1 PRE-QUALIFICATION FORM GREEN BUILDING CONSULTANT / SPECIALIST PART A Introduction The purpose of this document is to provide the applicant with necessary information required to complete the form and the submission process. This also provides details of supporting documents that are required as part of this submission. New Construction projects within PCFC/Dubai World Jurisdiction may require the services of independent green building consultants (also referred to as specialist consultant) to carry out the green building facilitation during design and construction. The following sections provide necessary information required for seeking pre-qualification within EHS as a Green Building Consultant Evaluation Process The Pre-qualification of the applicant will be based upon a comprehensive review using some or all of the specific evaluation criteria listed below. 1.0 Experience in one or more of the following areas 1.1 Design Integration & Eco charette 1.2 Experience in whole building integration 1.3 Experience with sustainable design and construction projects following international rating systems such as LEED, BREEAM etc. 1.4 Previous experience that demonstrates success in providing green building consultancy services which are consistent with program schedule, budget, and technical requirements, and are responsive to the specifics of the program 1.5 Simulation, modeling, life cycle costing and life cycle analysis (LCA) 1.6 Experience in energy-efficient equipment design and control strategy optimization 1.7 Multidisciplinary and systems thinking approach to planning, design and construction 1.8 Experience in obtaining / participation in green building certification process. 1.9 Knowledge in building elements, operation and maintenance, FM and training. 2.0 Operating Resources 2.1 Hardware as applicable 2.2 Software as relevant to the activity 3.0 Human / Technical Manpower Resources 4.0 Project Experience / Past Performance 5.0 Organization and Management 6.0 Quality Revision: 07 Feb 2017 Page 1 of 14

2 EHS shall register the Green Building consultants after a thorough completion of the pre-qualification process. EHS as an authority reserves the right to approve or reject any application without assigning any reason and no claim of whatsoever nature in this regard shall be entertained. Documentation to be submitted For New and Renewal pre-qualification applications, the documentary requirements are the following: 1. A valid trade license copy. The activities listed in the license should be consistent with the service for which the prequalification is being sought 2. Location map of their offices in Dubai 3. Organization chart of the core green building facilitation team.(please do not attach the generalized organization chart). Please do not club or indicate the green building commissioning team as part of this organization chart. 4. CV / Resume of the core team in the format provided below 5. Evidence of UAE / Dubai residence visa of the core team i.e. passport copy with valid visa page 6. Copies of academic and professional qualifications / certifications 7. Quality certification and/or quality manual copy where applicable 8. Prequalification / approval from other relevant authorities if applicable 9. Proven track record summary for major green building consultancy works undertaken. Identify certified projects. 10. Declaration regarding conflict of interest in the format provided. 11. Declaration regarding blacklisting / default / litigation with any local or international agency in the format provided. 12. Proof of payment of pre-qualification Application Fee Pre-qualified consultants have the responsibility to act as an independent third party and submit reports directly to EHS in a professional manner consistent with the intent of the prequalification. From time to time, EHS would review and revise the Pre-qualified list of Consultants. Should there be repeated unsatisfactory performance; EHS may remove any consultant s name from the list consistent with the clause 3.7 Disqualification / removal from the list of the procedure EHS-GBP-01 Note Green Building consultants whose pre-qualification has expired are not eligible under any circumstances to carry out the services for the clients.
