6 Risk Assessment Risk Assessment is five parts Hazard Identification Control (Mitigation) Selection Risk Assessment (Severity and Likelihood) Risk Mitigation Risk Monitoring Risk Management is ongoing, using Risk Assessments and its associated data to 1) Continually reduce risk and 2) Continually monitor and track 6

7 Safety Risk Management Problem: How do we verify risk reductions? Don t the regulations already tell us what to do and what controls to use? 7

8 Develop & Manage SMS Metrics > Leading and Lagging Metrics For example: from the Risk Assessment Process: Frequency When do we initiate a risk assessment? Failure Does Management Know the Top 3 Risks? Do we inspect controls? Any failures or non-conformance? In particular, High risk controls? Trends Any recurring trends? Closure Are corrections tracked fully? Each of these metrics are: Measured Trended, and Compared to a target or expectation Is risk being reduced? 8
