3 Job Description Purpose of Role The Inclusion Officer for Pupils with Medical Conditions, under the direction of the Inclusion Manager, will assist with the delivery of key service strands within the Children and Families Service. The role will support the Team Manager in managing statutory and non-statutory responsibilities with regards to supporting pupils unable to attend school because of medical issues. Working with the Team Manager for Inclusion and other partners, particularly schools and health, the role will provide a lead in key areas such as identification, tracking and monitoring of the provision, allocation and guidance across Southampton. Working closely with schools and health colleagues the role will develop a robust multi-agency process and policy that supports pupils through effective identification and review. The role will also oversee the allocation of education through a commissioned service and work with all parties to review appropriately in line with the pupils needs. Key Accountabilities 1. Support the development and co-ordination of the education provision for pupils with medical needs 2. Organise, develop the procedures and processes ensuring the local authority to ensure local authority requirements for children who are unable to attend school because of medical conditions are met. 3. Responsible for safeguarding and promoting the welfare of children at all times 4. To promote the continuity of education for pupils through effective 5. To ensure commissioned services meets the needs of the child and the requirements of the National Curriculum, including forward planning of the curriculum delivery and public examinations through effective reviewing processes 6. Maintain close liaison with health colleagues and the home school to ensure maximum continuity of study; including access to public examinations. 7. Review and monitor the child s progress as part of the normal cycle health care plans. 8. Write progress reports either termly or at the end of tuition alongside other key partners, parents and the pupil 9. Contribute to any relevant EHCP applications and further assessments needed in the interest of the pupil 10.Representing the local authority in attending and contributing to pupil reviews and professionals meetings 11.Liaise professionally, as appropriate, with staff at designated schools or other agencies in order to ensure continuity of educational provision. 12.Keep accurate records, including keeping accurate attendance records, Lesson records should be returned monthly to the MTS office and attendance sent in daily. 13.To network with other authorities and share best practice. 14.To offer training and development to schools, professionals and families around best practice and ensure all guidance and processes are up to date with current legislation

6 Person Specification Education/ Qualifications Essential Level 3 qualification (BTEC, NVQ or equivalent in Health, Social Care, Early Years) and relevant experience within the Education Welfare Service, schools, and/or working with children and families or willingness to undertake professional development Knowledge, Experience and Skills Essential Previous experience of working with children and families in an Education / Care / Youth / Early years setting (paid or voluntary). Knowledge of how schools function, policies, statutory regulations and guidance in relation to supporting pupils with medical needs and the LA statutory function. Knowledge of relevant policies, statutory regulations and guidance and practices and their implications, in relation to supporting pupils with medical needs who are unable to attend mainstream schooling. The ability to offer both challenge and support to various key partners in often, some difficult situations. Ability to show a high level of experience working with families and young people. Previous experience and proven record of an ability to negotiate and act as advocate on behalf of children and families and an ability to liaise with professionals in schools and other agencies. Proven ability to record observations and write clear and concise reports and record information to agreed agency standards. Undertake proactive observations and write reports when required including statements of evidence for court (magistrates and family proceedings court). An ability to manage a varied caseload, and at times deal with challenging and confrontational behaviour, and a skill to diffuse these potentially confrontational situations. Proven experience of working with a range of professionals on work relating to children and families. Experience of liaison and negotiation with schools and other agencies. Ability to advise a range of colleagues and families on matters relating to the supporting pupils with medical needs who are unable to attend mainstream schooling legislation and processes and the wider inclusion agenda. Ability to communicate clearly with children, adults and facilitate family meetings and lead on meetings in relation to supporting pupils with medical needs who are unable to attend mainstream schooling. Ability to collate and interpret statistical data and present them to a variety of audiences. Ability to communicate clearly, concisely and persuasively in writing and orally. Good presentation skills to a variety of audiences.
