5 CY2018 ecqm Reporting Methods 1. Direct submission though the Joint Commission Direct Data Submission Platform 2. Vendor submission through a Joint Commission-listed ORYX vendor All hospitals submitting ecqm data will be transitioned to the DDS Platform beginning with CY 2019 data and forward*. Both reporting methods use the QRDA-I file format Note: The Joint Commission will continue to use ORYX vendors for submission of hospitals chart-based data through 2019 and will be evaluated annually , The Joint Commission

18 DDS Selection 2018 ORYX ecqm Selection Form Question: How do I select Direct Data Submission for CY 2018 ecqm data submission? Answer: You may download a copy of the 2018 ORYX ecqm Measure Selection Form from The Joint Commission website > under the Measurement topic. This form is to be completed and submitted via to: HCOORYX@jointcommission.org

19 DDS Selection 2018 ORYX ecqm Selection Form 2018 ORYX ecqm Selection forms are due to The Joint Commission no later than October 31, Please include your HCO ID# and 2018 ecqm Selections in the subject line. If your organization believes it has an extenuating circumstance impacting their ability to submit CY2018 ecqms to The Joint Commission, the deadline to request an Extenuating Circumstance Request form is October 31, 2018.

20 DDS Selection 2018 ORYX ecqm Selection Form Question: Who should use this form? Answer: Individual hospitals selecting their ecqms should use this form. Instructions are included with the form.

21 DDS Selection 2018 ORYX ecqm Selection Form Question: What if we are part of a system of hospitals? Answer: Hospitals that are part of a healthcare system or have a corporate entity who specify which ecqms their hospital(s) submits, should verify that their system / corporate contact is submitting ecqm selections on their behalf via a System Spreadsheet. The System Spreadsheet may be requested by the system / corporate contact via an to HCOORYX@jointcommission.org

22 DDS Selection 2018 ORYX ecqm Selection Form Hospitals electing to use the DDS platform for the first time need to be on-boarded. Hospitals will be on-boarded in groups monthly between the months of August 2018 and February Hospitals new to the DDS platform should indicate the month their organization would like to be onboarded. Staff must be available to participate in the onboarding process during the month specified. Months are assigned in the order in which the forms are received. Hospitals will be on-boarded in a manageable quantity to provide the necessary support for all organizations. Note: Onboarding does not equate to submittal of ecqms. Onboarding provides HCOs access to the platform to upload, review, manage and submit their ecqm data.

23 DDS Selection 2018 ORYX ecqm Selection Form Include the name and address of the Designated DDS Contact. The designated DDS contact is specific to Direct Data Submission, and may or may not be the ORYX contact. The designated contact is the hospital representative who has authority on behalf of their institution to review and accept legal forms for using the DDSP. This individual is the first person to log onto the DDS Platform. The designated contact can invite additional users as needed to the Platform. The designated contact is not necessarily the same person who performs the data upload and submittal process.

24 DDS Selection 2018 ORYX ecqm Selection Form Hospitals can use an ecqm ORYX vendor to submit their CY2018 ecqm data to The Joint Commission. Hospitals electing to use an ecqm ORYX vendor must include their vendor when ing the ecqm selection form to The Joint Commission. NOTE: All hospitals submitting ecqm data will be transitioned to the DDS Platform beginning with CY 2019 data and forward. NOTE: The Joint Commission will continue to utilize ORYX vendors for submission of hospitals chart-based data through 2019 and the use of vendors will be evaluated annually thereafter.

25 DDS Selection 2018 ORYX ecqm Selection Form Minimum ecqm requirements: Hospitals with an ADC>10: Hospitals are required to submit a minimum of four (4) ecqms for a minimum of one calendar quarter. Any calendar quarter(s) of data may be submitted. Hospitals have the flexibility of choosing which calendar quarter(s) are submitted during the submission process and do not need to be specified at the time of ecqm selection. Hospitals with an ADC 10: Small Hospitals and Critical Access Hospitals (CAHs) are not required to submit ecqms, but may do so if they wish.

27 HCO On-Boarding Onboarding organizations between August 2018 and February 2019 Direct Data Submission Platform is open for submission of CY 2018 ecqm data until March 15, 2019, 8 pm CST. We encourage organizations to plan ahead. Adequate time is needed to ensure a successful upload and submission: To address any questions you may have with the upload and submit processes. Resolution of any issue(s) within your organization impacting your ability to upload and submit ecqm data requiring engagement with your IT staff. To address any technical issues you may experience using the Platform.

28 HCO On-Boarding Question: How will New HCO s access the DDS Platform? Answer: A communication is sent with the month your HCO is to be on-boarded with all the details Prior to onboarding, each HCO receives an invite to join the Direct Data Submission Platform Please note the link to access the platform expires after a set period of time Invite comes from <accounts@apervita.com> Keep in mind that onboarding does not equate to submittal of ecqms. Onboarding provides HCOs access to the platform to upload, review, manage and submit their ecqm data

34 Platform Education/Assistance EXAMPLE Q&A Topics for Office Hours Question: When uploading QRDA I.zip file, will I upload the exact file used when I uploaded to CMS? Is there scrubbing of PHI prior to upload? Answer: Yes, the same QRDA I.zip files submitted to CMS can be uploaded to the platform. The DDSP scrubs the PHI before the data is submitted to The Joint Commission.
