Identity Documents: Passport Bio Page/ Birth Certificate: It is preferable that you upload a coloured copy of your passport bio page if you have one. The passport bio page provides a picture of you as well as your relevant identity details. However, if you are not currently in possession of a passport you will need to provide an official copy of your Birth Certificate along with a certified copy of a recent passport size photo (stating the photo is a true likeness of you ) One recent passport size photograph Evidence of name change (if applicable): It is important to include official documents addressing the reason why any of the documents submitted for skills assessment purposes may not match. For instance, this is common for people who have married since completing their studies. However, other reasons may also apply such as changing your name by deed poll. Please be sure to address any anomalies if this applies to you Personal Details: Other Some countries issue their citizens with official identity documents such as the Chinese Personal Identification Cards (or equivalent). If this applies to you, we ask that you please submit a copy for skills assessment purposes. Resume/CV: If applying for a formal skills assessment, we encourage you to submit a CV as it may benefit the assessment process. A resume/ CV allows you to outline both your academic studies (describing the major focus of your studies including providing details of any major projects undertaken, internships etc as part of course requirements) as well as describing your employment/ career history in your own words. In order to provide us with relevant information for skills assessment purposes (for both your educational studies and any relevant periods of employment), please refer to the information in the Explanatory Notes (under the transcripts and employment section). 6 September 2016 Version 1.0 Page 1 of 6

2 Qualification/s Documents: It is requested that you provide us with the entire set of official documents that were issued to you on completion of your academic/ qualification studies. Your qualification award (such as your degree, diploma or certificate), must be submitted in the original language (if applicable) as well as an English translated version. The English translation documents must be performed by a registered translation service. Award certificate/ Completion letter: If you have not yet received your qualification certificate (and your official transcripts do not clearly state that you are now eligible to graduate OR that the qualification has been conferred/ awarded) the official completion letter you submit for skills assessment purposes MUST be issued by the Academic Registrar of the university or the Central Student Administrative area of the TAFE (or international equivalent). A letter of completion issued by a lecturer/ professor/ faculty will not be accepted for skills assessment purposes. Transcripts: Transcripts are the official copy of the courses/ units of study taken by a student as part of their academic program/ qualification/s. Transcripts should also provide the grades earned by the student throughout the course. If you attended a college affiliated to the awarding university, the transcripts submitted for skills assessment purposes MUST be issued by the awarding university (and not just by the college you attended). This is especially important for the final year/s of study. Often transcripts vary by the information contained in them. However, they are unique to each student and are required for skills assessment purposes. Therefore, if your transcripts do not provide sufficient information regarding the units of, or concentration of study undertaken, it is important that you provide some form of evidence regarding the main focus of your course. This is particularly important for any unspecified electives that are listed, major projects, thesis or dissertation requirements and/ or any other element of the program performed as a significant part of the advanced/ final years of your qualification/s. We request this additional information as, generally speaking, the final years are where the main focus of learning is usually concentrated. In this situation, please provide us with details describing course requirements (preferably under the qualification/ education section of your CV). There, you can detail what any major projects involved as well as the nature of your responsibilities performed as part of an internship (if applicable). Applicants from specific regions of the world Peoples Republic of China (PRC) Certificate of Graduation It is noted you hold qualifications that have been awarded by universities/ educational institutions from within the People's Republic of China (PRC). Therefore, you will be required to have your qualification/s documents verified by our organisation prior to lodging a skills assessment. 3 Europe Diploma Supplement Generally, only issued by higher education/ academic institutions within signatory countries (mostly from within Europe) to the Bologna Process/ Declaration (voluntary process; implemented 1999) Nigeria A verified copy of the award certificate and full academic transcript sent directly in a sealed envelope from the awarding body to VETASSESS. Pakistan An attested HEC (Higher Education Commission Pakistan) copy of the award certificate and full academic transcript sent directly by courier in a sealed envelope from HEC to VETASSESS. 