All responses to the subject solicitation must be received no later than 2:00 P.M. (ET) on Monday, April 23, 2018 (see Item #1). A response received after the exact time specified will not be considered. Failure to file a protest within the time prescribed in Section (3), Florida Statutes, or failure to post the bond or other security required by law within the time allowed for filing a bond shall constitute a waiver of proceedings under Chapter 120, Florida Statutes (F.S.). Written notices, formal requests and proceedings must conform with the requirements set forth in Chapter , Florida Administrative Code (F.A.C.). 3 Page 3 Item #2: REVISED SAMPLE CONTRACT Attachment A, Sample Contract, is hereby deleted and replaced with Attachment A-1, Revised Sample Contract, which has been uploaded separately to the Vendor Bid System. Item #3: QUESTIONS AND ANSWERS Question #1: We do have a few requests that we need in order for us to prepare a complete proposal and a detailed cost analysis for PEO services: Most recent payroll register available Gross payroll by Workers Comp code (if included with above, please disregard) Current Workers Comp Declaration page (or whole policy) 4 years if Workers Comp loss runs + current year State Unemployment Rate (if applicable) Answer #1: Most recent payroll register attached. There is no workers comp shown. The State of Florida is self-insured on Workman s Comp. There have been no claims on Workman s Comp for the past 4 years. The February 2018 state unemployment rate is 3.9 Question #2: Answer #2: If you are interested in us quoting our benefits suite, we need: Copy of applicable benefits invoices (i.e. health, dental, vision, supplemental) Upcoming renewal rates or prior year rates if renewal not available Copy of plan designs Census of Employees including Part-Time and Full-time (template attached) Group Health Questionnaire (attached) Health Insurance loss Runs (only if over 100 ee s) Only plans that are similar to what the State of Florida offers will be considered. Please refer to answer for Question #3. Employee Census for each Range and Health Insurance Premium Rate Table provided as separate documents on the Vendor Bid System. Question #3: Lastly, we offer our clients the ability to create a custom-tailored services package. In other words, FWC would be allowed to carve-out certain services if they like the option from their current provider better (i.e. if you need the PEO for Workers Comp, but would like to keep your health insurance, that IS an option).

4 Page 4 Answer #3: If you could rank the following areas in order of importance and need to FWC (1 = most important, 3 = least important), that would be very helpful in understanding your needs: Health Insurance Human Resources Workers Compensation Prospective contractors must provide bid prices which include all of the services requested. Custom-tailored service packages are not an option under this Invitation to Bid. Question #4: Answer #4: The bid requirements require a health plan comparable to what is currently offered by the State of Florida. Could you please send me back a completed Census (or use the attached) and the plan design along with premium amounts the what is currently offered by the State of Florida? For the individuals with Coverage elsewhere - they also will need to be specified as: Coverage Elsewhere. Question #5: Answer #5: Please send the details of what portion (%/amount) is paid by the State of Florida/FWC and what is paid by the insured/employee? Question #6: Answer #6: Please provide a list of other benefits offered to the employees and if any portion of that is paid by the State of Florida/FWC Final Benefits Guide provided as a separate document on the Vendor Bid System. Question #7: Does the State of Florida/FWC contribute anything towards the 401K/retirement plan? Answer #7: Currently not for Other Personal Services (OPS) employees. All Range employees are classified as OPS. Question #8: Answer #8: In accordance with participation guidelines... the earlier census must contain all employees with anyone under 30 hours specifically stated that they are part-time/not eligible. Employee Census provided as a separate document on the Vendor Bid System.

5 Page 5 Question #9: Answer #9: Do you have the RT6 Unemployment Rate on each of the three facilities? RT6 rate is very small perhaps 0.5% based off of Tenoroc Shooting Range. Bay County and Triple N have not been in business long enough to generate a track record for this. Question #10: Answer #10: I found the DBA thru Sunbiz for: Bay County Shooting Range owned by Fish and Wildlife Foundation of Florida. I found NO DBA or active Corporation thru Sunbiz for: Tenoroc or Triple N Ranch. In order for this to be done properly (because an entity cannot lease half their employees through a co employer relationship of a PEO), a DBA should be set up for each of shoot ranges. All facilities are owned/operated by Florida Fish and Wildlife Conservation Commission. All payments for services will come from the Fish and Wildlife Foundation of Florida a 501(c) corporation established to support programs of the Florida Fish and Wildlife Conservation Commission. Question #11: Answer #11: Will you be requiring separate reports/invoicing for each shooting range or will you need one invoice with payroll reports departmentalized by each location? Reports for each Range will need to be discernable from each other. One report and invoice is sufficient per reporting period and payment. Format for this report TBD. Question #12: How many W2's were issued in 2017 for each location? They all have approximately 15 employee's each. We'll need to get an approximate # for the Staffing/Recruitment expectations on an annual basis. Answer #12: There were (45) W2 s issued in 2017 for the Ranges. We don t necessarily have this broken out by range. Question #13: Answer #13: Will you be requiring Pre-Employment Drug Tests on all new hires? No. Drug tests may be required at any time during employment, but not to determine eligibility for employment. Question #14: Answer #14: Any Background screenings on New Applicants? Yes.
