4 PRIMARY PURPOSE: Responsible for contacting Green Prescription (GRx) Patients with a variety of medical conditions, to guide planning for appropriate lifelong physical activity for wellness. People with heart disease, hypertension, diabetes, depression, obesity, arthritis, cancer and some respiratory conditions or at risk cardiovascular disease (CVD) are most likely to benefit from a GRx. Their condition must be stable before they are prescribed physical activity. Unstable angina, uncontrolled heart failure or acute exacerbations of asthma must be stabilized. DUTIES AND RESPONSIBILITIES: Key Tasks Contact patients referred to the GRx programme within 7 days from the date of receiving the script and provide appropriate individualized physical activity recommendations, motivation and support. Follow up GRx patients at regular four weekly intervals to support and assist with their physical activity plan for 4 months (via face to face clinics, phone support, group settings). Accurately maintain patient records, complete data entry, administration tasks and reporting (as applicable) Ensure effective promotion of GRx programme in conjunction with the Team Leader. To achieve key performance indicators in line with the Sport Northland and the GRx targets. Report monthly to the Green Prescription Team Leader on GRx achievements as identified within the Management Plan. To fully support the philosophy and culture of the Trust and where practical support other Trust programmes. Undergo professional development training opportunities provided by Sport Northland when and where necessary. Undertake patient surveys (including the 10,000 Steps Northland telephone survey) and research as required. Operational Expenditure Authorised to sign Contracts No No Responsible For: Budget $0

5 PERFORMANCE MANAGEMENT Annual performance for this position will be measured against a Performance Management Plan developed specifically for the individual. The plan will include the following elements: Key Performance Indicators - targets for what you are required to achieve Expected Standards based on how you go about doing your job Career Aspirations - to identify which direction you might want to go in the future Individual Development Plan - to work on areas of personal development Monthly coaching will be undertaken by the Green Prescription Team Leader in order to maximize the effectiveness of the Performance Management Plan. In addition to the above, an annual performance review and a separate remuneration appraisal will also be undertaken by the Green Prescription Team Leader. 6 Competency Skill level Performance level required Technical Leadership Proven leadership qualities in physical activity and assisting people in achieving their goals Specialist expertise for the position Knowledge and understanding of (or the ability to quickly learn): Benefits of physical activity for health Appropriate tertiary qualifications and industry experience i.e. Training/qualification in human anatomy and physiology An understanding of the basic principles of exercise prescription and an ability to apply these to physically inactive people A knowledge of the various lifestyle related medical conditions An understanding of the precautions required when recommending physical activity options to people with medical conditions An understanding of Motivational Interviewing for Behaviour Change Have a broad based understanding of Maori values, knowledge and tikanga Knowledgeable about, and preferably have established networks with iwi, hapu groups and Maori organisations in Te Taitokerau Have an understanding of the Treaty of Waitangi and its importance Computer literacy Proficient in Microsoft Word, Outlook, Excel, Access General Local community, sport and recreation networks Time Management People empowerment Medium Strong awareness of recreation / physical activity providers and health related networks in the local mainstream and Māori community ly developed organisational/time management skills The ability to work flexible hours including evening and weekend work if required Proven skills in the empowerment of people the ability to bring the best out of a wide range of people General Medium An understanding of the strategic planning process and experience in developing and implementing business / annual plans Full drivers licence Current first aid certificate

7 JOB SIZE SCORE Sport Northland has undertaken a job sizing process in which positions have been sized according to five areas: Accountability, Work Complexity, Responsibility for People, Relating to Others, Expertise. Each position is sized on these areas giving an overall grade. This grade relates to the total remuneration scale for that size job in the Not for Profit sector. 9 GRx Patient Support Standards and Professional Development Model GRx Induction Unique to GRx and essential for all new PSP - contains principles of GRx, Behaviour Change and Motivational Interviewing skills. Exercise Prescription for Health e.g. Netfit Active Living & Wellness, Sport and Exercise Qualification may provide this knowledge if they have a spefic exercise prescription for healthy unit / module. Anatomy & Physiology e.g. Netfit A & P Appropriate A & P is the basis to progress from. Nursing, Physiotherapy, Sports Studies, PE Teacher are some of the qualifications that provide this knowledge. All 3 elements combined are the MINIMUM standard for working as a GRx PSP
