5 Five level rating scale Exemplary (E) The employee significantly and consistently exceeded performance expectations, and the quality of work overall was exceptional. Exceeds Expectations (EE) The employee frequently exceeded performance expectations, and the quality of work overall was outstanding. Achieves (A) The employee overall achieved and at times exceeded job expectations and the quality of work overall was of high quality. In Development or Improvement Needed (I) Employee is still developing competency or did not consistently achieve expectations; some improvement is needed. Unsatisfactory (U) Employee performance was consistently below expectations; immediate corrective action is necessary. Page 5

6 Self Evaluation Two ways to provide pre-evaluation input: Employee Self-Assessment form Employee provides input on form similar to standard evaluation Rates standard competencies/factors using 5-levels Attachments only Supervisor & employee agree on form and format Employee attaches at document tab Page 6

12 Executive Review Process Instructions Employee Information Certifications and Safety Overall Rating Attach Documents Draft may be started by Exec or designee Save, but do not submit Check fields for accuracy Select correct menu items from drop downs Enter eval working title, time frame or exception, delivery date Required training Position Description review Safety compliance Check most appropriate rating Attach feedback and objectives discussed with employee Submit to employee for optional response Close process Page 12
