1 CITY AND COUNTY OF DENVER DEPARTMENT OF PUBLIC WORKS Professional Engineering Services for 2016 Public Works Transportation and Mobility On-Call Project Control No. PWTES EBid Document No Q&A Addendum #2 May 11, Section I, B of the RFQ has been revised to clarify the previous discrepancy as follows: It is recommended that a team possess a minimum of 60% of the list of anticipated capabilities and/ or expertise (on the matrix) and will receive 5 points if this is demonstrated. Please see attached pages 7 and 8 of the RFQ for an updated version of the Selection Criteria and Scoring Ranges to reflect the 5 point change. 2. Question: Can the City provide Attachment 3 Commitment to DBE Participation in a format that can be modified? City Response: Please see attached. *** Also please note that the Attachment 1 Firm Responsibilities form has been updated. Please see attached and use this updated form in your submittal. 2 Proposers shall be required to submit in their transmittal letter a statement acknowledging their willingness to comply with the insurance requirements, as stated in the Sample Agreement and Sample ACORD. 2. RFQ Evaluations: The Selection Committee is charged with the responsibility of reviewing and evaluating all responsive submittals and other information received in accordance with the requirements of this RFQ, later directives from the City and the official advertisement. In assessing the strengths and weaknesses of each respondent, the Selection Committee may, at its discretion, request clarification of any response to this RFQ or other issue that may arise during the selection process. The City reserves the right to include or allow the Selection Committee to consult with any additional, non-voting commentators during the selection process. As provided for in the RFQ documents, proposals shall be screened by a City Selection Committee and Review Team. 2. The Selection Committee will make the final selection and recommendation following a comprehensive evaluation process. 3. Selection of firm or team is anticipated to be by Direct Selection based on the scoring that follows. However, pending the number of proposals received and the quality of the proposals, an interview with short listed firms or teams may be required. Proposals will be rated by the committee according to the following criteria. 1. Qualifications and experience of the team in the professional areas listed in this RFQ, as indicated by prior successful completion of similar projects. (15 points) 2. Qualifications and experience of the Project Manager, Project Engineer, and other prime key personnel (including sub consultants) in the professional areas listed in this RFQ and in similar projects. (15 points) 3. Ability of the project manager and other prime key personnel to effectively communicate and coordinate as demonstrated by proposal technical completeness and prior experience with public agencies. (10 points) 4. Proposed approach to providing professional area expertise to complete the possible scopes of work and displaying successful completion of similar projects. (10 points) 5. Ability of the team to meet the minimum reccomeded 60% of the list of anticipated capabilities and/or expertise. (5 points) 6. Diversity and Inclusiveness: Contractor demonstrates commitment to Diversity and Inclusiveness. (2 Points) 7. Diversity and Inclusiveness: Submit Receipt from Diversity and Inclusiveness Information Request form indicating successful completion and submittal of this required form. Licensed in Colorado with any required professional license and registered with the Secretary of State of Colorado (if a corporation, limited liability company, or limited partnership) with a local office within the Denver Metropolitan area, or a commitment to establish such an office within 10 days of being awarded this contract, or partnering with a firm that has such a local office. (1 point) 9. Within the proposal, a completed Qualification Matrix of services provided by the firm or team in the professional areas listed in the RFQ, see Attachment 8. (1 point) SECTION IV - SUBMITTAL REQUIREMENTS Proposers shall submit nine (9) paper copies of their completed RFQ response and one (1) labeled Compact Disk including an electronic PDF of the entire proposal. In the event of a discrepancy between the hard copy of the RFQ response and the RFQ response contained on disk, the hard copy shall control. All Proposals must be limited to the following prescribed information and be submitted in an 8 ½ x 11 size format and be in a comb or spiral binder, not submitted in a loose-leaf, three-ring, hard cover binder. Pages are to be numbered in sequence and font size will be at least 11pt. font. Tabbed divider sheets shall have no information, written or graphic, other than the section number corresponding to the following listing is to be included on the divider sheet. Proposals shall not exceed 25 single-sided pages in length (double-sided pages count as two pages). 11 x 17 pages may be used for charts or graphs and count towards page limit, however, this page limit is not affected by: cover pages, divider sheets, pages related to DBE Requirements, team member resumes, comments relating to the Sample Agreement, or required forms. Part 1 - Mandatory Requirements Letter of transmittal - Clearly indicate the single contact (principal-in-charge), mailing address, telephone, facsimile numbers, and address that will be used as a single point of contact should a contract be awarded. Indicate unique features of the organization and the project team that makes the team uniquely suited to undertake this specific type of on-call. Affirmative statement regarding willingness to comply with Insurance and Indemnity requirements listed within the Sample Agreement. Certificate of Good Standing from Colorado Secretary of State Website. Receipt of completed Diversity and Inclusiveness Information Request Form. Part 2 - Key Personnel Resumes / Biographical Data Provide resumes and biographical data for the key personnel including major sub-consultants, and identify who will be the City s consistent point of contact on the consultant team if awarded a contract. The data should focus on education, certifications, experience and successful completion of similar projects. Part 3 - Team Qualifications Provide Qualifications and Experience of the firms and companies who make up the consultant team by indicating prior successes of similar projects as the examples provided in this RFQ. State whether the firms have previously worked together, and if so, in what capacity. Complete and include Attachment 1 Firm Responsibilities Complete and include Attachment 2 Consultant/sub-consultant team members Indicate the team members successful administration of past/present on-call municipal and/or institutional contracts. Indicate what work the prime consultant will self-perform and what work will be performed by subconsultants. Indicate the ability to assign sufficient experienced personnel at all levels as needed to ensure manpower availability on an as needed basis.
