1 The Black Country and West Birmingham STP GP Career Support Pack A General Practitioner Intensive Support site (GPRISS) The Black Country and West Birmingham STP was announced as one of two GPRISS for the Midlands and East Region in June There are seven intensive support sites in England. The STP has particular issues with retention and critically has a strong level of local leadership in place. The aim for the intensive support site is to bring about real improvement to GP retention by bringing interventions together at different levels Personal Support for GPs, Practice Support and System Support / Improvements. The STP has been allocated funding to support the project and the various initiatives emanating as a consequence of this investment. In addition to local leadership, specific project management support has been assigned to the STP, and a change facilitator has been allocated to provide change management leadership and expertise to create and agree clear, measurable and practical plans for GP retention. They will also ensure that key stakeholders are enthused and motivated to make the changes required over this intense six month period. There are four distinct phases to this project: Diagnostics (July August 2018) Plan (August September 2018) Implementation (September March 2019) Evaluation / outcomes review (March 2019) NHSE Midlands and East Region Black Country and West Birmingham STP GP Recruitment Intensive Support Site Initiatives (Implementation Oct 2018 to Jan 2019) Support for Individual GPs: Scheme 1: Incentivising Portfolio Careers The scheme supports the development of flexible career opportunities and helps GPs enhance their skills and knowledge in area(s) of interest. Up to 10,000 has been made available for training needs and access to secondary care support. This scheme encourages a different approach to tackling recruitment difficulties by encouraging GPs to adopt a portfolio approach to their employment options. 2 To enhance skills and knowledge in area(s) of interest The scheme encourages flexibility whilst retaining GPs The scheme strengthens speciality level exposure, experience and flexibility in employment So what s included? The opportunity to work alongside colleagues in a different setting/ area of interest An expression of interest form that confirms the broad area of interest, skills and knowledge development sought Financial support up to 10k per portfolio career/person available until March 2019 A mutually agreed period of time for development into an additional role It s available to all GPs on the performers list employed in the Black Country. GPs willing to remain in general practice & keen to explore a blended career path. The scheme is tailored to individual preference. Reduction in GP clinical sessions / additional sessions in an alternative setting i.e. education, leadership, hospital, community etc. Preparation of joint development plan (portfolio match). Training, education needs may be identified. How long is the offer available? Offer is open to application till 1 March 2019 Support will run to March 31 st 2019 If successful the project is likely to continue as part of the Black Country sustainability plan. Complete expression of interest form & hand to a facilitator or Sarah Southall sarah.southall@nhs.net for further information Scheme 2: Support for Newly Qualified GPs (First 5) and GP Trainees The scheme has been developed in collaboration with the RCGP Midland Faculty. It encourages GP trainees and newly qualified GPs (First 5) to stay in the area after qualification by actively recruiting GP trainees and First 5s to a supportive and vibrant peer network. The network seeks to secure continued employment in the area and make the Black Country an exciting and career development home for the future. The scheme will commence with a 'Welcome to the Black Country in the Midlands, and the best job in the world!' event. Followed by series of events, that include:

3 In the Black Country, An event open to all recently qualified GPs in the Midlands, as well as GPs from other areas. The event will showcase the area, its attractions, offers and amenities. A full day Saturday with family/social aspect, to improve integration into the Black Country; an opportunity to meet peers; seek a learning mentor; find out about vacancies in the area; business/pension/accountancy support. A small group CPD sessions based on cohort need. The aim is to support newly qualified GPs and GP trainee s transition into General Practice as a GP in the Black Country. Encourage networking, peer-to-peer support, explore the range of possible career opportunities in the area, and offer a warm and supportive welcome to the Black Country. What will it include? Four networking and development events to provide opportunities to meet peers; an induction to the area; an opportunity to meet recruiting practices; an opportunity to link with senior GPs from the area; Practical advice on pension/tax (money matters); Planning and career development opportunities within the area; CPD support through small group sessions. Newly qualified GPs, First5 and GP Trainees within the Black Country and West Birmingham STP. Series of four events to run across the Black Country and West Birmingham STP. This scheme will run from November 2018 to 31 st March For further information and/or complete an expression of interest form please contact Jazz Singh on jazz.singh@nhs.net, call Scheme 3: GP Peer Support and Mentorship The initiative will be delivered through a Peer Mentoring Network. The option offers a dedicated peer support service for GPs in their mid or late (Wise 5) career. The PMN will be available to GPs who would specifically like to explore ways to develop themselves and their career within General Practice in ways that may not appear available to them now. This could be personally and / or professionally.

