12 Linking with Global Agenda The World Health Organisation Framework for people centred integrated care services is used to inform the overall vision and focus for the CeIC activity. Strategy 1 Empowering and engaging people in communities through education and focused discussion PAG IAG in flight Strategy 3 Reorientating the model of care towards defined Slaintecare service priorities, optimising harmonisation of data using state of art health informatics standards Strategy 4 Co-ordinating services within and across sectors is progressing through research proposals in development with patient groups and industry (Innovation2.0). Key principles empowerment and engagement of citizens, transparency in working approach and cultivating and enabling Self Management Support ( WHO, 2016 link )

24 Agile Co-design Approach based on Standards to Optimise Interoperability & Build Capacity

25 Conclusion To achieve clean data meta data registries and data standards are required. The CeIC in partnership with the IA are working on building capacity within HSE to set in place a core interoperability framework to support ehealth Ireland. This cant happen in isolation standards need to be instigated or a direct impact on transformation will occur. Decisions need to be made and implemented. Identity is the first block
