1 Sample Performance Review Health Fitness Professional, 4 mon. in position Developing 1. For last quarter, describe your performance both in terms of accomplishments and behaviors. In your description, please be clear and specific. For being four months on the job, I feel like I m doing well. I have made progress with the HEART tool and have communicated HealthForce 2020 goals to members. I have focused my efforts with the HEART tool on healthy eating and healthy movement. I m trying to build rapport and get to know members, along with meeting the HEART tool goal. This past month has been especially busy with member questions. I didn t realize how much time member questions would take! For healthy eating, I ve scheduled a meeting next month with the cafeteria vendor to brainstorm healthy items we could add to the lunch menus and put in the vending machines. I still need to price options for providing free fruit to members. For healthy movement, I met with facilities to discuss ideas. We have had discussions for ways to re-invigorate the outdoor walking path, and how to incorporate an indoor walking path within the building for use in bad weather. There appears to be a lot of barriers to making these changes. Unfortunately, being new to my role and not knowing how much time member questions would take, I haven t scheduled a follow up meeting with facilities yet. I hope to do so within the next two weeks. As a new associate, it can sometimes feel a little overwhelming to keep up with the variety of tasks you re asked to complete and provide great service to our customers. There is a lot to learn to do your best work. You re 4 months into the job and you are experiencing some of the typical challenges, such as meeting all of the deadlines for implementing new tools and building relationships so you can have more influence to make changes. By next quarter you should be back on track with rolling out the HEART tool and have made progress on your conversations with facilities. If you have concerns with that timeline then let s discuss at the performance review to create a plan that you re comfortable with and ensures you make progress in these areas. In the meantime, let s plan to have more frequent 1-on-1s. These conversations will be a chance to provide more coaching and feedback about how to collaborate more effectively with the facilities lead. Specifically, we will talk about how you can listen to his concerns and understand his point of view regarding the changes you want to implement and you can adapt your approach to proactively address his concerns head on, so they don t become barriers to progress. 2. For next quarter, my top priority is to get back on track with the timeline for implementing the HEART tool, and develop a plan to move forward with becoming compliant on HealthForce 2020 goals. 2 the most impact in supporting a healthy lifestyle for members. Another goal is for 200 members to have completed their health assessment by the end of next quarter. I kept all of our members happy, but didn t make as much progress as I would have liked with the HEART tool.
