1 Roles and Responsibilities The Integrated Team is comprised of the family, CWS Family Maintenance (CWS-FM) Social Worker (SW), StanWORKs Family Service Specialist III, agency partners, clerical staff, managers, supervisors, community partners, and administration. Family The roles and responsibilities of the family include: Participate and work with the SW and FSS III in developing the appropriate plan for their family Ask questions for clarification Cooperate as the case plan requires Sign contracts and case plans Participate in coordinated case planning and multi-disciplinary team meetings Report any changes in circumstances to SW and FSS III. Clerical Staff - Reception The roles and responsibilities of the clerical staff located at reception include: Provide the family the application packet consisting of the application forms and the screening tool for completion Upon completion, route the application packet to Administrative Clerk III to check the automated system If the family has an open case in the Child Welfare Services (CWS) Family Maintenance Program (FM), assign the family to a FSS III If the family has no open CWS-FM case, assign the family to a FSS II who will continue with the application process 1

2 Administrative Clerk III The roles and responsibilities of the Administrative Clerk III include: Checks automated systems and reports (e.g., ACIS, CWS / CMS, GEMS and Interface Report to screen for integrated cases) Documents on the screening tool the information gathered from the automated systems and reports Makes a copy for the screening tool binder Upon completion, routes the screening tool and application packet to reception to assign family to the appropriate staff Files a copy of case staffing form into the integrated services case staffing binder Files a copy of coordinated case planning / multi-disciplinary team meeting (CCP/MDT) form into the integrated services CCP/MDT binder Compile statistics of number of case staffing and CCP/MDT meetings per month Receives and Files the copy of Team Assessment Process (TAP) staffing form into the Integrated Services binder Assists the Manager, CWS-FM SW and StanWORKs FSS III in other activities related to integration of services 2

3 Child Welfare Services Family Maintenance Social Worker (SW) The roles and responsibilities of the SW include: Upon receipt of a new FM case, the SW verifies from the TAP staffing form if case is open in TANF/WtW or other StanWORKs programs If the case isn t open, the SW will refer the family for an Intake application When making a referral, the SW will assist the family in completing the application forms and screening tool If a case is already open in StanWORKs, TANF/WtW program, the SW will communicate with the Family Services Specialists within 24 hours of receiving the case from the Emergency Response SW. The SW will facilitate the coordination and linking of services that supports both CWS and WtW case plans Send a copy of the TAP form to the FSS III or the primary case manager on the case It is strongly encouraged that the SW receive input from the family, FSS III and other partners in the development of the coordinated case plan If CWS case plan is already developed, discusses the plan with the FSS III for the purpose of linking the CWS FM and WtW case plan Note: Modifies the CWS case plans as needed. Sends a final copy of the CWS plan to FSS III When possible, coordinates and conducts a joint home visit with the FSS III and/or partners Participates in meetings such as case staffing, coordinated case planning or multidisciplinary team meetings to link case plans and coordinate services that support the CWS and WtW plan 3

4 Child Welfare Services Family Maintenance Social Worker (SW) The roles and responsibilities of the SW include: Notifies the FSS III on an ongoing basis of any situations that may impact financial eligibility or ability to carry out the WtW plan (e.g., child removed from the home, caretaker incarcerated or hospitalized, child or caretaker entered a residential treatment facility, etc.) Communicates with family, FSS III and partners (e.g., DV Specialist, BHS Clinicians, Substance Abuse Counselors, Job Developers) of significant changes, concerns and/or progress Meets with FSS III and other partners for joint case management monitoring and evaluation Notifies the FSS III and partners involved with the family when the case is reassigned or closed Participates in program evaluation Family Service Specialist III (FSS III) The roles and responsibilities of the FSS III include: Reviews screening tool Takes Intake applications on integrated cases when Welfare-to-Work plan is present If possible, develops a coordinated case plan with CWS-FM SW and partners If a WtW plan is already developed, discusses the plan with the SW for the purpose of linking the CWS FM and WtW case plan Modifies the WtW case plan as needed Conducts joint visit with SW and/or partners and sends a copy of WtW plan to the SW 4

5 Family Service Specialist III The roles and responsibilities of the FSS III include: Participates in meetings such as case staffing and coordinated case planning/multidisciplinary team meetings to link case plans and coordinate services that support both CWS and WtW plan Notifies the SW of any financial sanctions either pending or imposed and problem solve with the SW and family to preclude it Communicates with SW and partners any significant changes, concerns and/or progress Meets with SW and other partners for joint case management monitoring and evaluation Notifies the SW and partners involved with the family when the case is closed or re-assigned Participates in program evaluation Supervisor StanWORKs and CWS The roles and responsibilities of the Supervisor include: Provides leadership in promoting the Coordinated Services Program by communicating and role modeling the Vision, Mission, Values and Guiding Principles (VMGP) of the CSA Coordinated Services Program Provides supervision of staff and clarifies program questions Acts as a resource person for staff involved in integrated cases Attends, if possible case staffing and coordinated case planning meeting Takes forward any issues and concerns presented by staff Monitors the progress of the integrated case Participates in program evaluation 5

6 Manager StanWORKs and CWS The roles and responsibilities of the Manager include: Provides leadership in consistently presenting, communicating and role-modeling the VMGP of the CSA Integrated Services Program Serves as a resource person and consultant for clarification of program mandates, agency s procedures, issues and concerns as/when needed Takes forward any issues/concerns presented by staff Participates in case staffing, CCP and MDT meeting as/when needed Partners Participates in program evaluation The roles and responsibilities of the Partners include: Provide appropriate services when referrals are received Participate in case staffing, CCP and MDT meetings Participate in coordinated case planning activities, such as coordinated case planning meetings and joint home visits Communicate with SW and FSS III regarding progress of family s participation in services, any significant changes and /or concerns May develop linked case plan with SW and FSS III Participate in joint visits with SW and FSS III Meet with SW, FSS III, and family for joint case management monitoring and evaluation Notify the SW and FSS III and other staff involved with the family when the case is closed or transferred Participate in program evaluation 6
