1 Criteria for applying for work placement If you are thinking of applying for a work placement at Dogs Trust Loughborough, please read the information below to ensure that you match our criteria before applying: You must be at least 16 years old and attending an animal related college course. You must be able to commit to a 2 week placement. You can request to do a longer placement if required. You must be able to work to the centres allocated hours which are 8.00am to 5pm. We are unable to accommodate any other working times and all placements must run Monday to Friday. The centre has no public transport links so you must give consideration to how you will get to and from the centre on a daily basis during the placement. We experience a high demand for work placement yet for health and safety reasons can only accommodate limited numbers of placements at any one time. We therefore may not be able to meet your proposed dates however where possible, we will try to offer alternative dates. A minimum of 4 weeks notice of your desired work placement dates must be provided. Offer of a placement Terms and conditions The offer of a placement is dependent on the following criteria: An applicant fulfils all of the criteria above All necessary paperwork is returned complete Receipt of a college reference Availability of the desired placement dates The withdrawal of offer and termination of a placement The centre reserves the right to withdraw the offer of a placement or terminate a placement at any time. A withdrawal or termination can happen for one or more of the following: Mistreatment of dogs Incorrect information on application forms Poor attendance or lateness Not adhering to Health and Safety policies and procedures Not adhering to Dogs Trust standard operating procedures, internal policies (so far as applicable) and protocols Poor conduct or application to duties Poor standard of appearance or personal hygiene Not able to fulfil duties due to undisclosed health issues Any other reason at the Manager s absolute discretion I have read and understood the terms and conditions and agree to comply by them: Name Signature. A parent/guardian signature is required for under 18 year olds. Complete the application form providing plenty of information about your suitability for the placement. Complete the health questionnaire and emergency contact information. Read and sign the volunteer agreement. Ask your college tutor to complete the reference request template. All references should be signed by the person completing the form. References will be verified. What happens next? The rehoming centre manager will review your forms and contact you via . 6 Health Questionnaire Prior to volunteering with Dogs Trust and in order to ensure the young person s personal safety and also meet our health and safety obligations, we need to ask you about your health. 7 Dogs Trust Volunteer Agreement and Understanding Dogs Trust recognises the important and valuable role that volunteers play in order to help us to achieve our aims and provide for the welfare of the dogs in our care. To ensure that our volunteers are fully utilised, safe and happy we ask that they agree to some simple arrangements and conditions before carrying out volunteer work. We ask for you to agree to the following arrangements & conditions: 1. Each time you attend the Centre please sign in on arrival and out before departure. 2. Volunteers will be required to undertake any training needed for the role and agree to abide by Dogs Trust policies and guidelines as directed by staff. 3. We expect that volunteers will meet agreed commitments, be reliable and let us know in good time if they are unable to attend. They are also asked to respect and abide by the decisions of the management team at all times. 4. We politely ask that if you intend to make a posting on a social networking site such as Facebook or Twitter, that you will consider our reputation with the general public and whether or not your posting is appropriate. In return Dogs Trust agrees to provide the following 1. A health and safety policy, accident procedure and insurance as appropriate. 2. Health and Safety induction training and use of personal protective equipment where appropriate. 3. Support and training to enable you to carry out the volunteer activity effectively. 4. Assurance that we will value your contribution and welcome any suggestions or comments. 5. Assurance that we will treat volunteers fairly without discrimination and provide feedback regarding their work and contribution. 6. Assurance that we will treat any information you provide in accordance with the principles set out in the Data Protection Act IMPORTANT Please note that by signing this agreement you are accepting that: No contractual employment relationship is intended to be created and that you do not have the status of an employee You are not permitted to derive any personal financial benefit from the volunteer activities Any training or support that you are provided with during your work placement is given by Dogs Trust on a voluntary basis only and shall not be treated as a payment or payment in kind under any circumstances You are not permitted to represent Dogs Trust or use any Dogs Trust logo in any way without the prior written agreement of Dogs Trust.
