4 Skilled Assessor Agreement - Frequency Purpose: Data Quality - assumption that those who complete assessments more frequently will be more comfortable with process and complete full assessment Access to information no one else in our system has access to Measure: Complete Assessments Entry into Project VI completed

5 Skilled Assessor Agreement - Quality Purpose: Quality data leads to better matches and more people in housing Measure: Complete Assessments Entry into Project VI completed Correct EDA Provider Missing Data

6 Skilled Assessor Agreement - Observed Disabilities Purpose: Build in tool so those who do not have insight into their conditions are not penalized for this lack of insight They can still meet chronic homelessness definition (requires disability) and be prioritized for supportive housing interventions Measure: Once matched, is person with observed disability eligible for program that requires disability?

7 Common Challenges - EDA Mode We have two different workflows Do you need to use EDA Mode? When logged into ServicePoint, look at your provide on the top left of your screen:

8 Common Challenges - EDA Mode If your provider is Chicago Coordinated Entry System (CES) - Skilled Assessors Project, you do NOT need to use EDA Mode. Continue to completing assessment If your provider is anything else, you must use EDA Mode before completing an assessment

10 Common Challenges - Creating an Entry There are only two places you should complete an assessment Entry Interim Review Assessments must be completed through an entry into the Skilled Assessors Project. An assessment created elsewhere will not count as a complete assessment, and more importantly, may prohibit the household from being matched.

20 Data Collection Improvements Chronic Homelessness In order to meet HUD s definition of Chronic Homelessness, the Head of Household must: be residing in an emergency shelter, on the street/place not meant for human habitation, or in a Safe Haven AND have a disabling condition AND been continuously homeless for the last twelve months OR have a minimum of 4 occasions of homelessness over the past 3 years, totaling a minimum of 12 months.

21 Data Collection Improvements Chronic Homelessness Occasions are broken up by breaks. A break is: having been in a place meant for human habitation (a friend s couch, a hotel room, etc...) for a period of at least 7 nights OR having been in an institutional setting for a period of more than 90 days.

22 Length of Homelessness Please do not ask these questions directly to the client Instead, create a housing history for the last three years and then answer questions based on the history obtained

24 HMIS Chronic Homeless Questions Where did the client sleep last night? How long did they stay there? When did this episode of homelessness begin? How many episodes of homelessness has the client experienced in the past three years? How many total months has the client experienced homelessness in the past three years?

Q: For victims fleeing domestic violence, does the file get locked automatically if the first preliminary eligibility question is answered as yes or current housing status question answered as fleeing domestic violence? A: The client s record will never be locked automatically. You need to lock the record or contact hmis@allchicago.org if you are unable to lock the record. Q: If you we see that a standard housing assessment has been completed recently and the information is incorrect what should next steps be? A: Complete an interim review update assessment using the procedure showcased here: Q: If we are to correct mistakes or update info, in which cases is an interim review appropriate versus changing/adding info directly to the original assessment? A: Updating the original assessment is almost never advisable. Generally better to use the update workflow.
