2 2 ESIC Model Hospital, Rajajinagar, Bangalore is a 500 bed Hospital and a Medical College and Post Graduate Research Institute is also attached with it. This Hospital invites sealed quotations from the Photo copier (Xerox) service providers for providing two Photo copier on contract basis by installing the photo copier in the premises of the ESIC Hospital for a period of one year under single bid system. Contract Period is initially for one year, extendable from year to year basis, subject to satisfactory performance of the firms during the contract period and subject to maximum of Two years (including the initial one year) Minimum eligibility: 1. The agency / Service providers should be Bangalore based. 2. Should have minimum of 3 years service in the field 3. Should not have been blacklisted by the Govt. / PSUs The tender document can be downloaded from the website & which should be accompanied by the requisite Earnest Money Deposit(EMD) of Rs. 5000/ - in the form of demand draft/pay order payable at Bangalore in favour of the ESIC MH Fund Account No. 1. Period of downloading of tender form: 24/08/2014 to 23/09/2014 and it is free of cost. The bid should be accompanied with the EMD of Rs. 5000/- The tender, complete in all respects, should be dropped in the sealed tender box available in the General branch II, ESIC Model Hospital, Rajaji Nagar, Bangalore 10. Last date of submission of Bid : 23/09/2014 at Hrs. If it happens to be holiday, on next working day at same time. Late / delayed tenders shall not be opened/entertained under any circumstances. Date and time of opening of Bids: The Bids shall be opened on 23/09/2014 at Hrs, if it happens to be holiday, then on next working day at same time and same venue by the duly authorized officer/team of officers of ESIC Model Hospital, Rajaji nagar in the presence of all such bidders who wish to be present. Authorized representatives of the quoting parties may be present for the opening of the bids, if they desire so. This Hospital invites sealed quotations from the Photo copier (Xerox) service providers for providing two Photo copier on contract basis by providing the photo copier in the premises of the ESIC Hospital for a period of one year under single bid system. 2. Contract Period is initially for one year, extendable from year to year basis, subject to satisfactory performance of the firms during the contract period and subject to maximum of Two years (including the initial one year) 3. Minimum eligibility: 1. The agency / Service providers should be Bangalore based. 2. Should have minimum of 3 years service in the field 3. Should not have been blacklisted by the Govt. / PSUs 4. Preference will be given to parties doing similar job in govt. office 5. The Medical Superintendent, ESIC Model Hospital, Rajajinagar, Bangalore 10 shall provide the paper for taking photocopy, space, Manpower for operating the machine & electricity for using the Photocopier machine at free of cost. 6. The contractor has to provide and install the Photocopier (Xerox) and consumables like Cartridges for the machine. 7. The maintenance and repair work for the Photocopier is also the responsibility of the Contractor. 8. The number of copies required per day may be around 500 for each machine (A4 and A3 size). No maximum and minimum copies assured. 9. Bid Security/ Earnest Money Deposit (EMD) a) EMD for Rs.5,000/- (Rupees Ten Thousand only) in the form of Demand Draft/Bankers Cheque from any Schedule bank drawn in favour of ESI Fund A/c-1 payable at Bangalore shall accompany the bid. b) EMD shall remain valid for a period of 30 days beyond the final validity period of bids (120 days). c) A bid received without Bid security (EMD) shall be rejected at the bid opening stage. d) The earnest money shall be refunded to the unsuccessful Tenderers after finalization of the contract. e) It shall be refunded to the successful Tenderer on receipt of Security deposit or shall be allowed to be adjusted towards Security Deposit. f) No interest is payable on the EMD. g) The bid security may be forfeited: (i) If a bidder withdraws his bid during the specified period of bid validity specified in the bid document. (ii) In the case of successful bidder, if the bidder fails to : o Submit the surety bond after awarding of the contract o furnish the Performance security within the time specified in the document 9. Submission of Bid The bid envelope should contain the following: a) The duly filled Application form b) EMD for Rs.5,000/- (Rupees Five Thousand only)

