1 NEW EMPLOYEE CHECKLIST-ADMINISTRATIVE (BENEFITS) This resource is to assist new employees with the new employee onboarding process. Please forward questions to the Office of Human Resources, or BEFORE START DATE These are tasks that are recommended to be completed prior to your start date. Also included is a list of items/activities/events that may be a part of your onboarding. Review and Acknowledge New Employee Orientation presentation Important University Reminders document New Benefits Orientation presentation Complete New Hire Paperwork Completion of these forms is required. New Employee Information form Form I-9 Gather documentation to present for Form I-9 (see list of documents in your onboarding materials) Direct Deposit Authorization form Form W-4 Agreement to Assign Rights Optional New Hire Paperwork Completion of these forms is not required. Voluntary Self-Disclosure Information form Employee Payment Consent form Twelve Month Payroll Redistribution Plan (9-month employees only) Receive a follow up from your supervisor regarding:

2 Work schedule Attire expectations Specialized apparel and footware, as needed Start time on start date and where to go Specialized needs (e.g. foot rest, sensitivity to fragrances) ON START DATE These are tasks that are recommended to be completed on your start date. Also included is a list of items/activities/events that may be a part of your onboarding. Visit Human Resources with any remaining new employee paperwork AND your Form I-9 documents Klamath Falls: 111 Snell Hall Portland-Metro: Trish Hower, Room 2 Salem: Tina Clarke Pickup Employee ID Card Klamath Falls: Visit the College Union, next to Snell Hall, after you visit HR Portland-Metro & Salem: Check in at 111 Snell Hall when you next visit the Klamath Falls campus Benefits Benefits enrollment must be completed within 30 days of hire. It is suggested that enrollment be completed and submitted by noon on the last business day of the month in which you started. Remember to sign up for benefits Enrollment Form Building Access Klamath Falls: Pick up key fob (Facilities Services building) Portland-Metro: Pick up prox card(s) from Trish Hower Salem: Check with your supervisor Review and sign position description Learn about your supervisor's management style Learn about the performance evaluation process and timeline Learn about the department structure

3 Work with supervisor to identify performance goals to be achieved during the next six to twelve months Receive a copy of the schedule for your first week Introductions to team, department, customers, and key contacts Tour of the building/facility Tour of the campus Learn about department policies and procedures Complete any trainings that are required immediately Receive an introduction to IT sytems Receive a copy of your training schedule FIRST WEEK These are tasks that are recommended to be completed during your first week. Also included is a list of items/activities/events that may be a part of your onboarding. Reminders: Complete or sign up for required trainings (check in with your supervisor) Remember to keep track of questions and concerns you have and to share them with your supervisor Klamath Falls: Remember to buy parking permit Portland-Metro: Obtain parking pass from Trish Hower Salem: Check in with your supervisor Remember to sign up for benefits, if you haven't already One-on-one meeting with your supervisor, may include discussing how your first week went and reviewing schedules More introductions, as needed Learn how your role fits within the structure of the department Discuss any questions or concerns you may have Questions to make sure you have the right tools do your job FIRST MONTH These are reminders to complete required tasks if you have not already done so. Register at Complete FERPA training, if required Klamath Falls: Purchase parking permit, if you haven't already Sign up for benefits, if you haven't already Weekly or bi-weekly meetings with your supervisor Learn about the department's strategies and goals and how they relate to university goals Work with your supervisor to set long- and short-term goals Departmental event to welcome you Invitations to Oregon Tech campus-wide events Information on various community outreach and involvement opportunities FIRST THREE MONTHS Work with your supervisor to establish a training schedule for you that will help you and the department meet future goals Invitations to Oregon Tech campus-wide events Review performance goals with supervisor Discuss issues and challenges with supervisor, work together to find ways to resolve them Information about networking opportunities Information about community outreach/involvement opportunities FIRST SIXTH MONTHS Review performance goals with supervisor Work with your supervisor to establish future performance goals for the next six months

5 Celebrate your successes and achievements FIRST YEAR Information about getting involved with campus committees and cross-functional teams, as appropriate Work with your supervisor to establish performance goals for the following year Celebrate the successful completion of your first year Receive a request to provide your input about the department's onboarding process - keep track of items or activities you feel should be improved and how they can be improved Work with your supervisor to complete your first formal annual performance review Work with your supervisor to discuss professional development goals and identify relevant learning opportunities
