Uttar Pradesh State Rural Livelihoods Mission Society has received a grant from the MoRD, GoI and Government of UP towards the implementation of Ajeevika program and intends to utilize a part of the proceeds of the funding to eligible payments under the contract for which this invitation for bid is issued. 2.1 Total requirement of the manpower at our different DMMU s and BMMU s under UPSRLMS is divided into 4 regions viz East, East-Central, West and Bundelkhand-Central. Each service provider agency is permitted to apply for maximum two regions only. If the department find any agency has applied for more than two regions the department has full right to reject the complete bid of that agency. 3. Important Dates and Times/Bid Document 3.1 Last Date & Time for Submission of bids: at PM 3.2 Date & Time of Bid Opening: at PM

Eligibility Criteria 4.1 Minimum average turnover of more than lacs in the last three financial years i.e , and Must furnish Registration number of firm/agency & Service Tax, Income tax, EPF/ESI registration number along with certificate of registration as applicable. 4.3 The bidder should have similar experience of working in at least six govt. organizations. Out of which the contractual amount in one single contract should be of at least Rs lacs and rest of the orders should not be less than Rs lacs. 4.4 EMD amount of successful bidder would be converted into Performance Guarantee. 5. Bid Price a) Interlineations, corrections, erasures and/or over writings shall be valid only if initialed by the persons or persons signing the bid. b) Applicable Service Tax must be clearly stated. All taxes, insurance, and other levies payable by the contractor under the contract shall be included in the quoted price. c) The rates quoted by the bidder shall be fixed for the duration of the contract and shall only be subject to revision in Minimum Wages by Govt. of Uttar Pradesh. The quoted rates should be inclusive of all statutory taxes etc. d) The Prices should be quoted in Indian Rupees only. e) The I.T., T.Taxes etc. will be deducted at source as per rules. 6. Submission of Bids: 6.1 Tender form can be obtained from the office of Mission Director, Uttar Pradesh State Rural Livelihoods Mission in any working day between 10 AM to 5 PM by depositing the DD of Rs in favour of Uttar Pradesh State Rural Livelihood Mission, payable at Lucknow. 6.2 There will be two bid process. A bidder shall submit technical bid in sealed envelope 'A' along with all related documents verifying the eligibility criteria will be properly flagged and the page-wise details of which will be given on the very first page of the proposal while the financial Bid will be given in a separate sealed envelope 'B', and then these two sealed envelope will be submitted in an another sealed envelope. A bidder shall not submit more than one Bid. 6.3 If a bidder will not submit the technical and financial bid separately as given above then his bid is will not be entertained. 6.4 Envelope containing bids must bear, on the cover itself, name and full address of the bidder, name of the item bidding for, and date & time of bid opening shall also be super-scribed on the cover. 6.5 For bids submitted by post or courier, it is bidder s responsibility that bids reach the purchaser s office before the scheduled time of closure of submission of bids. 7. Bid Security 7.1 The bid security is required to protect the Purchaser against the risk of Bidder's conduct which would warrant the security's forfeiture. 7.2 Unsuccessful bidder's bid security will be discharged/returned as promptly as possible after the expiration of the period of bid validity prescribed by the Purchaser. 7.3 The bid security may be forfeited: (a) if a Bidder (i) withdraws its bid during the period of bid validity specified by the Bidder on the Bid Form; or (b) in case of a successful Bidder, if the Bidder fails: (i) to sign the Contract or (ii) to furnish performance security.