4 4 c) The bid should be in a sealed envelopes superscribed as Tender for providing service of photo copier in ESICMH Bangalore -10. N.B. All the documents submitted in the bid must be legible and self attested along with the seal of the firm. Otherwise the bid is likely to be rejected 10. Deadline for submission of bids 1. The above envelopes shall be sealed and addressed to The Deputy Director (General Branch - II), ESIC Model Hospital, Rajajinagar, Bangalore 10 and must reach on or before the 23/09/2014 by 2.00 p.m. 2. No bids will be received/accepted after the expiry of the prescribed date and time for submission of the bids. 3. The Medical Superintendent, ESIC Model Hospital, Rajajinagar, Bangalore - 10, may, at his discretion, extend the deadline for submission of bids. 11. Bid Opening The bid shall be opened by the Officers authorized by the Medical Superintendent at p.m. on the last date (23/09/2014) for submission of the bids in the presence of the bidders or their representatives duly authorized by the bidder who wish to be present. 12. Contacting the officials 12.1 No bidder shall try to influence the Purchaser on any matter relating to its bid, from the time of the bid opening till the time contract is awarded Any effort by the bidder to influence the Purchaser in the Purchaser s bid evaluation, bid comparison or contract award decision shall result in the rejection of the bid. 13. Non transferability : This tender is non transferrable 14. Right to accept / reject any or all Bids The Medical Superintendent, ESIC Model Hospital, Rajajinagar, Bangalore 10 reserves the right to accept or reject any bid including the lowest and to annul the bidding process and reject all bids, at any time prior to award of contract, without thereby incurring any liability to the affected bidder or bidders or any obligation to inform the affected bidder or bidders of the grounds for the said action. 15. Bid Prices a) The price quoted should be valid for the one year. b) Any conditional acceptance is liable for rejection of tender. c) Rate for copying A3 and A4 size paper may be quoted separately. d) The rates/ prices including the taxes, if any, should be quoted in Indian Rupees only in words as well as figures. e) The Contract has to quote the price in the bid and who has quoted the least in comparison to the other tenderers shall be considered as the lowest tenderer. 16. Awarding of contract a) A formal contract shall be entered into with the successful tenderer. b) The successful tenderer will have to deposit a Security Deposit of Rs /- by way of Fixed Deposit Receipt (FDR) from a Scheduled bank and hypothecated In favour

5 5 of the Medical Superintendent, ESICMH, Rajajinagar, Bangalore -10, valid for 60 days beyond the expiry of period of two years contract. c) The successful Tenderer will have to deposit the Security Deposit and commence the work within 15 days of acceptance of tender. Otherwise the contract will be cancelled and EMD will be forfeited. d) The contract/tender, if awarded, shall be valid initially for one year from the date of award of contract subject to continuous satisfactory performance and on failure on this aspect by the contract the Competent Authority will reserve the right to terminate the contract. The period of the contract can be extended for further period at the discretion of the Competent Authority, to the maximum of one year, on terms and condition of the ESIC MC. 17. Mode of Payment details : a. Bills in Triplicate should be submitted once in a month. b. The payment shall be made through RTGS only. Hence the Bank details are to be submitted alongwith the bills. 18. The Medical Superintendent has the right to authorize / appoint / depute any person to supervise the services. 19.If for any reason the Photocopier goes out of order, it will be the responsibility of the contractor to arrange to supply the Xerox machine. 20.In case of dissatisfied service either in quality or in any other form, the Medical Superintendent reserves the right to terminate the contract with seven days notice. 21.The Medical Superintendent also reserves the right to terminate the contract before the contract period expiry with one month notice. 22.Security Deposit of the Contractor will be forfeited in case of breach of any condition stipulated herein. 23. For any dispute regarding quality / service and rates the decision of the Medical Superintendent shall be final and binding to the Contractor. 24.All disputes relating to this tender can be legally resolved through courts in Bangalore only. 25.The Medical Superintendent, ESIC Model Hospital, Bangalore -10 is the Competent Authority to renew Contract or to terminate contract. 26.The Medical Superintendent, ESICMH, Rajajinagar, Bangalore -10 the right to reject/cancel all or any tender without assigning any reason thereof. NOTE: The Firm who have already submitted tender accordance with the terms and condition of our tender dated: 21 July 2014 need not to apply again.
