1 Job Description: Job title: Location: Children s Healthy Lifestyle Practitioner West Suffolk (Bury St.Edmunds, Newmarket, Mildenhall, Sudbury, Haverhill) (Travel within Suffolk will be required) Hours: 37.5 hours (will incl evenings and weekends) Salary: 18,000 pa plus 1% pension contribution rising to 3% by April 2019 Reporting To: Key Relationships: Children s Service Programme Coordinator/ Children s Service Manager Managers/Client Services Team/Delivery staff/ Bank staff Background information: MoreLife deliver integrated healthy lifestyle services OneLife in Suffolk. MoreLife deliver non-surgical, clinically led weight management programmes for obese adults and children. Our specialist service takes a psychological approach to weight management, including behavioural change, dietary advice and guidance on physical activity. The programme mainly consists of regular group meetings. The team will also be trained to deliver Outreach NHS Health Checks, working alongside the Health Check Team to support delivery of these at events as required. Purpose of job: To ensure the effective set up, implementation and evaluation of the MoreLife children s services in line with the MoreLife delivery model. To provide feedback on the delivery and curriculum in your locality. The practitioner will also ensure the organisation and delivery of all activities on the programmes leading to the provision of a fun, safe and educationally stimulating environment for all participants. The role will also be involved in some promotion of the programme and may be required to engage with a range of stakeholders as well as marketing the programme to the general public to ensure that recruitment targets are met.

2 Responsibilities: Programme Delivery/Development Overview of role: Responsible for the delivery of the children s services; which include family clubs, holiday clubs and school programmes. Following the curriculum, plan the sessions to be interactive and engaging, considering the needs of the clients in the group Facilitate lifestyle & activity sessions that are developmentally appropriate and relevant for all involved Ensure an inclusive, fun, motivating and positive experience for all individuals and fellow staff To ensure the highest standards of programme delivery through performance management and the implementation of effective quality control processes, including adherence to health and safety procedures. Contribute towards the achievement of all local KPIs. Complete the necessary administrative tasks relating to the delivery of the programmes to include confirmation letters, creating monitoring folders and data entry. Monitor attendance on the programmes, contact non-attenders and dropouts, and re-engage with clients on a regular basis. In conjunction with the delivery team, ensure clients receive additional support and catch up sessions over the telephone in relation to the lifestyle sessions delivered on the community programmes. To be accountable for equipment and resources used on the programmes; to check and accept new equipment and resources, ensure all equipment is maintained in a good condition, minimise waste, monitor stock levels, account for breakages and order new supplies when required.

3 Marketing and Engagement Attend stakeholder events. Liaise with the Marketing Co-ordinator to ensure the appropriate marketing tactics are executed at a local level to increase numbers of people referred to the programme. With the support of the central team, ensure there is regular communication to the referrers and participants before during and after every programme. General To positively promote access to the OneLife offer and the aims and objectives of the OneLife service. To organise and engage with local community groups as necessary to enhance engagement and to develop and facilitate networks and opportunities. To competently take blood pressure and the NHS Health Check, to interpret and signpost/refer in line with protocol. To record, monitor and, with the team lead of the service, actively improve on KPIs. Monitor own activity, engagement work, training delivered, campaigns and events, as well as referral numbers to ensure programmes and activities meet necessary needs of stakeholders and commissioners and demand. Collate feedback from our stakeholders and service users. Support production of analytical reports with the focus on furthering the development of the programmes and team. To keep accurate records of work. To identify, reach and engage effectively with target groups/areas/communities in line with KPIs. To attend team meetings and contribute to the performance improvement process and other meetings as requested and appropriate. This includes attending monthly 1:1 meetings with the service lead, and team meetings, with the following focus: Performance Targets understand own contribution to agreed performance targets in line with KPI s; KPI s individual achievement against contractual KPI s; To manage and organise own time and activities responding effectively and in a timely manner to all work. 5 Person Specification: Children s Healthy Lifestyle Practitioner ATTRIBUTE ESSENTIAL DESIRABLE HOW IDENTIFIED QUALIFICATIONS Educated to degree level or equivalent. A relevant qualification in any of the following: Nutrition, Dietetics, Health Promotion, Public Health, Education, Physical Education, Sport & Exercise, Psychology or a related field. Application. EXPERIENCE Experience of working in an office, including administration and coordinating tasks with a level of responsibility. Excellent working knowledge of Microsoft packages, including Word and Excel. Experience of leading relevant physical activity/ weight management classes. Experience of dealing with people with sensitive medical problems. Application/ Interview.
