1 APPENDIX 2 INVESTING IN CHANGE NGB QUESTIONNAIRE JANUARY 2003 Many thanks for agreeing to participate in this survey. It is important that the views of all recognised governing bodies are heard, and contribute towards the Investing in Change project Notes and Instructions 1. For the purpose of this questionnaire, we have used the term NGB (National Governing Body) to cover all sporting organisations. Please complete this questionnaire even if you would not categorise your organisation as an NGB, but as another type of sports organisation. If you have received this questionnaire you should complete it as the distribution list has been carefully compiled. 2. This questionnaire should be completed by the Chief Executive (CEO) of your NGB. If your NGB does not have a CEO, the questionnaire should be completed by the person who has senior responsibility for the on-going management of the NGB. 3. We are seeking your own views and opinions of your NGB. There is no need to consult colleagues within your NGB to achieve a consensus 'opinion'. This questionnaire is to you as an individual representative of a NGB. 4. Please ensure your answers refer to the Governing Body you are directly responsible for. For example, if your NGB is a home country level NGB, or your NGB is a sub-discipline of a broader sports NGB, ensure your answers refer to your NGB only - not to the UK level or umbrella NGB. 5. Please answer all questions as fully and honestly as possible in order to provide useful feedback. All answers will remain anonymous at all times and responses will be collated and analysed to provide an indication of broad themes. No individuals or NGBs will be mentioned by name. 6.
