2 The Stichting Tripod Foundation (STF), the quality controlling body for Tripod, defined an Accreditation system for Tripod Beta users (practitioners), trainers and assessors. All practitioner and trainer certification is done through the Energy Institute. All future practitioners and trainers to go through the certification process as established. No other organisations are acknowledged by the Stichting Tripod Foundation to certify practitioners or trainers. 3 Introduction TRIPOD Beta Practitioner accreditation scheme A Tripod Beta Practitioner investigates and analyses incidents using the Tripod theory of incident causation. Accordingly, this person must be competent in the understanding of the Tripod Beta methodology and also its application when investigating and analysing incidents. When these competencies have been satisfactorily assessed, the person is said to be an Accredited Tripod Beta Practitioner. Why become an Accredited Practitioner? Tripod Beta is a powerful tool for incident investigation; however it requires a good level of competence to ensure it is used correctly and effectively. Tripod Beta Practitioner Accreditation is the means to assess whether you have the necessary skills and knowledge to make use of Tripod effectively. Your level of knowledge, skills and expertise are verified by an external Tripod Assessor (who themselves are certified by the Stichting Tripod Foundation). Feedback is provided on your Tripod incident investigation reports, giving you the opportunity to hone your craft, and in turn providing you confidence in your ability to use Tripod successfully. You will be provided with a certificate proving you are accredited, and listed on the EI s database of Practitioners, which can help your employer recognise, and be confident in, your abilities. 3

4 The Accreditation process Prerequisites As a prerequisite to start the Tripod Beta practitioner process, the candidate must be an Associate Practitioner. To do this, candidates must pass the Tripod Beta exam. The exam is included at the end of Accredited Tripod Training courses (or can be taken independently). The exam will focus on the theoretical elements of the Tripod Beta theory and will cover the understanding of: 4 The Tripod Concept Tripod trio s Barriers Tripod causation paths Classification of underlying causes Upon completing the exam the candidate will be classified as an Associate Practitioner. Accreditation process The candidate must submit two Tripod Beta analysis reports (Report A and Report B), both of which must be deemed competent by a Tripod Assessor. From these reports, a Tripod Assessor will evaluate the candidate practitioner s understanding and ability to perform a Tripod Beta incident investigation and analysis. This will be achieved by demonstrating: Ability to define Tripod trio s Ability to define barriers, in a consistent manner and placing them in the correct position on the Tripod Beta tree

5 Understanding of the process of incident investigation Ability to create Tripod causation paths leading to the identification and classification of Underlying Causes Understanding of the logical rules when creating a Tripod Beta Tree Ability to define appropriate remedial actions The Tripod Beta Reports to be handed in: Must be submitted along with a completed submittal form (Annex C). Must be in an editable format and include a Tripod Beta tree; e.g. as a PowerPoint presentation and Word document. Must be written in English, German, or Dutch (other languages only after approval). Should be of a medium sized incident e.g. a personal injury or minor loss of containment don t be too ambitious! May contain at most 30 pages. If completing Accreditation as part of a group of people, should be of a different incident to that of colleagues. This is to ensure reports demonstrate individual competencies. Report A and Report B must be of two different incidents. Report A can be resubmitted if it fails the first time, however Report B can only be submitted once. The timeline of submittal should therefore be as follows: Report Report A Report A re submittal Report B Comments Will be awarded a competent or not yet competent mark, and comments fed back to candidate. These comments will help the candidate in producing Report B and in re submitting Report A if necessary. If Report A did not pass first time, it should be revised and re submitted. Report A must now be deemed competent if accreditation is to be awarded. Will be awarded a competent or not yet competent mark, and comments fed back to candidate. Report B must be deemed competent first time if accreditation is to be awarded. Report A should be submitted first, however Report A re submittal and Report B can be submitted together. Duration of Accreditation process Upon becoming an Associate Practitioner, candidates have 2 years to begin the full accreditation process. After this time they will need to re sit the Tripod exam. Once the accreditation process has begun, the maximum time limit to complete accreditation is 6 months from subscription. Subscription is classed as the date the candidate submits a completed accreditation form (available at the end of this document). 5

6 Assessors reviewing term The maximum Assessor term for reviewing each incident analysis report is 4 weeks. As soon as the Energy Institute has received the report from the candidate, it will inform the Assessor and notify him or her that within 4 weeks, the report must be reviewed and returned. Taking into account mailing time, the candidate practitioner will receive the results of the reviewed reports in approximately 5 weeks. 6

7 Validity Tripod Beta Practitioner Certificate Accredited Practitioners will be provided with a certificate from the Stichting Tripod Foundation and Energy Institute, and listed on the Energy Institute Tripod Practitioner database. Validity Period of certificate A Tripod Practitioner certificate is valid for a period of 5 years. However, during that time the Practitioner should ensure they practice their skills in conducting incident investigations and analyses in order to remain competent. By maintaining accreditation, people show they also maintain their skills, knowledge and incident analysis capabilities. 7

8 Annex A: Practitioner Assessment criteria These are the assessment criteria used by the assessors for the Tripod Beta Practitioners accreditation: 1. Do the Events describe a happening to the Object? 2. Do the Objects describe items before their condition was changed to that described in the Events? 3. Do the Agents describe phenomena that had the potential /ability to change the condition of the Objects to that described in the Events? 4. Does the final core diagram contain 2 to 5 trios? 5. Does Time move from left to right? 6. Do the words in the Agent & Object appear in the Event either implicitly or explicitly e.g. ignition source {A} acting on flammable gas cloud {O} = Explosion {E} which is implied use of language test or Car Rollover {A} acting on driver {O} = driver injured in car rollover {E} which is explicit use of language test. 7. Are Failed Barriers described such that, had they been effective, they would have prevented the next Event from occurring? 8. Are the Barriers described in specific, and not general, terms, (e.g. the relevant part of a procedure rather than the title of procedure or type of procedure)? 9. Have the Immediate Causes led to the failure of their respective Barriers? 10. Has the person or persons that caused failure of the barrier been identified? 11. Do the Immediate Causes describe what was done or not done to defeat their respective barriers? 12. Do the Immediate Causes describe an act of doing, or not doing, something? 13. Do the Preconditions explain why the individual thought that their act was normal, acceptable or even commendable? OR Did the proposed preconditions have an influence on the behaviour of the person who made the error leading to the Immediate Cause that in turn led to the Barrier failing? 14. Are the Underlying Causes valid reasons for the perceptions and beliefs, (Preconditions)? 15. Are the Underlying Causes specifically described and do they have CONTENT? 8

9 16. Does the underlying cause represent a failure on a system level, i.e., its relation to the actual event is remote in time and! or location? 17. Are the remedial actions SMART? Specific Do they relate to a clearly identified action to be taken which is understood and agreed by the action taker? Measurable Can the results of taking action be measured in some way and closeout verified? Appropriate Do the actions specifically address a failed barrier or underlying cause identified in the report? Realistic Will the actions be able to obtain the level of change required given the resources and capacities at the disposal of the organisation? Time Based Is the time period in which the action must be completed stated? 9
