Proposal for IHE Development Cycle: Review of IHE ITI White Paper: Template for XDS Affinity Domain Deployment Planning from HIM Perspectives

2016 Proposed Effort AHIMA seeks to continue our collaboration with IHE experts to define how HIT products can support information governance policies, rules and procedures within and across healthcare organizations to address users challenges with HIT adoption

Proposal is built upon the AHIMA-IHE White Paper : HIT Standards for HIM Practices Published September 18, 2015 URL:

The Need: Patient Care Safety National Institute of Standards and Technology (NIST). Technical Evaluation, Testing, and Validation of the Usability of Electronic Health Records: Empirically Based Use Cases for Validating Safety-Enhanced Usability and Guidelines for Standardization. 5 Addressing the Need Addressing issues with EHR adoption: 1. Clinically relevant information is not available at the task at hand 2. Inadequate documentation 3. Inaccurate information and 4. Irretrievable information

Review of IHE ITI White Paper: IHE ITI White Paper: Template for XDS Affinity Domain Deployment Planning, 2008 We propose to validate/update the 2008 white paper as needed to assure that organizational policies, rules and operational procedures are represented and followed adequately in the HIS. These policies, rules and procedures are needed to support HIM practices within the information lifecycle including use of terminological resources and support of standardized document format and content (case definition templates), document metadata management, clinical workflow (clinical pathways) and other relevant information governance issues.

Review of IHE ITI White Paper: IHE ITI White Paper: Template for XDS Affinity Domain Deployment Planning, 2008 Objectives: 1. Validate IHE views on the HIM practices as they were depicted in Update the white paper as needed to address user needs on HIM practices outlined in the IHE-AHIMA White paper and other publications and 3. Enhance IHE methodology/a model for collaboration between IHE and professional health association (s) users. Thus IHE profiles will be responsive to the user needs.

Use Case The use case for this effort is based on the Use Case 1 from the 2015 IHE-AHIMA white paper entitled All documents are accounted for. We are focusing on specifying the rules and documentation (assembly of documents) for a specific function of the episode of care (i.e., scenario) such as patient registration and transition of care. Other functions (scenarios) of the episode of care (triage, assessment, testing, and diagnosis and care plan) are out of scope of this effort.

Use Case: All Documents Are Accounted for Scenario 1: Documents for patient registration Healthcare organization s Form Committee defines rules and documentation that is needed for patient registration. We will use the IHE ITI Template for XDS Affinity Domain Deployment Planning to define representation of the Form Committee directives to have all necessary documents and information available in the EHR for data capture. Scenario 2: Documents for transition of care Healthcare organization s Form Committee defines rules and all documentation that is needed for transition of care. We will use the IHE ITI Template for XDS Affinity Domain Deployment Planning to define representation of the Form Committee directives to have all necessary documents and information available in the EHR to assemble documents for transition of care.

Risks 1. Both AHIMA and IHE Teams are still in a learning phase with regard to amalgamating HIM practices into HIT interoperability standards. Building common understanding may require more time that we anticipate. 2. The review of the IHE ITI White Paper: Template for XDS Affinity Domain Deployment Planning may indicate that other IHE profiles in addition to HIE white paper and DAF implementation guide need to be reviewed as well. 3. There is a need to control project scope in terms of (a) number of clinical scenarios and (b) number of documents by scenario(s) that will be included in the review. 4. There is a potential overlap with IHE Content Committees profiles as well as IHE ITI Committee profiles focused on the content of a specific document, e.g., Enhanced Content proposed profile.

Commitment We are committed to work through the challenges listed above building on successful development of 2015 White Paper. Our commitment: 1. AHIMA Staff members (Harry Rhodes, Diana Warner, Anna Orlova) 2. AHIMA Task Force: HIT Standards for HIM Practices (over 20 SMEs) 3. ITI Experts agreed to assist with the review
