You must collect a portfolio of evidence which refers to real situations. Ensure there is no identifying information given about people. Your assessor or verifier will use the checklists in this Trainee Assessment when they observe your work. If you need help with this assessment, please contact your assessor. When you finish Make sure you have completed any parts where the assessor/verifier has said you need to do more work. Appeals If you wish to appeal against the assessment result or process, talk to your assessor. A service delivery model or approach provides a set of underlying principles, aims and objectives, operational parameters and reviewable outcomes that direct the nature of consumer support and the manner in which it is provided. An organisation s policies and procedures are the policies and procedures of the employer and include ethical codes, standards and other organisational requirements. 4 Portfolio of evidence It is recommended that you complete the trainee assessment for US Service delivery models before completing this assessment. For this task, you need to build a portfolio of evidence over a period of time. You must demonstrate how you have applied a service delivery model or approach for each of two people in your workplace. One of these two people you support must be Māori, Pasifika and/or from another culture. Your portfolio will consist of two parts First person and Second person. First person must be from another culture Please label all evidence relating to this person as first person. For this person you must demonstrate appropriate values, processes and protocols in relation to working with Māori, Pasifika and/or people from other cultures. [EN 2] Second person Please label all evidence relating to this person as second person. For both people 1 Collect and assemble evidence showing how you have done all of the following according to the service delivery model or approach: used an approach to support that was relevant to this person to meet their support needs. This could include getting assistance from colleagues, senior staff or experts. demonstrated the underlying values of the model or approach. used strategies to reduce and/or challenge stigma and discrimination. used strategies to encourage empowerment and advocacy. carried out all these tasks according to your own role and responsibilities and your organisation s policies and procedures. 2 Answer the questions that follow in support of the evidence you have provided. 3 Ask your verifier to complete your verification checklist to confirm your competence. Your verifier is likely to be your workplace supervisor, manager, team leader or assessor. They must be a person who has observed you work over a period of time and can confirm that you have completed the tasks to the standard required.
