1 JOB DESCRIPTION JOB TITLE: DEPARTMENT: RESPONSIBLE TO: TENURE: Community Development Lead Coach4care Spiritual Care Spiritual Care Lead 6-8 Month Fixed Term Contract Primary Job Purpose The development and implementation of a training programme for ex carers to support current carers involved in the day to day support of a relative, friend or neighbour nearing the end of life. The role involves identifying potential ex carers and current carers, matching ex carers (once trained) with carers and, providing ongoing support for ex carers and maintaining contact with current carers. Background St Christopher s has an opportunity to roll out a new development in end of life care referred to as Coach4care. Coach4care was designed as a prototype in a programme called Better Endings ( which focused on creating new solutions in end of life care led by the Innovation Unit. Coach4Care aims to improve support family or informal carers of people with a serious or life threatening illness. Designed in partnership with carers and professionals it offers an interesting and new approach to drawing on the experience and expertise of past family/informal carers to support those coping with the challenges of this role in the present. This new project draws together an exciting team comprising: 1. the service designer who led the development of the prototype (hereafter referred to as the designer ) 2. a professional coaching organisation called Animas ( 2gIVb5PtCh0IhAvxEAAYASAAEgJMe_D_BwE) that is interested to train the excarers in coaching methodology, 3. professionals from St Christopher s who are interested to educate and train current carers around issues pertaining to the end of life St Christopher s is keen to draw in carer organisations from across SE London also, as a source of referrals of carers who are interested in additional support or could offer support to other carers. Principal Accountabilities 1. To connect with a variety of carer, healthcare and other organisations locally who will already have relationships with carers who could benefit from the project and might be interested to take part 2. To identify and connect with current family/informal carers of people who face serious or terminal illness (hereafter referred to as family carers), to ascertain and encourage

2 their interest in seeking additional help and support that will enable them to sustain their role as carers 3. To identify people who have been family carers of someone who died (hereafter referred to as ex carers) who are interested to use their previous experience to support someone who is currently serving as a family carer 4. To continue to explore current carers needs for training and support, augmenting early thinking led by the designer as part of the prototype development 5. To work with the service designer, professional coaching organisation and education team at St Christopher s to design and provide a programme of training suitable for ex-carers. This programme will draw on the skills of Animas, professionals within the hospice and other related projects in response to the requirements of carers 6. To match current carers with ex carers once their training is complete 7. To establish regular opportunities for ex-carers to get together and reflect on their experiences of supporting a current carer in their role, working closely with hospice colleagues. We anticipate that this will be fortnightly or monthly in frequency. Where ex carers are struggling with their role, to discuss with hospice staff and Animas to agree any additional support required 8. To maintain regular contact with current carers to monitor their experience of coaching, in order to guide the designer and the hospice about its development 9. To connect with other community participation projects at St Christopher s in order that Coach4care informs the overall strategy for the organisation around community engagement and participation

Safeguarding/MCA and DoLS Staff are required to take responsibility in all areas of work for safeguarding people at risk (both adults and children) from any form of abuse or neglect. When delivering care and support, staff are required to comply with the requirements of legislation relating to care in the UK, including the Care Act (2014), and Mental Health Capacity Act 2005, which incorporates the Deprivation of Liberty Safeguards (DoLS). In order to achieve this, all staff are required to read St. Christopher s Safeguarding Policy and Procedure and to participate in both regular training and training updates and to incorporate preventative strategies into their work, as applicable. To maintain professional registrations (where applicable) and to keep up to date with mandatory training and other training requirements within own area of specialism/responsibility. 4 PERSON SPECIFICATION: COMMUNITY DEVELOPMENT LEAD COACH4CARE Qualifications Essential Desirable Degree level or equivalent Community development qualification Project management qualification Knowledge and Experience Essential Desirable Significant experience of community development as part of a previous role Significant level of experience of monitoring project delivery and project expenditure against targets Experience of working with volunteers Experience of communicating new ideas and/or services to diverse communities Significant experience of working autonomously and managing relationships with subcontracted agencies Experience of working with people who face a terminal or life limiting illness Skills and Competencies Essential Desirable Excellent interpersonal and communication skills in order to interact with professionals and members of local communities Proficient IT and social media skills, for example, able to send and receive messages, upload information onto a website, create documents such as agendas, use social media to communicate with stakeholders Experience of delivering training Proven project management skills, for example able to establish a project plan, identify resources needed, engage others in its implementation Resilient with ability to navigate distressing emotions and support others Good knowledge of the principles of community development Understanding of the multifaceted needs of individuals with life limiting conditions, their families and carers Awareness of challenges and opportunities of working with local community groups to effect change and development Persuasive and able to use knowledge to influence others. Understanding of the challenges and opportunities of working with volunteers Experience of establishing and reporting to a senior management team/board/steering committee
