1 Our Healthier South East London (OHSEL) The SEL STP Programme Director: Community Based Care Job Title: Programme Director: Community Based Care Band: 9 Responsible to: Accountable to: Responsible for: Programme Director SRO CBC Responsible for implementation of CBC strategy Introduction From 2013 to the end of 2015, the six Clinical Commissioning Groups (CCGs) in south east London (Bexley, Bromley, Greenwich, Lambeth, Lewisham and Southwark) worked with commissioning colleagues in NHS England (London) and in partnership with local authorities, NHS providers, patients, local people and other key stakeholders to develop a new five year commissioner-led, clinically-driven strategy across the six boroughs. The work produced a consolidated strategy in August In December 2015 NHSE published the requirement for the NHS to group together to produce Sustainability and Transformation Plans. These are place-based, whole health and social care system plans which are aimed at implementing the Five Year Forward view and addressing the three gaps: the health and well-being gap, the care and quality gap, and the funding and efficiency gap. South east London decided to build on the work undertaken through the OHSEL programme with a new governance structure to create a fully joint approach between commissioners and providers, and an expanded scope to include specialist commissioning and mental health. Because of our relatively advanced status we were chosen to work with NHSE as the national example STP footprint. Our STP was first submitted in October 2016, together with supporting strategies for workforce, estates, Digital Road map and communications and engagement. Details can be found on the programme s website: The development of the STP is overseen by a programme board - the Strategic Planning Group (SPG), comprising the chairs and chief officers of the six Clinical Commissioning Groups, the chief executives of the provider trusts, NHSE specialist commissioning and representation from the local authorities, Healthwatch and patient and public voices. Having developed the case for change and published the consolidated strategy in 2015, and having now developed the STP and underpinning strategies and its associated finance and 1

2 activity model, the focus of the programme now needs to be ensuring its effective implementation and delivery of the associated financial and non-financial benefits. The implementation of the STP is overseen by two programme boards, one for provider productivity and one for clinical transformation. There are three enabling streams supporting both boards: digital, estates and workforce. 1. Job Summary and key tasks This post will play an important role in delivering the south east London STP. The STP is a complex and challenging programme of work and requiring a very high standard of programme management co-ordination and leadership to ensure its success. The post holder will report to the STP Programme Director and will support him to lead the coordination and delivery of programmes The key tasks of the role will include: Leading on our collective work on the GP Forward View and the achievement of the London primary care standards Leading our collective work on the new models of care described in the Five Year Forward View. Leading our collective work on prevention and health inequalities. Leading our collective work on CCG collaboration and productivity, and the implementation of the high impact changes envisaged in the STP. 2. Key Working Relationships The post holder will be required to develop and maintain relationships with a broad range of senior internal and external stakeholders including: South east London CCGS, including governing body members, Chairs, Chief Officers, Directors of Commissioning and Chief Financial Officers NHS England (London Region) Provider chief executives, directors and clinicians Local Authority chief executives, directors and members Healthy London partnerships GP Federations Representatives of patients and the public, including the programme s Patient and Public Advisory Group South London Commissioning Support Unit (SLCSU) Other partner organisations, such as the Health Innovation Network, Health Education South London and the NHS Improvement, and wider stakeholders 3. Job Functions and Responsibilities 3.1 Delivery of the Programme 2

3 Deliver a major workstream of the STP, by working with senior clinicians and managers to implement models of care and associated benefits. Leading on our collective work on the GP Forward View and the achievement of the London primary care standards. This will involve close liaison with the London programme and CCG teams. Leading our collective work on the new models of care described in the Five Year Forward View including the development of approaches to accountable care organisations or similar variants. Working with GP Federations to enable them to develop their capacity and capability to deliver the new models of care and primary care standards. With public health and primary care colleauges, leading our collective work on the priorities identified in the STP on prevention and tackling health inequalities. Leading our collective work on CCG collaboration and productivity, and the implementation of the ten high impact changes envisaged in the STP with the aim of generating improved value and quality. Take the lead role in coordinating the development and ensuring the delivery of the CBC Programme a range of projects, either through direct management of the project or by working within multi-disciplinary project teams to deliver a number of projects. Ensure delivery of projects and initiatives on time, to agreed quality standards and in a cost effective manner, adjusting plans as required Identify interdependencies across project/functions, potential impacts in the wider system, resource requirements and other issues to ensure that the strategy as a whole is coherent and delivers its aims Develop and implement with the PMO project management systems that will provide accurate and timely data to the programme and its governance structures. Work with teams across south east London to provide direction, help and advice and support to help local planning and secure alignment between the CBC model of care and local strategies. Support the STP Programme Director in further developing the structure required to deliver the strategy and tracking, managing and delivering agreed benefits Proactively manage the risks and ensure appropriate actions are taken to mitigate or respond Communications Participate in relevant internal and external working groups and other forums to provide information and expertise 3