3 8. Performance Security 8.1 EMD of successful bidders would be converted as security for performance of the services which will be returned after settlement of contract. 9. Validity Period of Bid Bid shall remain valid for a period not less than 90 days after the deadline date specified for submission. 10. Evaluation of Bid 10.1 The Purchaser will first evaluate and compare the technical bid Regionwise, of envelope 'A' determined to be substantially responsive i.e., which (a) Are properly signed; (b) Confirm to the terms and conditions, Qualification and Eligibility criteria are submitted. (c) If the Regionwise bidder is found to be qualified in the technical bid then only his Regionwise financial bid of envelope 'B' will be opened. 11. Terms of service 11.1 Contract period for this work will be initially for one year and it may further be extended up to 3 years or more depending upon the need and performance of the agency / persons deployed by the agency and mutual consent of the parties Rate of Increase in wages for the personnel on account of revision in Minimum Wages by the Government of Uttar Pradesh will be considered, in case the present payment would be below the Minimum Wages. 12. Award of contract 12.1 The Purchaser will award the contract to the bidder (i) Whose Bid has been determined to be substantially responsive and (ii) Who, in the assessment / judgment and sole discretion of the UPSRLMS, has technical and financial capability to execute the contract and (iii) Who has bided the lowest Bid price Notwithstanding the above, the Purchaser reserves the right to accept or reject any Bid, to reject Bids for some or all items and to cancel the bidding process at any time prior to the award of contract The bidder must furnish all the necessary license/certificates/tax clearance certificates from concerned authorities for carrying similar services Prior to expiration of the bid-validity-period, the award of contract/contracts by the Purchaser will be notified to the bidder/bidders, whose offer/ offers has/have been accepted. The terms of the accepted offer/offers shall be incorporated in the purchase order/agreement. 13. Terms & Conditions of Contract 13.1 Selected staffs of the Placement agency/ies i.e. in East Region there are 4 Computer Operators cum Office Assistants at our 4 District Project Management Unit (DMMUs) while 8 Computer Operators cum Office Assistants at our 4 Block Project Management Unit (BMMUs) i.e. 2 in each BMMUs. Similerly in East-Central Region there are 4 Computer Operators cum Office Assistants at our 4 District Project Management Unit (DMMUs) while 8 Computer Operators cum Office Assistants at our 4 Block Project Management Unit (BMMUs) i.e. 2 in each BMMUs. While in West Region there are 5 Computer Operators cum Office Assistants at our 5 District Project Management Unit (DMMUs) while 10 Computer Operators cum Office Assistants at our 5 Block
4 Project Management Unit (BMMUs) i.e. 2 in each BMMUs and in Buldelkhand-Central Region also there are 4 Computer Operators cum Office Assistants at our 4 District Project Management Unit (DMMUs) while 8 Computer Operators cum Office Assistants at our 4 Block Project Management Unit (BMMUs) i.e. 2 in each BMMUs. The detailed list of all the 17 districts and their 17 corresponding blocks with their requirement of Operators cum Office Assistants can be seen at Annexure-C Service agency/ies will require to supply qualified and experienced Computer Operator cum Office Assistants as per the qualification and experience mentioned in Annexure-A through wide publicity to capture the best and efficient manpower available The number of manpower may increase/decrease subject to the requirement of UPSRLMS Normally, the office hour will be 10:00 am to 5 pm. Modifications in office hours could be done as per the requirement of the DMMUs or BMMUs If any staff of the placement agency remains absent from the duty for more than one (01) day, the placement agency will have to provide alternative manpower In case if any candidate found to be non-performing, the agency will be required to replace him within 5 days All the proposed staff of the placement agency may have to face interview at SMMU for selection if required and may have to prove his capability to perform on the demand of DMMUs/BMMUs Head All the required manpower should be medically fit, well disciplined, and have to follow UPSRLMS rules The responsibility lies with the bidder for and in respect of employees concerning Minimum wages Act, Workman Compensation Act and other Labour Laws Any other jobs not specifically mentioned but reasonably implied to be executed for satisfactory completion of Job in all the respect shall have to be carried out by the personnel without any extra cost. Decision of UPSRLMS shall be final and binding in this regard The bidder will be responsible for the conduct of their personnel If the bidder fails to do the jobs satisfactorily or is unable to complete the job, UPSRLMS reserves the right to cancel the Contract and forfeit the performance security without assigning any reason UPSRLMS will not be liable or responsible for any unethical activities of the service agency The selected Service Provider Agency have to transfer the amount of Monthly Honorarium to personnel in their bank account through RTGS and provide monthly receipts of the same to UPSRLM. Any other mode of payment of Monthly Honorarium to personnel will not be acceptable and result in cancellation of contract of the Service Provider Agency. 14. Payments Payment shall be released on monthly submission of bill along with the challan of Service Tax (separate challan is to be filled for UPSRLMS) along with the statement of accounts of Honorarium of personnel supplied to DMMUs & BMMUs, after due certification of attendance from DMMUs & BMMUs. Document towards PAN issued by the income tax department, must be submitted by the bidder before release of payment. Income Tax shall be deducted as per rules. 15.
