In order to assist us with the updating of our records you are requested to complete the attached questionnaire and return it to Ms Yogie Naidoo. 1. GENERAL INFORMATION I. TECHNICAL PERSONNEL Enter the number of employees under each job description for each branch office of the firm. If there are any other employees not mentioned in the list which you feel should be included, please enter them under item Professional Engineers refer to engineers who are registered with the Engineering Council of South Africa (ECSA) as professional engineers. 3.2 Engineering trainees refer to engineers qualified with a Degree or relevant Diploma in Engineering who are still in training and not yet eligible for registration. 3.3 Professional Technologist (Eng) refers to engineering technologists registered with ECSA as professional technologists. 3.4 Registered Technicians (Eng) refer to engineering technicians registered with ECSA as technicians. 3.5 Technician Trainees refer to technical staff in training for registration as Technicians. 3.6 Geologists refer to qualified geologists with a B.Sc (GEOL) degree or equivalent appropriate qualification. 3.7 Surveyors refer to: Qualified surveyors with a B.Sc (Survey) degree or equivalent qualification and registered with PLATO as a Professional Surveyor (Cadastral & Engineering)

2 Surveyor registered with PLATO as a Surveyor or a Surveyor Technician. Surveyor in training 3.8 This item refers to the : Number of personnel in the Financial section with financial qualifications. 3.9 This item refers to : Number of administrative personnel, including secretaries, personal assistants and public relations officers Insert the detail of : Any other personnel in the firm not within the above categories. Specify the categories and provide details Indicate the number of Directors in each office. The Directors must also be taken into account in the categories above in 3.1 to EXPERIENCE Provide the number of personnel per each category of work and within each range of years of experience, i.e. In your firm you have 25 engineers, registered professional engineers and engineering trainees, with experience in geometric design of roads. Group them in the categories of experience in that field, e.g. 8 between 0 and 5 years experience 12 between 5 and 15 years experience 5 with more than 15 years experience. The same categories apply for Technologists and Technicians. Other personnel, for example draughtsman, tracers, surveyors etc will be indicated in the last group OTHER. You may specify these job categories in a separate note, if you prefer to have them specified. 5. PROJECTS COMPLETED Provide a list of the most important relevant projects completed recently for the South African national Roads Agency Ltd (SANRAL), Road Authorities or other Institutions. The following information is required for each project description: Whether the work was performed by your firm only, or in Joint Venture (JV) with another firm or firms. In case of a JV, whether your firm was the senior or junior partner of the JV. The actual project cost or the estimated cost. Whether it was a design appointment or construction supervision or both. Initial or detail assessment appointments will fall under design. 6. SPECIALIST FIELD Your firm may be prepared to offer engineering services in fields other than your specialized field. Please indicate in which fields you are specializing and also indicate what other fields of expertise can be found within your company. Please fill in the team leader(s) in each field with their qualifications and experience in detail. This information would enable us to select the appropriate firm for a particular project.