4 Creation of a peer career planning service for GPs in their early, mid or late career. Peer mentoring options that enable GPs to explore ways to develop themselves and their career within General Practice. This could be personally and / or professionally, and will be delivered by GPs. What will it include? One to one peer mentoring support, initially for 3 sessions, fully funded. These sessions will be 2 hours, face to face, and provided by a GP. GPs in mid and late careers. GPs need to practice in the Black Country and West Birmingham STP to qualify. Up to 3 sessions of 2 hours, face to face mentoring. GPs are required to complete a questionnaire pre and post the mentoring session(s) with the information used to inform and improve the service offered. How long is the offer available? This offer is available between 1st October 2018 and 31st March For further information and/or complete an expression of interest form contact Daniel King daniel.king@nhs.net Scheme 4: Pre-retirement Group Coaching Forum The scheme utilises the information gained from scheme 3 and local intelligence to operate a forum that targets support to those GPs approach retirement age/considering retirement. This forum seeks to ensure Wise 5 GPs (late career) are well informed about the options available to them when planning their future within practice, with the aim of retaining their skill and knowledge within the profession. Options may include salaried roles or the opportunity to and broker discussions with practices that have vacancies / opportunities available. The aim is to have 'honest conversations that offer workable and feasible options, helping sort issues, concerns and worries, and provide concerted support. To retain GPs considering retirement and return recently retired GPs to clinical practice. To offer support through pensions and estates advice, as well as indemnity costs to affected GPs.

5 What will it include? A professionally assisted forum to explore options and provide advice on pension, estates and finance. Support with indemnity costs of up to GPs working in the Black Country and West Birmingham STP who are contemplating retirement in the near-future or who have recently retired and are interested in returning to practice for a few sessions per week. 1 evening, for 2-3 hours. How long is the offer available for? 2 sessions, provisionally November and January (participants would only attend a single session) For further information and/or complete an expression of interest form contact Leon Mallett, leonmallett@nhs.net Supplementary Schemes available at a Personal, Practice and Systems wide level as a consequence of being an intensive support site: Personal and Practice based: To open up national offers of support which it is anticipated will: Maximise 1:1 specialist coaching available through the scheme to individual GPs Support the redesign general practice releasing time to care. Making best use of quality improvement tools and professional input to develop and nurture effective and efficient collaboration and partnership across the system. Making best use of the Primary-Secondary Care Interface Toolkit to improve the interface between primary and secondary care. Making Primary Care a healthier and happier place in which to work. What will the offer include? Access to the Learning in Action Programme (LIA) Groups of 10 practices working through a series of facilitated 3.5 hour workshops. Productive General Practice Quick Start (PGP) Groups of 8-12 practices working with lean tools and external specialist support, receiving intensive hands on input over 8 weeks. The PGP programme will run December 2018 to March National Coaching for GPs 3 x 1.5 hour sessions for up to 107 GPs across 3-4 months

6 Access to the Interface Toolkit Offering Practical ways in which practices / Networks can collaborate locally to improve the delivery of care across the primary and secondary care interface. LIA 3.5 hour workshops (maximum of 6) across 5-6 months PGP 8 weeks (majority of support session in practice) Coaching 1.5 hours x 3 Primary Care and Secondary Care Interface Toolkit an STP wide workshop Learning in Action / PGP / Toolkit practice teams National coaching offer General Practitioners in the Black Country and West Birmingham STP. Support available across the lifespan of the programme Oct st March To discuss these options and how they may support you and your teams please contact Alice O Neill at - alice.o neill@nhs.net Supplementary Information on 1:1 GP Coaching: 107 one to one coaching places have been secured through the General Practice Development Team NHS England for GPs within the STP. GP participation is voluntary and is not performance/appraisal related The Black Country and West Birmingham STP will work with GPs to identify those that would like to be supported through the scheme. A link will be provided from November 2018 to the Black Country and West Birmingham STP for participating GP s to complete online to gauge need. The simple format will take no longer than 15 to 20 mins to complete and will ensure GPs are mapped to a coach with the skill set best placed to support their identified needs. GP s to be contacted by allocated coach to set-up 1:1 coaching sessions early December All coaching sessions to have been delivered by 31/03/2019 Systems Level Support: We will commission new models of care throughout the STP focussing on primary care at scale and adopting service delivery to meet the demands of the system. We will regularly promote the Black Country STP as a great place to work making best use of marketing material such as the promotion of portfolio careers across the STP.

7 We will be actively supporting practices to participate in a range of other projects associated with GPFV recruitment and the introduction of new roles, in addition to flexible career options for early, mid to late career GPs. We will undertake a recruitment programme (including advertising practice vacancies across the STP, working with universities, staging recruitment events, and providing relocation support, etc.). We will continue to undertake and expand engagement activities across the system within primary and secondary care e.g. learning events, progress updates, sharing learning and best practice through effective use of the primary care-secondary care interface toolkit. We will commence work to further develop Primary Care Networks and continue the STP's transition to ICS.
