2 Award * Healthy Workplace Recognition Purpose of the Award The Healthy Workplace Recognition Award acknowledges workplaces in Simcoe County and the District of Muskoka. These employers and employees have demonstrated a commitment to workplace health by promoting and supporting healthy lifestyles for their employees. The Award The annual recognition award consists of a framed certificate for your workplace. It will be given to those workplaces that meet the criteria in either the Bronze, Silver or Gold categories. How to Apply 1. Register your workplace by completing an Award Application/Consent Form. Send to Julie Cloutier, Good For Life Project Coordinator by May 01, Choose activities that best meet the needs of your employees. Record activities. Track activities completed using the summary tracking form. Compile evidence of these events in a supportive binder for submission. 3. Submit your completed tracking forms and binder by September 15, Please include additional details, digital pictures, evaluation and support materials. 4. Upon fulfillment of the award criteria, as determined by an independent review panel, your workplace will receive the Healthy Workplace Recognition Award. The fourth Annual Awards Event will be held on Thursday November 5, 2009 at the Royal Victoria Hospital in Barrie, Ontario. 5. Participants may apply for the Healthy Workplace Recognition Award annually. Organizational Wellness Support: for example healthy eating guidelines for meetings, flexible hours policy Note: The SCWWN would like to recognize all workplace applicants, and will therefore offer an Award of Merit to workplaces who are making progress towards reaching their goals. STEP TWO: CREATE YOUR SUPPORTIVE BINDER Include concrete examples of activities, such as pictures*, promotional materials and evaluation feedback. Include a copy of the summary tracking form for each wellness activity at the front of the workplace supportive binder which demonstrates your workplace activities. Submit by September 15th, *If including pictures, please provide copies of pictures on a disc. STEP THREE KEEP TRACK OF YOUR ACTIVITIES Use the attached award summary tracking form, for each activity, to indicate the Healthy Workplace Award Topics and Health Promotion Level achieved by your workplace. A sample form has been included to help with this process. 4
