2 John Gardner described leadership as the process of persuasion or example by which an individual or team induces a group to pursue objectives held by the leader or shared by the leader and his or her followers. Senior management is a vital stakeholder in the VSL process, without senior management participation and buy VSL will not succeed. When visible safety leadership is present, employees not only feel responsible for their own safety, they feel responsible for their peers safety, and the employer supports them acting on that responsibility. Employees have the necessary tools and methods, as well as a feeling of personal empowerment to make an impact on how work is performed. 2

3 Every company desires safe operations, but the challenge is to translate this desire into action. Written rules, standards and procedures while important and necessary, are not enough. Companies must develop a culture in which the value of safety is embedded in every level of the workforce. Safety culture is the unwritten standards and norms that shape mind-sets, attitudes and behaviours. A culture of safety starts with leadership, because leadership drives culture and culture drives behaviour. Leaders influence culture by setting expectations, building structure, teaching others and demonstrating stewardship. A commitment to safety and operational integrity begins with management. But management alone cannot drive the entire culture. For a culture of safety to flourish, it must be embedded throughout the organization 3

4 Do workers understand what the weak signals are, or could be, for the tasks they perform? Are workers able to identify and discuss the hazards and risks associated with their work? Are workers and supervisors aware of their own biases and do they seek input from others to mitigate as needed? Have lessons from incidents where weak signals were missed been embeded into the work and in behaviour? Is there sufficient communication about Chronic Unease and about how to apply the Chronic Unease mindset? Are workers empowered to stop work when they identify unsafe or noncompliant actions? Do workers feel comfortable intervening? 4

5 Are work activities and information shared through toolbox talks, JSAs, FLHAs, etc. aligned? Do workers and supervisors have the same areas of concern? Was communication between and amongst workers and supervisors suitable for the tasks? Did worksite behaviour demonstrate commitment for safety? Were exclusion zones understood and in place? Is hand safety, hands free and hand tools understood and used? 5

6 Those who make the key decisions as to how an organisation is run are integral to determining, supporting or undermining the organisational culture Are issues with potential to prevent worker safety addressed swiftly and sufficiently? Are the Life Saving Rules reviewed on a regular basis? Do Leaders support workers when extra time is required to ensure safety and clarity prior to starting a job? Did workers have ideas about how Shell could prevent people from getting hurt? Are workers comfortable with intervening on Shell personnel? Do workers/supervisors have adequate information about the standards that they require? Are any workers located in "out of sight" locations? Are appropriate mitigations in place for the risks associated with their work? Are supervisors engaging effectively and regularly with workers to ensure open lines of communication? 6

7 Did workers use the PAUSE process to identify hazards and changing conditions? Do all workers work as a team/participate in the PAUSE? Were the workers able to lead you through a PAUSE? Did the workers PAUSE appropriately or frequently during a job? Have workers on the worksite taken PAUSE Training? Are PAUSE submissions audited / recognized 7

8 Do workers understand the purpose of a JSA and use them effectively to achieve that purpose? Does every task have a JSA? If not, what is the process to ensure that this does not occur again? Are the hazards noted in the JSA specifc to the work and worksite, and not generic or overly broad? Is everyone engaged in the discussion about hazards of the job, worksite, and controls required? Did everyone review the JSA prior to the task? Did the JSA discuss the Line of Fire? Did the JSA discuss Hand Safety? Did the JSA discuss exclusion zones? 8

9 Are workers aware of energy hazards? Were there proper precautions around moving vehicles? Are workers aware of line of fire hazards and their impacts? Is access restricted to areas posing danger? Are barricades and signage in place? Are only authorized people allowed to enter a restricted zone? Do workers know where they can and cannot place their hands and body when working with rigging equipment? Is appropriate PPE selected for the task being completed? Is the right PPE available? 9

Are appropriate type and quantity of "hand protection made available to workers? Are the various types of "hand protection" being used? Are appropriate type and quantity of "hands free" tools made available to workers? Are the various types of "hands free" tools being used? Does the worksite orientation address areas of hand risk and the appropriate control measures put in place? Is there suitable and sufficient signage to indicate where hands and fingers are at risk? Is hand safety included as a discussion topic at meetings? Are workers aware of "Hands Free" guidance, and when to PAUSE if they cannot be followed? 10

11 Are the workers aware of the Journey Management requirements? Do the employees have IVMS in their vehicles? Do the workers understand the risks and issues that can prevent them from driving/working safely? Have employees attended an accredited Defensive Driving Course? Is road transportation included as a discussion topic at meetings? Are the workers aware of the Hot Line Number? Do the vehicles appear appropriate for the work activity and conditions? 11

12 Does the worksite have enough time and resources to ensure appropriate planning? Did the workers know the purpose of a Permit and how to use it? Can they explain activities requiring a permit? Are workers engaged and do they understand the work scope, hazard controls and personal responsibilities? Was all permitable work being done under a Permit? Hazards identified, controls in place? Have those involved in permitting received appropriate training for their role? Is the site supervisor versed in roles of Job Supervisor, Job Coordinator, and Area Authority? Were all Permits posted on the Permit Board? 12

13 Are Preventative Maintenance and Corrective Maintenance of Safety Critical Elements up-to-date? Do workers know when was the last LOPC? Do workers understand what actions to take in case of a significant barrier event? Do workers understand the process for reporting LOPC or Barrier Event? Have workers participated in a process safety drill to test emergency response plans? Is a hose management program in place? Are drums stored in secondary containment? Are tanks double walled or contained within secondary containment? 13

14 Are workers familiar with LFIs and do they review applicable learnings? Do supervisors communicate applicable learnings and ensure actions or recommendations are adhered to? Are LFIs reviewed as part of the job planning process to minimize the risk of similar events occurring? Is there easy access to LFIs and are they shared appropriately to personnel? Are there relevant and recent LFIs posted on location? Are LFIs included as a topic at safety meetings and other job planning events? 14