4 Employment Documents: Please upload the documents that relate to your most recent employment position first (or the position that you want us to use as the benchmark for your other employment positions, usually the employment role that, you think, holds the most relevance to the occupation you are selecting for skills assessment purposes). It is important to try to provide us with documents that assist us to establish your scope of authority within the organisation as well as the level of your responsibilities. For example, a letter of reference simply listing ANZSCO listed occupation titles and/ or tasks does NOT always prove helpful. Contract documents: Please submit only the official appointment letter/ contract document page/s that describe the position you were appointed to. However, we only require the main contract document page/s that describe the following: o The primary documents stating your contracted position title and commencement date only (we do not need pages detailing your working terms and conditions unless they detail your employment tasks. However, any pages that state your remuneration package/ salary/ annual/ consultative income etc will be considered useful). Organisational Chart: The organisation chart is a diagram showing graphically the relation of one official to another, or others, of a company. It is also used to show the relation of one department to another, or others, or of one function of an organisation to another, or others. This chart is valuable in that it enables one to visualise a complete organisation, by means of the picture it presents. A company's organisational chart typically illustrates relations between people within an organisation. Such relations might include managers to sub-workers, directors to managing directors, chief executive officer to various departments, and so forth. PLEASE NOTE organisation charts may not always be made available (or not applicable) to all employment positions. Payment Evidence: It is recommended that the following (official evidence) be submitted for formal skills assessment: o Pay slips (ideally the first and most recent) o Payment Summaries for taxation (bearing company and applicant name) o Taxation Records of Assessment (bearing company and applicant name) o Bank Statements showing at least two salary payments, your name and the employer's name 6 September 2016 Version 1.0 Page 4 of 6

5 o Employment-linked insurance/ superannuation records (bearing company and applicant name) Scholarships/ government grants providing taxation exemption: Exemption from paying tax (this may normally be issued by the funding university/ college/ education or research institute or the relevant Government authority granting the exemption. The official document must bear the funding entity s and the applicant name) Sole practitioner/ Self-employed: As a private/ sole practitioner (engaged in any industry), applicants are advised to provide evidence of sole trading through official statements issued by their (registered) Accountants and/ or Legal teams. Sole traders will also need to provide evidence of company registration details, client testimonials and/ or invoices, bank statements showing regular income, official taxation evidence etc Please note, Accountant testimonials should indicate as much information regarding the applicant's primary tasks and responsibilities in order to assist the skills assessment. Position description: A Position (or job) description is generally referred to in order to describe the primary tasks, or functions, and responsibilities that an employee is required to perform within their contracted employment role. It may include to whom the position reports, specifications such as the qualifications or skills needed by the person in the job, and a salary range. Statement of Service: Please include a high quality colour scan of any Statements of Service or reference letters from each employer for whom you have worked in positions related to your nominated occupation within the past 5 years. Employment reference letters should be on the official company, university or government departmental letterhead of the organisation which employed you. Official letterheads should include the company/ organisation's full address details, telephone, and website addresses. The content of the letter must be issued or endorsed by your supervisor or the manager of the human resources department (whichever may be applicable). It should be signed by an authorised person whose name and position is legible (typed or stamped) below the signature and should include the direct employment contact details of the person writing the reference and their designation. Statements of Service (SOS)/ reference letters should also typically, include the following: a. The exact period of your employment b. The required working hours per week c. Whether the employment is/ was permanent or temporary d. Whether the employment is/ was full-time or part-time e. The position/ s held f. The main five duties undertaken g. 6 Other Supplementary Employment Evidence: Applicants may include other evidence to support their application. The document types listed below are examples of supplementary employment evidence only and cannot be accepted in lieu of formal or official forms of paid employment evidence. o License or Registration documents and details of the requirements for obtaining the license or registration o Organisation Chart (on company letterhead) highlighting applicant s duties o Evidence of professional development completed during applicant s employment o Prizes/ Certificates or other forms of commendation Statutory Declarations and Affidavits Please note, in order for VETASSESS to consider the contents of a Statutory Declaration for skills assessment purposes, the statutory declaration (or equivalent legal document) must be properly executed by the appropriate legal professional/s in the country of origin. Applicants may need to seek appropriate legal advice as to how this is carried out in the country of origin. Also, a skills assessment outcome cannot be provided based on the contents of a statutory declaration or affidavit alone. VETASSESS will require a range of official employment documentation as evidence to support of any claims made through the use of statutory declarations. An example of employment evidence that may be produced by applicants without the need to approach their current employer includes the following: Official taxation documents (group certificates issued by the company as well as official taxation receipts), signed contract documents, appointment letters, pay slips, letters of promotion, employment training certificates (if applicable) etc 6 September 2016 Version 1.0 Page 6 of 6
