1 Rewards Program The new LiveWell Rewards program is designed to help you improve and maintain your own personal good health. All benefit eligible employees are automatically enrolled in the program. You will earn points for completing your Total Health Screening and meeting certain measurement and lab value goals. Additional points may also be earned see the list below for other ways to add to your total. Each day is another opportunity to earn points for each of your healthy lifestyle behaviors. Choose just a few activities or choose them all. It s up to you! 2 Redeem Points for Prizes As you meet targeted health goals and accumulate points, redeem them for the prizes listed below. Points do not expire and will roll over year to year. Access the LiveWell application in the Telephone Directory in WebHIS. Open up your account and click redeem. Some sample rewards are listed below but are subject to change periodically. Once you select your prize, you will be contacted by Human Resources about retrieving your reward. It s that easy! 4 As you are aware, Holy Name Medical Center is invested in your health and well-being. To demonstrate this commitment, we ve implemented a LiveWell Rewards program that enables employees to earn points for healthy lifestyle activities. Those points can then be redeemed for prizes. Points do not expire and will roll over year to year. All benefit eligible employees will automatically qualify to participate and are auto-enrolled in the LiveWell Rewards program. We hope the LiveWell Rewards program will serve as another way we can help you improve and maintain your own personal good health. Use your points to redeem prizes such as Employee Fitness Center dollars to pay your monthly membership, personal training sessions, or even exercise equipment. Where do I find the new LiveWell application? Open the Telephone Directory in WebHIS and insert your name (1). Look to the right of your name and you will see the LiveWell icon. If you simply hover over the icon, you will see your total point balance. Click on the icon (2) to open the LiveWell application and view/ redeem your LiveWell points. 1 Once you ve opened the LiveWell application, look at the top where you will see your LiveWell point balance (3). Each activity/ event that accrues points is listed below that total (4). This is your current 3 point balance 2 LiveWell Points Points are earned when you participate in various activities. Here are a few examples on how you can earn points: Complete your Total Health Screening Swipe in each time you are at the Employee Fitness Center Join the Dr. K. Weight Loss program and attend sessions Attend a Lunch and Learn Attend a special educational session Get your annual Flu Vaccine Participate in a LiveWell designated event Attend the Employee Health Fair Achieve normal values for key measurements blood pressure, waist circumference, and BMI Achieve normal values for key lab work HDL, LDL, Triglycerides, HemoglobinA1C, and Vitamin D The new LiveWell application is available on WebHIS and ready for your use. Sign in to see your accumulation of points and look at the different types of rewards. Individual points earned and redeemed 2. How do I know if I am enrolled? If you are a benefit eligible employee, you will automatically be enrolled and will see the LiveWell icon. If you do not see the icon and believe that you are eligible to participate please call Human Resources or Employee Health Services. 3. What does benefit eligible mean? Benefit eligible means that the employee status is either full time or part time. Per Diem staff is not eligible to participate in this program. 4. Why don t I see points for completing the measurements for the Total Health Screening (THS) when I know I completed it? Once you complete both the measurements and the lab work, points will be earned. 5. When can I redeem points for rewards? Once you have enough points you can redeem them immediately or continue to add to the total in order to earn a more substantial prize. Continued on back 4

5 6. How do I redeem points? When you are ready to redeem your points, open the app and hit the redeem button (5). Select the reward you would like and click Enter!
