1 Capstone--Longitudinal PGY2 - Health-System Pharmacy Administration (87405) Faculty: Tyler, Linda S. Site: University of Utah Hospitals Clinics Status: Active Required Description: Preceptor: Linda Tyler Overall rotation duration: 6 months (as a longitudinal rotation) Overall Rotation Description: This rotation is designed to support the resident in meeting all of the goals objectives by the end of the residency year. The rotation will evaluate objectives that will occur during the year on multiple rotations through project experiences that are hard to assess on a given rotation are demonstrated developed throughout the year. If projects have not occured to demonstrate these objectives within the first 6 months of the residency, plans will be developed with specific projects (as applicable) to meet these objectives by the end of the residency.rotation activities group discussions will be held to assist in meeting other objectives, such as writing vision statements. Site Description: University of Utah Hospitals & Clinics (UUHC) is comprised of 4 hospitals, 12 free sting clinics, 16 retail pharmacies, 4 infusion centers, ambulatory clinic services, home infusion service, a comprehensive drug information service. University Hospital is a 490 bed, level 1 trauma center with strong critical care, emergency medicine, surgical services, obstetrics gynecology, neonatal, internal medicine subspecialties, neurosciences, rehabilitation. The University Neuropsychiatric Institute is a 130 bed inpatient psychiatric facility. The Huntsman Cancer Institute consists of a 100 bed inpatient service including an intensive care unit, an ambulatory infusion suite, extensive ambulatory cancer clinics, including bone marrow transplant. The University Orthopaedic Center provides mostly ambulatory care surgery services plus orthopaedic specific clinics. The ambulatory care network includes the Community Clinics located throughout the Salt Lake Valley the four surrounding counties that provide both primary specialty care, plus the specialty clinics infusion room at University Hospital. Role of the Pharmacists The pharmacists in leadership roles that the resident works with on this rotation provide the leadership for the department as well as contribute to the overall leadership of the organization. The role of the pharmacist includes: --Set the vision strategic direction of the business units department consistent with the organizational mission, vision, values, goals. --Drive for strong operational performance including efficiency, financial strength, high value, labor utilization. --Develop implement plans for excellent patient customer experience. --Strive for continued excellence in the quality of services provided including evaluating operations clinical services. 2 --Manage growth of the department to match the organizational goals --Manage change. --Prepare requests for new services resources including FTEs, space, equipment, programs, informatics resources. --Managing day to day operations --Organize prioritize tasks projects. --Delegate work engage team members. --Demonstrate strong problem solving skills. Examples of these situations include: Urgent, emergent disaster situations; day to day activities; actions needed based on reported medication events; actions to improve stardize processes; making strategic decisions on the use of personnel resources --Facilitate team functions. --Manage department finances including capital expenses. --Forecast trends that will impact pharmacy such as rising drug cost, health care policy, technology advances --Support employees by conducting HR functions including recruitment, onboarding, orientation, retention, motivation engagement, coaching development, progressive discipline. --Advocate for the role of the profession of pharmacy both within the organization, the community, at the national levels. Expectation of Learners: Expectations of the resident: Residents will meet with preceptor for a midterm goals assessment (conducted as part of the Q2 development plan). Based on this assessment, the resident preceptor will make a plan for meeting the goals assigned to this rotation.assessments from completed rotations will also be used to make these assessments. Resident may be working with other preceptors in the department to accomplish the goals. During the second half of the year, the resident should touch base with the preceptor monthly to discuss progress on goals. Another assessment will be conducted at the Q3 development plan discussion. In the last 3 months, the remaining goals will be conducted. The last 3 months, the resident will schedule meetings with the preceptor for topic discussions to complete the remaining objectives. Pre-rotation preparation: No preparation is needed for this rotation. It is expected that all evals are current to get a clear picture of what goals are signed off which have not so that an accurate assessment of progress to goals can be done. Typical daily/weekly/monthly activities: The first 3 months of the rotation, the resident will touch base monthly to discuss progress to goals. The last 3 months, the resident will set up at least monthly topic discussions to complete the remaining goals objectives. Projects/Presentation Description: Residents will be assigned projects to meet goals as needed. As much as possible, it will combined with existing rotations to be sure all goals objectives are met by the end of the year. Evaluation: Page: 2 of 6

3 There is an initial evaluation, a custom midpoint combined with Q3 assessment, a final summative evaluation combined with end of year evaluation discussion. Prior to the final evaluation meeting, the resident needs to complete the self-evaluation, evaluation of learning experience, preceptor evaluation in PharmAcademic. The preceptor will conduct the final evaluation in PharmAcademic. The resident needs to be sure to use the template for evaluations ( in preparing their evaluations. Resident progression: Residency midpoint: meet with preceptor assess progress to goals. Develop a plan to meet goals assigned to this rotation if they have not already been assessed. Be sure all evaluations are current. Month 1: Touch base with preceptor discuss progress on goals assigned for this month. Month 2: Touch base with preceptor discuss progress on goals assigned for this month. End of Month 3: Meet with preceptor for Q3 development plan conduct midpoint for this rotation. Sign off goals objectives completed to date. Update plan to meet remaining goals. Month 4: Set up topic discussions to meet assigned goals. Objectives in R2.1, R2.2, R2.3, R2.4,R3.2, R3.3 R3.4 should be completed or in progress. Many of the R2 goals are completed as part of project developing the budget. R3 goals are usually completed as part of recruiting for residents developing a recruiting plan for additional staff. Month 5 6: Set up topic discussions to complete remaining goals over next 2 months. The remaining goals to be signed off are in R6. These include developing a vision statement, identifying management issues to be studied. Have a clear plan for any projects necessary to complete goals objectives. End of Month 6: complete final evaluation as part of end of year assessment. During the year, residents will have many opportunties to network at Leadership Conference, hospital visits, Mountaion States conference, Midyear. Residents will have the opportunity to participate in national committees that will also develop their network. Residents will have a network of current past residents preceptors to launch their careers. Throughout the year, residents will identify problems that need to be solved. It is expected that residents bring these issues to light participate in solving them. In addition, some will be areas that lend for further research.
