develp and maintain their wn supervisin skills. Such an apprach can result in mre stable and predictable staffing that is easier t manage frm a recruitment perspective, is cst neutral, prvides a cnsistent service t the cmmunity thrugh mre reliable staffing, and ffers better pprtunities fr leave cver. The prcess als presents the pprtunity t invest in successin planning and building a lcal wrkfrce. Grwth funding: Frm time t time services may receive grwth funding frm their rganisatin. A lcal wrk unit may allcate this funding fr a designated AHRG Training Psitin, rather than extending an existing flat structure. Centralised funding: In sme rganisatins, particularly larger service prviders, funding may be rganised at an rganisatin-level and prvided t lcal wrk units t implement a psitin. This may resurce an entire supernumerary AHRG Training Psitin, r just meet the additinal resurce requirements f an AHRG Training Psitin such as enrlment fees. In this scenari, an rganisatin selectin prcess may be required, as well as dcumentatin that defines the funding ffer and bligatins f all parties with respect t the funding. The templates prvided in the Allied Health Rural Generalist Pathway Tlkit can be used by rganisatins develping a centralised funding mdel. External funding: Funding cntributins twards AHRG Training Psitins are made available frm an entity that is external t the rganisatin and lcal wrk unit. In this scenari, a service agreement r similar is likely t gvern the arrangements between the health service and external funding prvider. The templates prvided in the Allied Health Rural Generalist Pathway Tlkit may ffer useful cntent t guide selectin f funding recipients and the develpment f an agreement. 3. What is the term and status f the psitin? Permanent: The psitin is established permanently in the wrk unit. Nte that this relates t the status f the psitin and the incumbent des nt necessarily need t be recruited permanently t the rle (see questin 4 belw). Temprary: The psitin is established temprarily in the wrk unit. This is mst likely when there is nnrecurrent supprt funding. Establishing AHRG Training Psitins, June 2017 Page 5 f 6

6 NOTE: The minimum term a psitin may be established is 2 years if the incumbent will be required t cmplete Level 1 r Level 2 f the Rural Generalist Prgram; r 3 years if required t cmplete bth the Level 1 and Level 2 Rural Generalist Prgrams. 4. What is the term and status f the incumbent s emplyment? Temprary emplyment: The incumbent is emplyed n a temprary basis in the rle. The term f emplyment must be adequate t meet the Rural Generalist Prgram requirements (see abve). Temprary emplyment is mst relevant fr nn-recurrently funded (temprary) psitins, and psitins that the service wants t use as an nging graduate / early career rle t build an expanding grup f clinicians with lcal experience. This can supprt services in high grwth areas t have an nging cycle f intake t the rles that leads t recruitment pprtunities as ther rles becme available in the rganisatin. Permanent emplyment: The incumbent is permanently emplyed by the service. The emplyment mdel will need t be established such that the incumbent will transitin t a standard psitin nce the educatin milestnes are met i.e. cmpletin f the Level 1, Level 2, r the Level 1 and Level 2 Rural Generalist Prgram are cmpleted. Details f the terms f training supprt arrangements shuld be specified in the ffer f emplyment. Permanent emplyment may be feasible even if the psitin is nn-recurrently funded if turnver in the peratinal unit prduces an acceptable risk f managing the incumbent s appintment.
