CHIEF TRANSFORMATION OFFICER POSITION SUMMARY: The Chief Transformation Officer (CTO) reports to the Chief Information Officer and is a valuable member of TransForm s senior leadership team (SLT). The CTO is accountable for ensuring effective strategy and operations are in place to have TransForm deliver on the various clinical transformation initiatives that our customers undertake. This role will involve extensive collaboration with senior operations, practice, and medical leaderships, front-line clinicians and support teams across TransForm s customer base, to advance the utilization of clinical information systems and to transform clinical practice consistent with the goals and objectives of our customers regional clinical and strategic initiatives. Additionally, the CTO will provide leadership and guidance to align clinical information systems with practice and operational processes, targeted to support excellence in care delivery across TransForm s customers. A key aspect of the CTO role, within TransForm, is to be the operational lead for our hospital customers, as they undertake a major change process associated with the replacement of their hospital information systems (HIS). The CTO will provide overall leadership associated with all clinical and technical disciplines in the design or re-engineering of clinical work in readiness for the adoption of advanced clinical systems. The future regional HIS will be designed to support efficient, effective, safe and high quality patient care and hospital operations. The successful candidate will manage this high risk and complex project from initiation through to completion and will be responsible for the design and implementation of operational support plans to ensure sustainable use of the system and ongoing adoption of new processes.

2 ACCOUNTABILITIES: Clinical Transformation Strategy Develops vision, goals and objectives for a clinical transformation strategy, in alignment with the priorities and strategic objectives of TransForm and its customers. The CTO will proactively engage with internal and external stakeholders to identify their business challenges, opportunities, and goals, which clinical transformation could enable. CTO would lead the development of a shared strategy and roadmap that uses (existing or proposed) ehealth, LHIN, and/or Hospital programs and solutions. Further, the CTO will play a leadership role in ensuring that stakeholders progress from a continuum of heightened awareness through to understanding, involvement and ultimately adoption and ownership of these initiatives and the expected outcomes. The CTO would collaborate with the CIO to execute the clinical transformation roadmap; this will include, for example, securing funding sources, developing business cases, and identifying stakeholders to champion the initiatives. The CTO would lead, plan, facilitate and monitor the implementation of initiatives from the roadmap. Maintains awareness of provincial and international clinical transformation strategic directions, including IT and Clinical initiatives. Collaborates across TransForm customer base to ensure proposed solutions and technology initiatives align with industry standards, health care trends and best practices (eg: HIMSS) HIS Renewal Provide leadership, guidance and facilitation, as evidenced through problem solving, decision making, conflict resolution, prioritization, and the advancement of the HIS renewal project on schedule through its milestones to project conclusion. Responsible for the portfolio coordination with other key leaders and staff who are supporting clinical engagement and operations, organizational change and risk management, communications, and other disciplines. Ensures alignment with other key organizational initiatives. Responsible for development and execution of the HIS renewal project execution strategy, plans, and timelines and related HIS renewal project budget, in collaboration with key stakeholders. Oversee and coordinate the various initiatives of the HIS renewal project. Leading the project team, identify, facilitate, monitor, and manage the project tasks of the various components of the HIS renewal project including identifying project risks and establishing appropriate mitigation plans. As a member of the core HIS renewal leadership team establish an organizational structure including lead positions in project management, technical expertise, clinical leads, systems architecture, governance, and (potential external) consultants. The CTO will work closely with the Chief Medical Information Office in providing leadership and direction to the clinical change management teams. Job Posting Chief Transformation Officer 2

3 The CTO will work closely with the Chief Information Officer to jointly manage the project implementation, including project s scope, schedule, resources, risks and budget. CTO would lead the design and execution of an outcome realization plan; also including an outline of quantitative and qualitative returns on investment. Essential to the success of this role will be an investment of time, energy and resources to building collaborative working relationships with clinical and corporate services teams to undertake this transformation initiative. Corporate Participates as an active member in strategic planning, policy development and review, establishing priorities, allocating resources, and on committees as appropriate Participates in Canadian and global clinical transformation and informatics bodies, standards organizations and best practice forums Financial and Human Resources Management Develops and monitors operating plans and budgets for designated projects and ongoing operations Promotes job performance, job satisfaction and the optimal self and professional development of immediate subordinates Defines resource and skill mix requirements necessary to support initiatives Exhibits strong coaching and mentoring skills with proven track record of developing high performing teams and team leaders Communication/Mentoring Fosters an environment of learning and development for direct reports and peers, recognizing and rewarding contributions Actively participates in appropriate staff meetings and leadership events Establishes collaborative partnerships with practice, operations and other key stakeholder groups EDUCATION/SKILLS Master s of Business Administration or an advanced degree in a related field Management experience of at least 10 years in a related field Solid foundation in Information Technology, Clinical Informatics, and Project Management Extensive experience leading, planning and managing multiple, complex projects involving major changes to business and clinical processes, workflow, and preparation of staff and/or stakeholders for the changes Over 10 years of proven technology management experience with a focused responsibility supporting Clinical Information Systems Job Posting Chief Transformation Officer 3

4 Demonstrated ability to effectively prioritize and execute tasks in a highpressure environment Superior interpersonal, change management, project management, team building, communication, negotiation, decision-making skills, conflict resolution and critical thinking skills Extensive experience working directly with senior administrative and medical leaders to support information and analysis required to inform organizational planning, manage performance, identify improvement opportunities, and make decisions Highly effective influence, negotiation and collaboration skills with the ability to work across the enterprise. Strong financial management skills to establish controls, track resource/costs and provide good business solutions.
