The ollege dheres to ll pplible federl nd stte lws, rules nd regultions governing the dministrtion of these types of leve s it reltes to full time employees. Full time employees re eligible to reeive vtion, sik, nd/or personl leve bsed on their respetive position lssifitions nd reditble yers of servie. The President or designee(s) my estblish progrms nd proedures to implement this poliy, to inlude determining the disposition of unused vtion leve in exess of the mximum rry over. 2 Proedures: I. Generl Provisions for ll full time employees A. A request for pplible leve shll be initited by the eligible employee on Certifite of Absene (COA) form through the eletroni leve system nd should be submitted prior to the effetive dte of leve for supervisor review nd pprovl bsed upon deprtment needs, when possible. B. A full time employee who is unble to perform their job duties t the College nd hs to be bsent from work due to personl sikness, ident disbility, extended personl illness, medil or helth relted ppointments, or beuse of illness or deth of the employee s lose fmily member or member of the employee s household, shll be eligible to tke rued sik leve. An employee who finds it neessry to be bsent due to illness or emergeny shll provide s muh notie to the immedite supervisor before the beginning of the dy s work on the dy of the bsene or s soon s possible when the notie nnot be mde in dvne. This notie my inlude the employee s bility to provide the supervisor the ntiipted durtion of the bsene. Upon the employee s return to work, the employee shll immeditely file the COA form, whih sttes the reson for the bsene nd shows knowledgement by the employee s immedite supervisor. A supervisor my submit COA on behlf of n employee who is unble to omplete the request themselves before the end of the py period dedline. C. Vleni College s Orgniztionl Development nd Humn Resoures (ODHR)/Pyroll Offie mintins the offiil reord for vtion, sik, nd personl leve blnes. Vtion, sik, nd personl leve my only be used fter it hs been rued nd doumented s erned by ODHR pyroll systems. D. At the time of seprtion from employment or trnsfer to position tht is not eligible for one or more leve types (i.e. sik nd/or vtion), erned nd unused leve shll be pid to eligible employees or to the eligible employee s benefiiry or estte, in the se of n employee deth in ordne with estblished proedures ssoited with College Poliy 6Hx28: 3F-02 Terminl Py for Full Time Employees. Leve for Full-Time Exeutive Mngement, Administrtors, 12-month instrutionl employees, stff, nd grnt funded positions (12-month instrutionl employees re librrins nd ounselors filling 12-month positions) A. Vtion Leve 1. Vtion Leve Provisions Full-Time exeutive mngement, dministrtors, 12- month instrutionl stff, nd stff positions ern vtion leve bsed on yers of servie nd rul guidelines. The sheduling of vtion leve is subjet to supervisory pprovl nd it must be sheduled so tht there will be minimum disruption to College nd/or deprtmentl opertions. b Employees in full time grnt funded positions shll not rry over more thn ten (10) dys of vtion leve into new lendr yer. Vtion leve redits erned in exess of ten (10) dys in ny lendr yer shll be used tht lendr yer or be forfeited t the end of tht lendr yer unless the grnt inludes funding for dditionl dys nd is pproved by the president or designee(s). An employee shll not rue ny dditionl vtion leve during ny periods of pid leve tken between n employee s Lst Work Dte nd Dte of Seprtion s refleted in the Employee Seprtion Form (ESF) or its equivlent s filed by n employee s supervisor or designee. 2. Vtion Leve Guidelines Eligible employees ern vtion leve for eh lendr month in whih the employee hs worked or been on pproved pid leve for the mjor frtion of the lendr month. The vlue of this leve shll be lulted by multiplying the unused rued vtion leve in exess of the mximum rry over by the employee s dily rte s determined using the proedures ssoited with College Poliy 6Hx28: 3F- 02 (Terminl Py for Full Time Employees) effetive Jnury 1 of the lendr yer the ontribution shll be mde. The leve time trnsferred from vtion to sik leve is non-ompensble nd shll not be used in the lultion of terminl py for unused sik leve. e For employees designted s dministrtor or 12-month fulty nd stff, unused rued vtion leve in 6Hx28: 3D-03 4 of 8

5 B. Sik Leve exess of the mximum rry over shll be trnsferred to the employees sik leve ount effetive Jnury 1 of the new lendr yer. The leve time trnsferred from vtion to sik leve is non-ompensble nd shll not be used in the lultion of terminl py for unused sik leve. 1. Sik Leve Provisions Eh eligible full time employee shll ern one (1) dy of sik leve with ompenstion for eh lendr month or mjor frtion of lendr month of servie. One-hlf (1/2) of the work dys in month or more shll be onsidered mjor frtion of tht month. Sik leve shll be umultive from yer to yer. b When rued sik leve is used by the employee, it shll be deduted from the vrious tegories of sik leve in the following order until exhusted: i Sik leve onverted from exess vtion leve; ii Sik leve erned on or fter to July 1, 2001; iii Sik leve erned prior to July 1, 2001 Aumulted sik leve my be trnsferred from nother Florid publi institution of higher edution but will only be redited s sik leve is rued t the ollege; speifilly, s n employee erns one (1) dy of sik leve eh month t the College, they re lso redited with 1 dy of trnsferred sik leve. d The employee shll not rue ny dditionl sik leve during ny periods of pid leve tken between n employee s Lst Work Dte nd Dte of Seprtion s refleted in the Employee Seprtion Form or its equivlent s filed by n employee s supervisor or designee. 2. Sik Leve Guidelines After three (3) onseutive dys or five (5) dys in thirty (30) lendr-dy period of bsene, the supervisor or designee my require tht n employee furnish medil ertifite signed by qulified helth 6Hx28: 3D-03 5 of 8

6 re provider tht douments the reson for the bsene nd ertifies tht the employee is ble to return to work. This doumenttion shll be obtined by the employee t their own expense. After the supervisor or designee hs reeived nd evluted the supporting medil doumenttion, the medil doument(s) shll be forwrded to ODHR to be mintined in seprte employee reord. b For deth of the employee s lose fmily member or member of the employee s household, sik leve my be used for mking funerl rrngements, trvel, ttending funerl servies, nd/or relted mtters. The deprtment my require tht n employee furnish supporting doumenttion tht substntites the reson for the bsene. 3. Personl Leve Provisions Personl leve is for personl mtters other thn illness of self or fmily member nd the leve is hrged ginst sik leve blnes. b A mximum of four (4) dys (32 hours) per fisl yer my be hrged ginst sik leve for personl resons. Unused leve for personl resons shll be nonumultive. Employees who re pre-pproved for personl leve nd do not hve suffiient sik leve blnes t the time of usge to over the bsene request, shll only reeive ompenstion for the hours equl to the rued sik leve blne t the time of bsene. The remining hours my be designted s n unpid leve of bsene; or in the lterntive, n employee my request n lternte form of pid leve (i.e. vtion leve) subjet to supervisory pprovl nd/or deprtmentl needs. III. Leve for 4-, 8-, nd 10- month full time instrutionl employees A. Vtion Leve 1.
