This Addendum provides the following: 1. Written answers to Vendors written questions; 2. Attachment I, Scope of Services, replaces Page 5 of 77 with Revised Page 5 of 77; 3. Attachment I, Scope of Services, Adds Page 5a; 4. Adds Exhibit 3 Site Visit Sign-In Sheet. All amendment(s) to the ITB are highlighted in yellow. Deletions are stricken and additions are underlined. All other terms and conditions of the ITB remain in effect. To the extent this Addendum gives rise to a protest, failure to file a protest within the time prescribed in subsection (3), Florida Statutes, shall constitute a waiver of proceedings under Chapter 120, Florida Statutes. 2 Question(s) from Francis Uriel Electric, Inc. WRITTEN ANSWERS TO QUESTIONS RECEIVED 1. Question: Is there a budget for this project? If yes, can you tell me the amount? Answer: Approximately $40,000 annually. Question(s) from T-Solutions Corp. 2. Question: In Section 3.06, CONTRACTOR S SERVICE FACILITY, of Attachment I, Scope of Services: Does the Bidder certify that its service facility is located in Leon County, Florida? If it does not have a service facility in Leon County, Florida, does the Bidder certify that it will establish a location within sixty (60) days of contract execution? We are located in Tampa FL, but do Telecommunications cabling work, all over the State of Florida Can we still Bid on this?? Answer: Yes, pursuant to the requirements and qualifications identified in the solicitation. There are no geographical requirements/limitations regarding acceptance of bid submittals. Question(s) from technologease, Inc. 3. Question: In reviewing the ITB, I noted the requirement in FORM 2 - BIDDER QUALIFICATION QUESTIONS (Page 1 of 2) Question #5, Does Bidder certify that it has at least five (5) years of experience providing services significantly similar to the services described in Attachment I, Scope of Services? My question is whether this is referring to the company experience or the individual experience of the team? Our company was founded on August 24, 2014, therefore the company itself has two years and seven months of experience. However, our ownership has over 65 years of collective telecommunication experience and our technicians met the meet the all of the requirements as well. Aside from the length of time in business, we meet all of the other requirements. In the event that the company must be in business for 5 years, I would like to request an exemption to that qualifier. I am including links to the following documents that will help you in evaluating our capabilities to perform on your projects. It includes the resumes of the key associates that will be involved in working on your projects, an overview of the company, equipment that is used in the field, our certifications, customers comments, past performance, credit/distributor references, and sample shop drawings. In addition, please note that we have 2 full-time RCDDs on staff and our entire technical staff hold current Jessica Lundsford badges. We meet or exceed all of the requirements in the ITB except Form 2 Question #5. Please let me know your interpretation of From 2 Question #5. Answer: FORM 2 BIDDER QUALIFICATION QUESTIONS (Page 1 of 2), Question #5, requires the Bidder s company, not its employees cumulative experience, to certify that it has at least five (5) years of experience providing services significantly similar to the services described in Attachment I, Scope of Services. Question(s) from Premise Network Solutions 4. Question: Can you send a copy of the sign in sheet for the ITB ? I appreciate it. Answer: See Exhibit 3 Site Visit Sign-In Sheet (attached to this Addendum).

4 3.04 SERVICE AREA A. Services shall be provided at the following location: 1. Department of Highway Safety and Motor Vehicles Neil Kirkman Building (and surround buildings on DHSMV campus) 2900 Apalachee Parkway Tallahassee, Florida Florida Highway Patrol Troop H Headquarters 2100 Mahan Dr. Tallahassee, FL Florida Highway Patrol Training Academy 75 College Dr. Suite 221-FHP Havana, FL The Department reserves the right to add additional geographical locations to the resulting contract pursuant to the provision in Section 3.10 ADDITIONS/DELETIONS. B. Service Hours The Contractor shall provide services Monday through Friday between the hours of 8:00 a.m. and 5:00 p.m. However, there may be times when the Contractor may be required to provide services after 5:00 p.m., Monday through Friday, on weekends and/or holidays. Depending on the work to be performed, the Contractor shall charge the Department the applicable hourly rate, if not already included in the unit cost for the work to be performed, per FORM 3 PRICE SHEET WORK ORDER REQUESTS All work to be performed by the Contractor shall be pursuant to an approved work order only. A. The Department will or call-in a work order request (Project or MAC) to the Contractor. The Contractor shall a work order to the Department s Contract Manager within twentyfour (24) hours of receipt, for review and approval. The work order shall reflect the following information: 1. Date/Time call received; 2. Problem description; 3. Location, Building, Room number; 4. Project or MAC Number; 5. Name and telephone number of the Contract Manager; 6. Cost Estimate in accordance with pricing provided in FORM 3 PRICE SHEET; 7. Date/Time of anticipated Dispatch; and 8. Date/Time work order is anticipated to be completed. Revised Page 5 of 77

Work Order Approval The work order and related documentation, where applicable, shall be ed to the Contract Manager for review and written approval within twenty-four (24) hours of receipt of the work order request. The Department will provide written notice within five (5) business days of receipt of the work order indicating whether the work order is approved or identifying items that require revision.
