Position purpose N/A Education and Support Services Health literacy is integral to effective diabetes self-management. At an individual level, health literacy is one s ability to obtain, process and understand basic health information and services needed to make appropriate health decisions. At a system level, health literacy includes a service s ability to effectively engage and work with current and potential service users. These are essential components when working with harder to reach communities, particularly people from Aboriginal and/or Torres Strait Islander and other culturally and linguistically diverse (CALD) backgrounds. The Health Promotion Officer will support the implementation of Diabetes Victoria s Aboriginal and Cultural Diversity Program. Activities aim to improve health outcomes and health literacy for people with diabetes from CALD and Aboriginal communities and those at risk. Responsibilities include increasing community awareness and understanding of diabetes in Aboriginal and CALD groups and capacity building activities such as resource development and dissemination, supporting the update and evaluation of the Feltman resource for Aboriginal communities and establishing a successful Diabetes Victoria wide Diversity working group. B. Business Unit / Departmental Purpose The purpose of Diabetes Victoria s Diabetes Education Department is to support and empower people in the selfmanagement of their diabetes and to ensure the community has access to informed and educated health professionals for their diabetes. Consumer education programs and telephone advice, a comprehensive health professional education program, individual advocacy, and programs for people from culturally and linguistically diverse (CALD) and Aboriginal and Torres Strait Islander (A&TSI) communities are key services. C. Key accountabilities KEY RESULT AREAS (KRAs) The key measurable outputs or deliverables required of the position. 1. Cultural awareness activities TASKS The main activities required to achieve a KRA. Provide support to the Diabetes Victoria Diversity working group Support the Aboriginal cultural audit process, ensuring cultural competency principles are applied Contribute to increased community awareness about diabetes prevention, early detection and management KEY PERFORMANCE INDICATORS (KPIs) The quantitative/qualitative evidence used to measure how well the KRAs have been achieved. Health promotion programs and projects Facilitate the update, evaluation and distribution of the Feltman suite of resources for Aboriginal communities Identify and support training needs for Feltman resource package Support the development of programs for Aboriginal and CALD people with diabetes and those at risk Present information about diabetes to relevant Aboriginal and CALD community groups and health professionals. Review and develop culturally appropriate resources when required Readability assessment completed on all new or updated resources Evaluation of all relevant resources completed and documented Resources promoted through culturally relevant media All resources and community information meet health literacy guidelines 3. Administration Prepare correspondence, monitor mailouts and maintain databases. 4. Relationship management 5. Professional development Keep accurate records of program activities, budget expenditure and correspondence Positively represent Diabetes Victoria when meeting with community organisations and groups Seek opportunities to build partnerships with relevant organisations and health professionals to enhance program development Seek opportunities for the Cultural Diversity Program to work with collaboratively with other Diabetes Victoria departments Respond to questions and enquiries from health professionals and the general public Be an active contributor and enthusiastic participant in Aboriginal and Cultural Diversity team and Programs and Services activities Represent Diabetes Victoria on relevant committees and working parties Maintain a thorough knowledge of health promotion principles and practice, particularly in relation to health literacy, Aboriginal and CALD issues Keep abreast of current cultural diversity policies, practices, research, strategic plans and developments in health promotion and public health 6. Other duties Provide support to the Diabetes Victoria Health Literacy working group Program files are maintained appropriately within the Diabetes Education filing system Data entered on Salesforce as appropriate Relationships established with Diabetes Victoria staff to enhance Aboriginal and CALD strategies within other program areas Relationships established with health professionals to develop and promote program resources and information Attend appropriate professional development courses as agreed with Aboriginal and Cultural Diversity Program Team Leader Read current journals and relevant reports Submit grant applications as required Meet fortnightly with Aboriginal and Cultural support empower campaign Page 2 of 5

3 Contribute to grant applications for fundraising purpose. Assist in the organisational reporting processes by providing relevant information and reports to senior management Maintain relevant content on the CALD program pages of the Diabetes Victoria website Assist in arranging training for Diabetes Victoria staff on working with interpreters, translation of resources and cultural appropriateness Other duties as required Diversity Program Team Leader, or as agreed. 4 Experience 1. Knowledge and experience in applying health promotion principles and practice to the development and implementation of relevant programs 2. Experience working with CALD and/or Aboriginal communities 3. Experience of developing and facilitating training modules for staff and health workers 4. Knowledge and experience of evaluating health and training programs and resources 5. Experience in working with health professionals 6. Understanding of the needs of people from culturally and linguistically diverse backgrounds F.
