1 VACANCY Job Title: Hours: Ageing Better Middlesbrough - Outreach Project Team Lead 37 hours per week Duration: Temporary contract until 31 st March 2017 Salary: 24,472-27,123 (starting salary 24,472) Location: Middlesbrough Community Fire Station Cleveland Fire Brigade is looking for highly motivated individual to join its Commissioned Services Team. Funded by the National Lottery through the Big Lottery Fund, Ageing Better Middlesbrough aims to fundamentally change the way in which people over 50 have access to the rest of their community. We believe that together we can produce long-term, significant changes that will connect older people to their peers and community resources, preventing loneliness and isolation more effectively for more people. The Outreach Project will be the first point of contact for some of the loneliest and most isolated people in Middlesbrough. We are looking for an individual that can support and manage skilled and experienced outreach workers to effectively engage people, mostly in their own homes. They will help them to overcome the barriers that have prevented them from reconnecting with other people and their communities. If you have experience of engaging with some of the most hard to reach individuals in local communities and delivering one to one support that is person centred, strength based and makes a difference to people, then we would like to hear from you. 2 Personal Role Profile Role Title: Location: Purpose of Role: Key Responsibilities Ageing Better Middlesbrough Reporting To: Commissioned Services Manager Outreach Project Team Lead Middlesbrough Community Fire Role/Grade: Grade F Station Lead in the delivery of the Service s Outreach project for the Ageing Better Middlesbrough programme in partnership with Middlesbrough and Stockton Mind. Enabling the Fire Authority to meet its statutory duties and to improve community safety by management and development of a team of Outreach Project Workers supporting lonely and isolated older people in Middlesbrough. Ensure the Brigade complies with relevant legislation and guidance particularly in relation to vulnerable persons and data protection 1. Management and development of a team of specialist Outreach Project Workers 2. Responsibility for delivery of Ageing Better Middlesbrough Outreach Project targeted outcomes 3. Responsibility for processing and maintaining records relating to clients of the Ageing Better Middlesbrough Outreach Project 4. Development of partnerships with key agencies linked to delivery of Ageing Better Middlesbrough 5. Contribute to the quality assurance process and lead partner management relationship with Middlesbrough and Stockton Mind for the Ageing Better Middlesbrough programme 6. Liaise with station personnel and key agencies on issues related to lonely and isolated older people in the Middlesbrough area 7. Evaluation of work activities linked to the delivery of the Ageing Better Middlesbrough Outreach Project and production of reports as and when required 8. To wear the workwear uniform in accordance with the Dress & Appearance Policy and Procedure 9. Take part in Personal Development Reviews and complete Personal Development Records in accordance with Brigade procedure 10. Maintain relevant skills and knowledge aligned to key responsibilities and National Occupational Standards to determine continued maintenance of competence in role 11. 6 CLEVELAND FIRE BRIGADE AGEING BETTER MIDDLESBROUGH OUTREACH PROJECT TEAM LEAD NATIONAL JOINT COUNCIL FOR LOCAL GOVERNMENT SERVICES CONDITIONS OF SERVICE Contract: Temporary until 31 st March 2017 Role/Grade: Grade F ( 24,472-27,123) Payment of Salary: Direct to Bank or Building Society on the 15 th day of each month Annual Leave: 26 days rising to 31 after 5 years continuous service Hours of Work: 37 hours each week (an annualised hours working scheme is in operation) Pension Arrangements: Career Average Local Government Pension Scheme (Optional) Car User Status: A pool car will be made available for business use as necessary Uniform: A full uniform will be provided The successful candidate will be required to undergo an Enhanced DBS check
