1 JOB DESCRIPTION Job Title: Salary: Hours of Work: Adult Carers Development Worker 22,169 per annum pro rata 35 hours per week This may include working some unsociable hours including evenings and weekends. Contract Type: Fixed-term until 30 th June 2018 Qualifications Required: A minimum of an SVQ II in Social Care BACKGROUND PKAVS is a diverse charity that improves the quality of life for the people of Perth & Kinross through four main service areas - Carers; Mental Wellbeing; Minority Ethnic Communities and Voluntary Action Perthshire. PKAVS advances the lives of thousands of disadvantaged local people every year, as well as supporting, promoting and developing the wider Third Sector in Perth & Kinross. PKAVS Carers Services improves the quality of life for unpaid Carers of all ages, including young carers and young adult carers. The service supports Carers to achieve positive outcomes relating to their own personal wellbeing and development and supports Carers in managing their caring role whilst progressing towards their own goals. Promoting choice and person-centred approaches, PKAVS helps carers to be individuals first, and carers second. PKAVS Adult Carers Service offers information, advice and outcome-focussed support to adults. This approach includes the provision of direct support services, including benefits information, respite through PKAVS Day Club service, group and one-to-one support, advocacy, and access to respite opportunities through carers therapies and other person specific breaks. We work collaboratively with a range of partners to ensure signposting to statutory carer s support plans and to connect Carers to support in their community. PKAVS Carers Services works in partnership with Perth and Kinross Council and NHS Tayside to develop and provide services for carers throughout Perth & Kinross in response to identified needs. From April 2015, PKAVS transformed its service model to lead on a new multiagency Carers Hub model, offering coordinated and localised supports for Carers in their community and providing a centre of excellence for Carers support and engagement in Perth & Kinross. Provision through the Carers Hub will include: A single point of contact for Carers Locality-based carers support planning and assessments Access to high quality information & advice Flexible respite, including day services and short breaks Carers support groups Wellbeing support through Carers Therapies and self-management approaches Carers training and events Volunteer support Carer engagement and involvement in shaping services

2 JOB PURPOSE PKAVS has an exciting opportunity for an Adult Carers Development Worker to join its Adult Carers Services Team as part of a dynamic, multi-agency Carers Hub. The post holder will lead on Carers training, engagement and information events on behalf of the partnership, while supporting improved collaboration between agencies and driving forward best practice around the Carers agenda within our employee and volunteer workforces. The key objectives of the role are to: Ensure seamless support pathways for hidden and known carers thus increasing carer s access to support plans. This to be developed through collaborative working practices with all key statutory and voluntary sector carer organisations. Support Carers in managing their caring role and maintaining their own wellbeing through: the identification and coordination of training opportunities for Carers; identifying and filling any gaps for training opportunities for carers; planning and coordinating the 2 key events for carers each year which are the Carers Conference and Carers week events and providing high quality information to Carers via the Hub. Increase awareness of Carers as key partners in care across sectors, promoting the Carers Hub approach and facilitating training for staff and volunteers who work with Carers. Support Carers to have a voice and contribute to the shaping of future services. The Carers Development Worker will support Carers engagement with strategic planning particularly through the role played in developing and delivering the P&K Carers Voice (unpaid carers forum). Promote with employers across Perth & Kinross the Carers positive initiative. Ensure the support packages offered when adult carer support plans are completed are put in place. This will be determined through discussion with unpaid carers within 4 weeks of the support plan being completed. This post holder would then discuss any shortfall in services offered with the support worker for that carer and corrective action taken. The post holder will be required to provide effective outcome focused reports in line with our funders, the Carers Information Strategy, to demonstrate that identified objectives are sufficiently evidenced in a positive manner.

3 KEY TASKS & RESPONSIBILITIES To work collaboratively with colleagues within PKAVS, Perth & Kinross Council, NHS Tayside and the wider Third Sector to develop and embed person-centred support pathways for Carers. To identify carers training needs and gaps in training provision and coordinate an annual programme of training opportunities. To develop and promote a calendar of events for Carers and individuals working with Carers. This will include a focus on Carers Rights Day and Carers Week. Working with partners, the post-holder will support the delivery of the annual Carers Conference. To coordinate workforce Learning & Development around the Carers agenda, identifying learning opportunities for staff and volunteers across sectors. This will include the delivery and coordination of training and awareness-raising talks and presentations. To develop existing and new engagement mechanisms with Carers across Perth & Kinross, supporting links to health and social care strategic planning. To build capacity of the Perth and Kinross Carers Voice Forum, with a view to increasing its membership, supporting self-sustainability and developing a network of locality-based carerled forums. To establish and maintain working relationships with key Carers staff in the Local Authority, NHS Tayside and Third Sector agencies. To attend Carers partnership meetings as required on behalf of PKAVS. To deliver key tasks within an annual work plan that will be reviewed on a quarterly basis to ensure all progress is recorded and all tasks are being met. To ensure an effective monitoring and evaluation framework for all key tasks, with a focus on evaluating the outcomes of the work on Carers and professionals working with Carers. To provide written and verbal reports as required by the partnership, including statistical data and qualitative feedback to demonstrate that the aims and objectives of the post are being met. To maintain knowledge of local and national services in relation to Adult Carers, and policy and relevant legislative frameworks. To work as part of an integrated staff team within PKAVS Carers Hub in line with the organisation s policies and procedures. To positively promote the work of PKAVS Carers Hub. To contribute to PKAVS staff meetings, development days and working groups as required. To promote the Carer Positive initiative with employers throughout P&K. 4 PERSON SPECIFICATION A minimum of SVQ II Social Care An awareness of local and national services relevant to Adult Carers. An awareness of relevant policy and legislative frameworks relevant to Adult Carers and outcomes for Carers Previous experience of working with Adult Carers/Vulnerable Groups and supporting them to engage within their communities Previous experience of working effectively with a wide range of agencies and partners. Demonstrable experience of delivering person, centred outcome focused approaches Ability to maintain accurate records and in accordance with Data Protection requirements The ability to provide effective signposting to Adult Carers/Vulnerable Groups allowing them to access effective support. Strong communication skills both written and verbal Good report writing skills, including the presentation of statistical information and evaluation reports Excellent presentation skills Ability to work as part of a team Ability to use own initiative and prioritise workload effectively Ability for form effective professional relationships at all levels and with partner organisations Confident and assertive in difficult situations Full driving licence and use of own car Good IT Skills including word processing, , internet and PowerPoint Excellent organisational and project management skills An awareness of Adult Carers rights, needs and issues Previous experience working within the voluntary sector Experience of supporting engagement activity with Carer/ Service user groups Previous experience of organising and delivering events Ability to work empathically with others Essential Desirable

Carers Hub Manager This post carries a 6-month probationary period. A planned Induction Programme will be offered together with relevant job specific training. An offer of appointment will be made subject to two satisfactory references. The post holder will receive regular support and supervision carried out by the Adult Carers coordinator PKAVS determines annual leave entitlement in hours and not days to ensure equity for all employees by ensuring that staff who work variable hours/shifts do not receive either more or less leave than colleagues who work a standard shift pattern. On appointment 238 hours Equivalent to 34 days (28 days annual leave and 6 public holidays) After 2 years 252 hours Equivalent to 36 days (30 days annual leave and 6 public holidays) After 3 years 273 hours Equivalent to 39 days (33 days annual leave and 6 public holidays) Please note that annual leave hours stated above are based on you working full-time.
