4 Task 1: Observe, report and document changes For this task, you must demonstrate that you have observed, reported and documented changes in the condition of a person you support. These changes can be physical, behavioural, environmental or emotional. You will be observed demonstrating this task and the verifier or assessor may also ask questions to clarify your understanding of the tasks you are carrying out. Your observation, report and documentation must comply with your organisation s policies and procedures. Observation checklist The information in this column is for you. The right column is for the verifier to check off for the observation. Observe changes in the condition of a person you support. Report the changes in the condition of a person you support. Your report must be factual and describe the changes in condition. Follow your organisation s policies and procedures when reporting the changes in condition. Document the changes you have observed and reported for a person you support. Attach examples of the documents for your assessor, or show them to your assessor. You will describe the change in support services and the impact of these changes on the person. You could use one person, or two different people, but the examples you describe should have different changes in support services and different impacts. If you do not have a person going through changes, talk to a colleague, team leader or supervisor about a person you can use for this task. 1.
