1 PSU Human Resource Services PSU Budget and Human Resource Services The 2018 PSU Service Award & Retirement Recognition Ceremony will be held Tuesday, April 24, 2018 at 2:00 pm in the Overman Student Center Crimson and Gold Ballroom. Employees with 10, 15, 20, 25, 30, 35 & 40 years of service in benefits-eligible position at PSU will be recognized along with individuals who have retired or will retire this fiscal year. The 2018 ceremony will recognize a larger group of employees because we are transitioning to a new time frame to determine award recipients. Previously, we looked at PSU service dates that fell between 7/1 and 6/30 of the following year. The 2018 ceremony will recognize employees who have attained one of the above noted milestones between 7/1/2017 and 8/30/2018. Future ceremonies will use the time period form 9/1 to 8/30 of the following year to determine award recipients. This means that some will receive their service award earlier than expected. 2 Each department on campus now has a designated HR Representative who is the first point of contact for issues and questions. Please find your department's HR Rep here: Flex Schedule Survey The Flex Schedule Task Force is asking for your feedback and input by completing a survey on telework and flexible schedules. Even if you believe these are not work options for you, they are very interested in your feedback. The survey should take about 6-8 minutes to complete. Deadline for responses is February 9 th The link for the survey can be found here

3 If you are a part of the HSA program through your health insurance plan you received an that a new tax form is available in your account. To access your tax forms you will need to visit and select Statements and Notifications *Contributions made through April 15 may qualify as prior-year contributions if specifically designated as such. *The IRS requires all HSA accountholders to submit IRS Form 8889 and, if you received one, Form 1099-SA with your tax return. Form 5498-SA should be kept with your tax records. Please consult with a qualified tax professional if you have specific questions about these forms. This Only Applies to Those Enrolled in an HSA account! The 1095-C Employer Provided Health Insurance forms are now available for the 2017 tax period. To print a copy of your 1095-C: 1. Login to the State of Kansas Health Insurance Portal 2. Click on the Forms Tab 3. Tax Forms