4 Present information and issues, and be able to explain complex subject matter in an accessible way, to a wide range of internal and external stakeholders Seek and use feedback to support the development of the strategy Engage with internal and external stakeholders on a wide range of issues relating to the programme, some of which may be sensitive, complex and contentious Nurture key relationships and maintain and develop effective networks locally, across London and nationally as appropriate Support the development of good communication and information flows within the team and across the six CCGs and partners, to support development and share best practice 3 Working Relationships Operate effectively in a flexible and demanding environment and proactively engage with NHS staff, other stakeholders including local authority staff and patient representatives, consultants and contractors. Work effectively with a variety of external agencies working on other projects and programmes within other NHS organisations as required Provide and receive highly complex, sensitive and contentious information, presenting information, to a wide range of stakeholders in a formal setting. Nurture key relationships and maintains networks internally and externally, including national networks Link with managers and members of other functions, to address inter-dependencies and ensure alignment 5. General The post holder may be required to work at any of the CCG or other appropriate sites in line with the service needs. This job description describes responsibilities, as they are currently required. It is anticipated that duties will change over time and the job description may need to be reviewed in the future. 6. Individual Responsibilities The post holder has a responsibility to adhere to South East London and all policies and procedures and relevant legislation including the requirements of any professional bodies; maintain satisfactory personal performances and professional standards and to achieve agreed objectives for their role. 7. Confidentiality All Southwark CCG staff and contractors working for the organisation have both a common law duty and a statutory duty of confidentiality to protect patient (and indeed any personally 4

5 identifiable) information and only use it for the purposes for which it was intended. The disclosure and use of confidential patient information needs to be both lawful and ethical. 8. Information Governance Southwark CCG staff must keep up-to-date with the requirements of Information Governance and must follow organisational policies and procedures to ensure that the organisation information is dealt with legally, securely, efficiently and effectively. Staff must appropriately manage all the Information they handle during the course of their employment with Southwark CCG, making the information available for sharing in a controlled manner, subject to statutory requirements and the CCG Information Governance Policy, and formal Information Sharing arrangements. 9. Data Protection The CCG is registered as a data controller under the Data Protection Act All personal Information we hold, obtain, record, use and share as an organisation is governed by this Act. All employees have a legal responsibility for all personal information handled and must not at any time use the personal data in a way incompatible with the guidelines stipulated in this act. Any doubts regarding what should or should not be done in connection with the Data Protection Act should be discussed with immediate line manager. 10. Records Management All employees have a legal responsibility for all records worked with e.g. patient records, financial records, personal, administrative etc., that are gathered or used as part of working within the organisation. The records may be held in a variety of formats such as paper, electronic, microfiche, audio and video tapes etc. Any doubts regarding the correct management of records must be discussed with immediate line manager. 11. Health and Safety All Managers have a general accountability for ensuring, so far as is reasonably practicable, the health, safety and welfare of the employees under their direction at work. Each employee has a responsibility to take reasonable care for his or her own acts or omissions and the effect that these may have upon the safety of themselves or any other person. Every employee must use safety equipment or clothing in a proper manner and for the purpose intended. Any employee who intentionally or recklessly misuses anything supplied in the interests of health and safety will be subject to disciplinary procedures. Every employee must work in accordance with any health and safety procedures, instructions or training that has been given. No employee may undertake any task for which they have not been authorised and for which they are not adequately trained. Every employee is required to bring to the attention of a responsible person any perceived shortcoming in our safety arrangements or any defects in work equipment. All employees are under a duty to familiarise themselves with local Health and Safety Policies. 12. Risk Management The post holder will ensure the implementation of systems and procedures at a local level to fulfil the requirements of the CCG Risk Management Strategy, including local management and resolution of complaints and concerns, management of SUIs/incidents and near misses. Specific responsibility for risk management will be clarified at local induction. 13. Emergency Planning 5
