It is expected that the incumbent will have a minimum of 3 years experience in dementia care at a senior level. 3. Tertiary qualifications in a related clinical field with specialist knowledge or experience in dementia care is essential. 4. Strong ability to improve the quality of life for people living with dementia and to be able to impart that knowledge to managers and staff to improve their ability to support people in need. 5. Displays initiative and an attitude of service 6. Ability to work independently and as part of a team 7. Strong negotiation and conflict resolution skills 8. High level communication and interpersonal skills 9. Strong planning and organising skills with the ability to prioritise tasks to meet deadlines. 10. 2 2. Context and Scope HammondCare is an independent Christian charity that exists to improve quality of life for people in need. We specialise in aged and dementia care, palliative care, rehabilitation, mental health services for older people, and other related health and aged care services. HammondCare seeks to embed evidence based best practice in its services and we provide these services through home care, sub-acute hospitals and residential aged care. Bringing these health, hospital and aged care services together, HammondCare has been able to develop innovative, flexible care models designed to serve people with complex health or aged care needs, regardless of their circumstances. HammondCare works to provide its staff with support, career development opportunities, education, responsibility and recognition, work/life balance, and satisfying roles. The Dementia Centre was founded by HammondCare in 1995 as an impartial resource and provider of research and expertise to the aged and dementia care community and is key to HammondCare s commitment to excellence in dementia care. Summary of Position There are two main operating arms within the Dementia Centre: Consultancy support and advice on dementia care and Dementia Support Australia (DSA). DSA delivers the Severe Behaviour Response Team (SBRT) and the Dementia Behaviour Management Advisory Service (DBMAS). As a Consultant working within the Dementia Centre, you may be asked to work in any one of the two areas of focus, depending on your skills and experience. This role description outlines the requirements of each area. More about the role The role of Consultant provides knowledge, skill and advice on matters relating to excellent dementia care in clinical and non-clinical matters that is in line with HammondCare s Mission, Mission in Action and approaches to dementia care. The essence of this role: Providing dementia specific expert consulting and advisory services within and outside of HammondCare Delivery of the DSA (incorporating SBRT and DBMAS) service, that includes assessment, case management, supporting, mentoring and modelling to reduce the impact of behaviours on people living with dementia, their carers and staff Key responsibilities 1.0 Dementia Centre internal consulting assignments Mission in Action Delivering Results Excellence in Work Adapting Approaches As a member of the Dementia Centre team, implement the Dementia Centre s consistent dementia specific case management approach across

3 HammondCare and to external residential and community services. Utilising a set of tools and techniques to support case management. Be available to support and guide facility and service managers and coordinators with patient, resident & client issues where your dementia specific knowledge and expertise can improve the quality of life for people in need. Assessing residents and clients with behaviours of concern and providing resources and ideas for effective care provision and support for that person s needs. Liaising with relatives and representatives in relation to complex care needs. Be competent in the use of HammondCare s environmental audit tools and utilise these to report on key aspects of environmental design, Contribute to the sharing of best practices in relation to dementia care. Includes attending groups such as design schools, clinical forums, leadership meetings, working groups as required Review and provide analysis of clinical indicators and provide recommendations for improvement or action for HammondCare and external customers Be available to provide managers with support and advice on daily operational issues. Liaise with the Quality, Safety and Risk team in relation to trigger audits required resulting from observations focusing on areas of risk in particular areas with a clinical or lifestyle focus. Includes attending Quality Framework reviews and providing ideas in relation to innovation or advice on areas for improvement. Work with Dementia Centre education team to provide technical expertise and to support appropriate dementia

4 specific training and coaching as required. 2.0 Dementia Centre External Consulting and education assignments 3.0 Dementia Centre team membership 4.0 Delivery of DBMAS / SBRT service Mission in Action Delivering Results Adapting Approaches Mission in Action Providing Leadership Adapting Approaches Mission in Action Delivering Results Excellence in Work Undertake initial assessment of external customers needs Utilize a set of dementia specific tools to inform assessment Provide reports and presentations to external customers and participate in peer review Use the Dementia Centre customer management system to record key information on the consultancy service Contribute to the development of consultancy services Contribute to the range of activities of the Dementia Centre, including promotion of services, support at events and conferences and contribution to the development of research and publications. Access current evidence based guidance and knowledge, ensuring it is aligned with the Dementia Centre; utilise this to inform all activities in the role. Participate, contribute and learn as part of the Dementia Centre team in team meetings, professional supervision and team activities. Work on other projects and assignments as required and in line with areas of skill, expertise and knowledge. Ensure that customers and clients of the service are the top priority, receiving excellent service and the support appropriate to their needs and in accordance with the core elements of the DBMAS / SBRT program Appropriately escalate issues and concerns to your delegated line manager Carry out daily activities and client support in line with the contractual requirements and HammondCare s Mission, Motivation and Mission in Action

5 5.0 Administrative Duties Mission in Action Delivering Results Excellence in Work Ensure telephone, face to face, telehealth or service delivery to clients operates in line with the underpinning principles of assessment and intervention within the psychosocial case management approach of the Dementia Centre. Demonstration of best practice principles for managing behavioural and psychological symptoms of dementia, utilising DCRC / DBMAS practice guides and the Dementia Centre evidence based knowledge model, experience, practice and expertise. Demonstration of best practice principles for managing severe behavioural and psychological symptoms of dementia, utilising SBRT practice guides and HammondCare experience, practice and expertise. Ensure systems and practices in each team operate within the DBMAS / SBRT Service Delivery Pathway. Work with clinical specialists, resources, including external consultants and brokerage services required for the delivery of services needed by clients in line with the Pathway. Work closely with resources and interventions as required. Participate with the Dementia Centre and Hammond College team in appropriate training. In conjunction with the Team Leaders and Manager, utilise an agreed set of assessment tools and case management techniques to support client needs. Escalate challenging or difficult cases and situations to your line manager as required Ensure that all data is entered into the CRM system and that data entry and record keeping and information security is maintained. Ensure after hours hand-over and hand-back arrangements are followed up effectively, ensuring no case details

Includes knowledge clisters, attending groups such clinical forums, improvement meetings, working groups as required, Ensure operation within budgetary boundaries, following HammondCare s Policies and Procedures, including the Approvals and Delegations Policy, as appropriate. Undertake annual Dementia Centre accreditation and meet required standard to remain a consultant Participate in a peer review model of work, including and review of reports, Be available to support and consultant colleagues with difficult or challenging client issues as required Ideas are encouraged and explored Contribute to evidence based knowledge and learning forums where you encourage, contribute to and listen to feedback You intentionally support the professional development of yourself by equipping yourself through your Annual Accreditation and Annual
