1 UVA Employee Self-Service Benefit Enrollment Visit and select the Benefits button. Login into the Integrated System to access your Employee Self-Service account. TIP: Select Click HERE to re-set your password and follow the instructions if you need a password reset. Open the UVA Employee Self-Service Menu and select Benefits. 1
2 Read the Legal Disclaimer. Select Accept, then click Continue to proceed. If you are going to add a spouse or child(ren) to your benefits, select the Add Dependent button. TIP: Confirm Social Security Number and Birth Date for any dependents you add. 2
3 Complete all required fields, then click Apply to create a form for each dependent. TIP: Contact from the Relationship drop down menu is used to add a life insurance beneficiary that is not a spouse or child. Once you are done adding dependents, click Continue. TIP: You will have a chance later to add dependents to your benefits. TIP: Click the yellow pencil to update dependent information. 3
4 Select Enroll or View Benefits, then click Continue Don t see the Enroll or View Benefits button? Contact us for help, , AskHR@Virginia.edu. This page is informational only. Click Continue to proceed to benefit enrollments. Newly hired employees see default benefits, and can ignore this page if requesting changes. Existing employees see what benefits they currently have before they request any changes. 4
5 In the Update Enrollments form, check a box for each benefit. When done, click Continue. New employees will be able to select new hire benefits. Current employees will only be able to select changes to certain benefits based on IRS and Plan rules. Health Health Savings Account (HSA) for Basic Health Only Spousal Affidavit for Health Plan if applicable Dental Vision Healthcare Flexible Spending Account (FSA) Dependent Care Flexible Spending Account (FSA) Mandatory Retirement Basic Life Employee, Spouse, Child Optional Life Short Term Disability Long Term Disability Note: offerings may vary based on one s benefit eligibility and federal requirements (Ex. J1 or J2 visa status). TIP: You will have a chance on the next page to add dependents to your benefits. If you have dependents, check the Cover boxes next to each benefit to add them. Select Continue when done. TIP: If you are missing a dependent, select the Add Dependents button to go back and add them. TIP: If you added your dependent but don t see them here click the Add Dependent button, go back to their form, and confirm the Relationship Start Date is prior to your date of hire. 5
6 You can name your spouse, your child(ren), and/or other types of individuals as beneficiaries. Select the Enrollment Requirements link to learn more about what you should do next for retirement and life insurance. Follow the steps, and then select Continue when done. If you add or change dependents on your benefits, enroll in a mandatory retirement plan for the first time, or update your life insurance, here is where you will upload required documents or forms. Follow these steps in red, then click Continue when done. NOTE: Uploads will be reviewed by the HR Service Team, and when approved, will move to your Dependent Certifications history at the bottom of this page. Click Add Attachment select File from the drop down menu to attach each document. 6
7 Confirmation Statement Any Warning or Confirmation messages displayed at the top of the page remind enrollments are not final until you Turn in required documents before the deadline (Please pay attention to any follow-up s from the HR Service Team in case we have any questions about your attachments or completed forms.) Review and complete the Next Steps checklist on this page Print or save the Proof of Coverage PDF TIP: Click the Back button if you need to make any edits before you exit Self-Service. To return to the Employee Self Service Homepage click the Home icon, or click the Door icon and close your browser to completely exit Integrated System Employee Self-Service. 7
